You can feel perfectly well and still have high LDL cholesterol: it often causes no symptoms. A lipid panel—a blood test—shows cholesterol levels; how often you need one and what the results mean depend on your health and risk factors. The American Heart Association (AHA) says many people do not have symptoms of high cholesterol (AHA patient guidance).

Myth 1: If I feel fine, my LDL must be fine

High LDL cholesterol usually does not announce itself with a reliable feeling or symptom. That means feeling energetic, exercising regularly, or having no obvious health problems cannot tell you whether your LDL is in a healthy range. A blood test is needed to measure it.

This is not a reason to assume that a high result means an immediate heart attack or stroke. LDL is one part of cardiovascular risk, and a clinician interprets it alongside other information.

Myth 2: If my clinician has not brought it up, I do not need to ask

Cholesterol can go unmeasured unless it is checked. The AHA encourages adults to know their cholesterol levels and work with a healthcare professional on follow-up. Ask whether a lipid panel is appropriate for you and when to repeat it; the right timing depends on your health and risk, rather than a single schedule for everyone (AHA cholesterol guidance).

A lipid panel reports blood lipid measurements, including LDL and other cholesterol measures. Bring the full result to a clinician rather than trying to judge your heart health from how you feel or from one number in isolation.

Myth 3: Only weight, diet, and exercise determine LDL

Diet and physical activity matter, but they are not the whole explanation. Age, overall health, family history, diabetes, prior heart attack or stroke, and inherited conditions can affect cholesterol and the decisions made about it. Being thin does not rule out high LDL.

If high cholesterol runs in your family, or you have a relevant health condition or cardiovascular history, mention it when discussing testing. Those details help put a result in context (AHA guidance; CDC cholesterol information).

Myth 4: Cholesterol is only a concern for men or older adults

Cholesterol matters across groups and life stages. The AHA says both men and women need to pay attention; levels tend to rise with age, and some life stages can affect women’s levels. Do not assume age or sex alone tells you whether testing or follow-up is needed (AHA cholesterol myths).

Myth 5: Everyone needs the same diet, tests, or medication decision

There is no single LDL result or lifestyle prescription that determines the right plan for every person. The 2026 AHA/ACC dyslipidemia guideline replaces the 2018 blood cholesterol guideline and frames prevention around a broader, individualized assessment. Its recommendations incorporate clinical evidence through late 2024, according to the guideline summary (AHA/ACC guideline summary).

What clinicians may consider beyond LDL

  • Overall risk: For eligible adults without established cardiovascular disease, the guideline promotes PREVENT-ASCVD risk assessment as part of primary-prevention decisions.
  • Other blood markers: It highlights measuring lipoprotein(a), or Lp(a), at least once in a lifetime and selective use of apolipoprotein B (ApoB) testing.
  • Imaging in relevant cases: Coronary artery calcium (CAC) assessment may help clarify risk for some people; it is not a test every reader needs.
  • Personal context: LDL and non-HDL cholesterol goals, medical and family history, age, and whether cardiovascular disease is already established can all affect treatment discussions.

These tools are not a checklist to order on your own. Ask a healthcare professional which, if any, apply to your situation. Medication decisions should reflect individual risk and circumstances, not the assumption that everybody with a particular lifestyle—or everybody who feels healthy—needs the same treatment (2026 guideline summary; AHA guideline announcement).

Heart-healthy eating is a pattern, not a single-ingredient rule

AHA dietary guidance published March 31, 2026 emphasizes vegetables and fruit, whole grains, healthy protein sources, unsaturated fats in place of saturated fats, and minimally processed foods, while limiting added sugars and sodium. It says dietary cholesterol is no longer a primary cardiovascular risk-reduction target for most people. That does not make diet irrelevant; it is a reason to focus on the overall pattern rather than treating one nutrient as the entire answer (AHA dietary guidance).

What to do if you feel well but do not know your LDL

  1. Ask about a lipid panel. Tell your healthcare professional when you last had cholesterol checked, if you know, and ask whether testing is appropriate now.
  2. Share relevant context. Mention family history, diabetes, prior heart attack or stroke, and other health conditions or medicines that may matter.
  3. Review the whole result. Ask what the LDL and other lipid measurements mean in light of your overall risk, and whether additional assessment is useful.
  4. Agree on follow-up. Ask when to recheck and what lifestyle changes or treatment options make sense for you.

The AHA’s 2026 announcement estimates that one in four U.S. adults has high LDL-C (AHA newsroom, March 13, 2026). It is a population estimate, not a prediction about any one person. A test and a personalized discussion—not subjective wellness—are how to learn where you stand.
