If you want another game after Fallout 4 or Fallout: New Vegas, start with what you want to keep: Fallout’s wasteland setting, New Vegas’s Obsidian connection and faction-led role-playing, or the freedom to explore a large RPG world. No single alternative offers all three. The Outer Worlds is the closest fit for Obsidian-style sci-fi; Skyrim and Cyberpunk 2077 trade the wasteland for different open-world settings; and Fallout 76 keeps you in the series but adds multiplayer.
Choose by what you want from Fallout
Fallout 4 is an open-world RPG set in the post-nuclear Commonwealth, as Bethesda describes it on its Fallout 4 page. Fallout: New Vegas has a direct connection to Obsidian, which presents the game on its official game page. Those are different reasons to look for a follow-up: one points toward exploring a recognizable wasteland, the other toward Obsidian’s sci-fi RPG lineage and faction-centered role-playing.

If New Vegas is the Fallout game you most want to recapture, The Outer Worlds is the most direct recommendation here. Obsidian’s official site connects the studio to Fallout: New Vegas, and a GameSpot roundup of games like Fallout 4 also connects Obsidian with The Outer Worlds.

Skyrim: trade the wasteland for fantasy
If what you miss most is roaming through a large RPG world and deciding where to go next, Skyrim is a Bethesda fantasy alternative. GameSpot’s roundup highlights its Bethesda RPG feel. The major tradeoff is the setting: expect fantasy rather than post-nuclear America, guns, and wasteland survival.

Cyberpunk 2077: trade the wasteland for a futuristic city
Cyberpunk 2077 is a better fit if you want a detailed world to move through but are ready for a different tone and place. GameSpot describes its world as interactive and content-filled. That recommendation is about the appeal of exploring a richly populated setting, not a claim that it reproduces Fallout’s structure or role-playing.

Bethesda describes Fallout 76 as the franchise’s multiplayer addition on its Fallout 76 page. It is the option to consider if being back in Fallout matters more than preserving a purely single-player experience. Multiplayer is the key distinction to weigh before choosing it; the fact that it shares the franchise does not make it the same kind of follow-up as another single-player RPG.
