Build the schedule from the person’s current medication containers and confirmed instructions—not by choosing whichever of several old lists looks most complete. Record every prescription, over-the-counter medicine, vitamin, herb, and supplement; have a pharmacist or prescriber resolve conflicts; then keep one dated copy that caregivers can update and share.

Gather the bottles, lists, and instructions first

Before formatting a schedule, collect the sources that show what the person has and what they actually take. Ask the person and caregivers to gather prescription and nonprescription medicine containers, vitamins, herbs, supplements, pharmacy printouts, discharge paperwork, and existing notes. Bring the bottles to an appointment when practical, or take a complete current list. MedlinePlus recommends discussing medication management with providers and bringing medicine bottles or a list: Caregiving – medication management.

Include medicines used only occasionally as well as those taken routinely. Note allergies and any medicine that previously caused a problem, along with emergency contact details. What to put on the medication list

Make one inventory entry per medicine or supplement. Use the label for the product name and strength, and record the directions as written. If a reason for use is not clear, leave it for the pharmacist or prescriber to confirm rather than guessing.

  • Strength shown on the container.
  • What it is for, as documented or confirmed by a clinician.
  • How much to take, how to take it, and when.
  • Special instructions, such as taking with food or remaining upright.
  • Whether it is taken every day, on a regular schedule but not every day, or only as needed.
  • A notes field for questions or instructions that need confirmation.

The Agency for Healthcare Research and Quality’s My Medicines List provides fields for medication details and separates daily, regularly scheduled non-daily, and as-needed medicines. The National Institute on Aging also offers caregiver worksheets for recording prescriptions, over-the-counter products, and supplements to share with caregivers and health professionals.

Organize confirmed directions by time of day

For medicines taken daily, use time-of-day columns such as morning, noon, evening, and bedtime. Put a medicine in a time slot only when the current directions support that timing. Preserve the prescribed amount and any food, posture, or other special instruction; do not convert a vague direction into a more precise schedule yourself. AHRQ’s list format uses time-of-day columns and can retain additional directions alongside each entry.

Keep regularly scheduled medicines that are not taken every day in a separate section. Record the confirmed days or pattern exactly rather than making them appear to be daily medicines.

Give as-needed medicines their own section instead of placing them in a routine time slot. Include the situation or symptom they are for, the confirmed amount, and any confirmed timing limits. If any part of those directions is unclear, ask a pharmacist or prescriber before filling it in.

What to do when medication lists disagree

A bottle label, hospital discharge sheet, provider list, pharmacy printout, and the person’s actual routine may not match. Do not resolve the mismatch by picking one version, and do not independently change or stop a medicine. Mark the conflicting details and contact the pharmacist or prescriber to confirm what the person should take. MedlinePlus specifically advises caregivers not to reduce or stop medicines on their own: Caregiving – medication management.

Bring the bottles or full inventory to the conversation. Ask which medicines are current, what each is for, the dose and timing, which ones are for symptoms only, and what to do if a dose is missed or a side effect occurs. If instructions remain uncertain, leave that part unresolved on the working list until a qualified professional clarifies it. Choose a format caregivers can keep current

A paper sheet, phone image, app, or provider template can all serve as the working copy. Choose based on what the household can read, update, access at appointments or in an emergency, and share with the people involved in care. FDA guidance recognizes several ways to keep and share a medication list; the useful format is the one caregivers can maintain.

Put the creation or last-review date at the top. Update the list whenever a medicine starts, stops, or changes, and replace old copies so nobody relies on stale directions. AHRQ puts it plainly: “Remember to update your new My Medicines List every time you start, stop, or change a medicine.” Share the current copy with trusted caregivers and health professionals, and keep it accessible at home and when traveling or going to appointments.

Use reminders and pill organizers only as aids

Checkboxes, alarms, phone notifications, or linking a dose to an established routine can help caregivers follow a confirmed schedule. A weekly pill organizer may also help some routines, but it cannot determine the correct medicine or dose. MedlinePlus advises keeping medicines in their original bottles and choosing an organizer with a suitable number of days and compartments: Keeping your medicines organized. Ask a pharmacist whether an organizer is appropriate when a medicine has handling or packaging instructions.

Keep the verified schedule as the reference, and use any organizer or reminder to support—not replace—it. If the plan changes, update the written or digital list and the practical aid from the confirmed instructions.
