Measure the offset for your setup rather than using a universal preset, then verify the result in a new recording and, if possible, a controlled live test.
Load the pre-recorded video in OBS

In your scene, add a Media Source and select the local video file. OBS documents support for common video formats including MP4, TS, MOV, FLV, MKV, AVI, GIF, and WebM, as well as audio formats such as MP3, AAC, OGG, and WAV. Media Sources are available on Windows, macOS, and Linux. See the OBS Media Sources guide.

Check the source’s playback options. Restart playback when source becomes active restarts the file when its source is in the current scene and visible. Loop starts the file again after it finishes. Set these to suit whether the clip should play once or repeat.

Confirm which source provides the audio you hear. If the Media Source contains the sound, adjust that source’s audio timing; do not accidentally change an unrelated microphone or desktop-audio source.

Measure the sync error before changing a value
Make a short OBS recording that includes a clear sound and matching visible event, such as a clap or a sharp action in the video. Review it in a media player or editor and determine whether the sound happens before or after the matching picture. OBS community guidance recommends recording and reviewing the output when diagnosing sync.
This test is a way to diagnose your particular setup, not a published universal calibration value. The needed adjustment depends on the file and signal path; a value reported by another user, including a reported 200 ms, is not a general recommendation.
Adjust the audio Sync Offset

In OBS, open Edit → Advanced Audio Properties.

Find the row for the audio source that is actually heard and edit its Sync Offset (ms) value. The control is measured in milliseconds. OBS community guidance identifies it as the audio-delay control; users report that positive values delay audio and negative values advance it. Confirm the direction with a short test on your installed OBS release and source type.

If the audio is early, try a positive offset to delay it. Change the value in small, measured increments based on what you observed, then make and review another recording before adjusting again.

If the audio is already late, adding more audio delay moves it further out of sync. Investigate whether a video delay/filter or another part of the playback or capture path needs attention instead. Community troubleshooting discusses both audio delay for a constant offset and a video-source filter as possible adjustment points.
Tell a fixed offset from audio drift
A fixed offset means the audio is early or late by roughly the same amount throughout the clip; a Sync Offset can compensate for that stable difference. If the picture and sound start close together but drift farther apart as playback continues, a single fixed delay is unlikely to solve the underlying timing problem. Check whether the mismatch occurs in the OBS recording, only in the outgoing stream, or only for downstream playback, and inspect the source or device timing and monitoring path.

For Windows Audio Input/Output Capture sources, OBS documents a Use Device Timestamps option intended to help prevent desynchronization. It is not documented as a universal fix for audio embedded in a Media Source. See the OBS Audio Sources guide.
Check separate audio capture devices
If you also use an Audio Input/Output Capture source, check whether that same device is captured globally under Settings → Audio. OBS warns that capturing the same device both globally and as a scene source can cause echo. Keep the sources distinct while diagnosing timing so you can identify which one supplies the sound.

Verify the result on the path viewers will receive
After changing the offset, make a fresh local recording and inspect the same clear audio/visual event. A local recording can show whether OBS’s recorded output is aligned, but it cannot establish by itself what viewers receive through YouTube Live. Where practical, verify with a private or otherwise controlled live test as well. The available documentation does not establish a YouTube-specific timing guarantee or diagnostic procedure for a particular stream.

Troubleshooting
The offset seems to have no effect

Confirm that you changed the row for the source whose audio is actually audible, then record and review a fresh test.

Check that the file’s playback behavior and scene visibility are as intended, especially if the source is set to restart when active or loop.

A single GitHub issue opened on 2024-12-22 reports offset behavior changing after opening Advanced Audio Properties in one macOS 14 / OBS 30 and 31 setup; a later commenter described a workaround in a different setup. This is one version-specific report, not evidence of a general defect or an official fix. Check current OBS releases and include an OBS log if you ask for support. See OBS Studio issue #11656.

Audio and video separate further as the clip plays

Do not keep increasing a fixed offset to chase a growing mismatch. Compare the start and end of a recording and check whether the drift is already present in OBS’s local output.

If a separate capture device is involved, review its timing options and make sure the same device is not being captured twice. The device timestamp option documented for Windows capture sources does not guarantee a fix for embedded Media Source audio.
The recording is synced but the live stream is not
Identify whether the error is in the OBS recording, in the stream received by YouTube, or only in a viewer’s playback. A local OBS recording alone cannot confirm sync for every viewer; check the live path in a controlled test before relying on it.
