Check which concat method the command uses, whether the playlist contains valid file entries and paths, and whether the inputs have compatible streams, codecs, and time bases. Without the command, playlist, and log, there is no way to identify one definite cause.
First, check how the command joins files
FFmpeg has three concat approaches, and they are not interchangeable. The right choice depends on whether you are joining files as-is, re-encoding them, or using a container that supports file-level concatenation.

For the concat demuxer, FFmpeg reads a text script containing one file directive per input. The official concat demuxer documentation describes its input requirements; FFmpeg’s FAQ on concatenating video files distinguishes the demuxer, filter, and protocol approaches.
Check the concat playlist
A basic demuxer list can look like this:
ffconcat version 1.0
file 'story-1.mp4'
file 'story-2.mp4'
file 'story-3.mp4'Use one file line for every story, in the order you want them played. For automatic format recognition, ffconcat version 1.0 must be the very first line of the script. Check for a blank line or other text before it. Quote or escape filenames that contain spaces or special characters, following the official concat script syntax.

Check whether FFmpeg can access every path
The concat demuxer defaults to safe=1. In that mode it accepts relative paths made from a limited portable character set, so an absolute path or a filename with disallowed characters can be rejected. For a playlist you trust, -safe 0 permits absolute paths and other path forms; inspect every entry first, because the option relaxes path checks.
For example, the input option is commonly placed before the playlist input:

ffmpeg -f concat -safe 0 -i stories.txt -c copy output.mp4This example uses stream copy; it is appropriate only when the inputs satisfy the concat demuxer’s compatibility requirements. Consult the concat demuxer documentation for the safe-path behavior and supported options.
Compare the story files’ streams and timing
The concat demuxer expects the files to have the same streams, codecs, and time base. If one story has a different audio or video stream, codec, or time base, the virtual input may be invalid or may produce playback problems. Check that each file has the same relevant stream layout before trying stream copy.

Durations matter, too: the demuxer adjusts timestamps between files using file durations. If a file’s stored duration is missing or incorrect, timestamps at a boundary can be wrong. The official documentation explains the demuxer’s compatibility and timestamp behavior. If the files cannot be made compatible for stream copy, use a concat-filter workflow that re-encodes them rather than treating the concat protocol as a playlist.
Collect the evidence needed to diagnose this playlist
If the checks above do not explain the stop, a useful diagnosis needs these details:

- The exact FFmpeg command, including all options and their order.
- The complete playlist text, with sensitive local details removed if necessary.
- The FFmpeg version.
- The formats and stream details of the input files.
- The full error message or log output from the run.
Without those details, it is not possible to tell whether FFmpeg is using the wrong concat method, rejecting a playlist entry or path, encountering incompatible inputs, or reporting a different error.
