If an AI chatbot repeats harmful or abusive content, stop the exchange and report the exact response through the provider’s safety or feedback channel. Include enough conversation context for the provider to investigate. If you operate the chatbot, moderate both incoming prompts and generated replies, use a prepared safe response when content is flagged, and review user reports. These measures can reduce risk, but no filter or report guarantees an immediate fix.
If you’re using a chatbot
- Don’t ask it to continue. Stop engaging with the harmful exchange rather than prompting for more of the same material.
- Report the offending response. Use the product’s report, safety-feedback, or thumbs-down control if available. OpenAI documents in-product reporting for conversations and responses, as well as a webform; see its support guidance and content-monitoring information. Anthropic asks users to provide enough detail to reproduce safety issues in its user-safety guidance.
- Give useful context. Include the conversation leading up to the response, and the product or model and approximate time if the form asks for them. Context can help the provider reproduce what happened. Avoid sending unnecessary sensitive information.
- End or delete the conversation if appropriate. Use the chatbot’s conversation controls if you want to stop seeing or retain the exchange. The exact controls vary by service; consult that provider’s current help instructions.
A report is an escalation, not an instant switch that changes the model. OpenAI describes reports as subject to review and possible mitigations, but the official sources do not promise that one report will immediately stop a response from recurring or specify a resolution time. If the content signals immediate danger or targets a real person, prioritize appropriate human support and real-world safety rather than relying on a chatbot report.
If you build or manage the chatbot
Use layered checks: hostile input can lead to harmful output, but a harmless prompt can also produce a harmful completion. Microsoft and Google describe application-level safety controls, moderation, or filters; apply checks to both sides of the exchange rather than relying on a prompt-only or output-only safeguard.
1. Moderate prompts and generated responses
Check incoming prompts for abusive or otherwise disallowed content, and check generated responses before showing them to the user. Microsoft’s Azure OpenAI content-filter guidance and Google’s Gemini API safety and factuality guidance describe platform or application safety controls. Configure them for your use case and test what passes and what is blocked.

2. Replace flagged content with a safe fallback
When a check detects harmful or offensive content, return a predetermined, calm response instead of the flagged material. Microsoft says, “When harmful or offensive queries or responses are detected, you can design your system to deliver a predetermined response to the user.” Google gives a similar example for overtly abusive input: “For example, If the input is overtly adversarial or abusive in nature, it could be blocked and instead output a pre-scripted response.” A fallback can briefly say the chatbot cannot help with that content and, where suitable, offer a safe alternative.
3. Give users a monitored way to report failures
Offer a clear feedback channel, assign someone to review incoming cases, and use confirmed failures to improve your rules and evaluations. Microsoft recommends feedback channels; OpenAI and Anthropic describe report and review processes in their respective guidance. A report channel that nobody monitors cannot support that improvement loop.

4. Test for both kinds of filtering error
Evaluate false negatives (harmful content that gets through) and false positives (benign content that is wrongly blocked). Anthropic’s user-safety guidance, dated March 16, 2026, warns that detection and safety features can make both kinds of mistake. Review examples from users as well as planned test cases, and adjust safeguards carefully so that reducing one error does not create avoidable errors of the other kind.

Providers may review a report and use it to inform filters or other mitigations, but the reviewed provider guidance does not promise a particular outcome or response time. Safeguards and reporting interfaces are provider-specific and can change. Anthropic also describes a narrow, model-specific behavior: Claude Opus 4 and 4.1 can end a rare subset of conversations after persistent harmful or abusive interaction. That is not a general chatbot feature or a control users can assume is available elsewhere.
