Choose an over-the-counter cold or flu medicine by matching its active ingredient to the symptom bothering you most—not by choosing the product with the broadest promise. Check the Drug Facts label and compare ingredients across every medicine you take. These products may temporarily ease symptoms; they do not cure a cold.
Match the medicine category to the symptom
Cold and flu products can contain one or several active ingredients. Start with the symptom you want to relieve, then check that the product’s ingredient and warnings fit your circumstances. Categories describe what an ingredient is intended to address; they are not a recommendation for a particular person or brand.

For a straightforward choice, compare products by symptom target, active ingredients, age suitability, label warnings and interactions, and dosage form. Treat only the symptoms you actually have: a multi-symptom product is not automatically better if it adds ingredients you do not need.
Read Drug Facts before taking a product
The U.S. Food and Drug Administration (FDA) calls the Drug Facts label the key guide to using nonprescription medicine. Check its uses, directions, warnings, interactions, side effects, pregnancy or breastfeeding information, and instructions on when to stop use or seek advice. Read the label every time: formulations and directions can vary between products.

Pay particular attention to active ingredients in combination products. A cold-and-flu medicine may already contain a pain reliever, so adding a separate product with the same ingredient can lead to an accidental overdose. FDA reports a maximum recommended adult dose of acetaminophen of 4,000 milligrams per day across all medicines. That is a safety ceiling in FDA guidance, not an individual dosing target; follow the exact product label and ask a healthcare professional about your own circumstances.
Take extra care when choosing medicine for a child
Age recommendations are not expressed as one universal cutoff. CDC says over-the-counter cough and cold medicines are not recommended for children younger than six. FDA guidance separately says not to give cough or cold products containing a decongestant or antihistamine to children under two, and notes that manufacturers voluntarily label these products “do not use in children under 4 years of age.” Follow current local guidance and ask a pediatric clinician or pharmacist if you are unsure.

FDA estimated that 1,519 children under age two were treated in U.S. emergency departments in 2004–2005 for adverse events, including overdoses, associated with cough and cold medicines. This is historical context for the need for care, not a current estimate of how often such events occur.
When to ask a healthcare professional
Ask a clinician or pharmacist to help interpret the label if the person taking the medicine is a young child, is pregnant or breastfeeding, takes prescription or other over-the-counter medicines, has a chronic condition, or has a warning or interaction they cannot reconcile. Contact a healthcare provider for severe or concerning symptoms. If flu or COVID-19 is diagnosed, antiviral treatment may be an option; symptom-relief products are not a substitute for assessment.

What cold and flu medicines can—and cannot—do
Over-the-counter medicines can provide temporary relief, but CDC says they do not cure the common cold. Choose based on the symptom you need to manage, use the label to avoid ingredient overlap, and seek professional advice when age, health, or other medicines make the choice uncertain.
