Iowa plans to move public-health funding and oversight into seven districts, and Polk County has applied to lead Region 5. Polk County Health Department Director Juliann Van Liew estimates that administrative costs could leave the region with a roughly $300,000 funding gap. That is a projected funding pressure—not a confirmed reduction in vaccine clinics, school audits, staffing, or doses.
What Iowa’s regionalization plan would change
Iowa plans to group its 99 counties into seven public-health districts. A lead agency in each district would administer state funds for the other county health departments in that region. Polk County has applied to serve as the lead agency for Region 5, according to Iowa Public Radio’s September 30, 2026 report.
The state’s stated aim is better coordination and more consistent access to essential services, regardless of where Iowans live. Under the reported proposal, county departments would retain authority over their staff and services and work with local providers on district service plans. Regional administration therefore would not, by itself, mean that Polk County loses control of its local programs.
Why Polk County officials warn about vaccination services
Van Liew told Iowa Public Radio that Polk County receives about $2 million in grants supporting public-health work, including immunizations and childhood lead testing. She estimated the lead agency could need to use around 15% of those funds for administration, leaving Region 5 with an estimated $300,000 gap. She described 15% as a cap and said the amount could vary by region.

Those figures are estimates attributed to Van Liew, not a finalized Region 5 budget or a vaccine-program cut. The concern is that if administrative costs come from the same grant pool used to support services, fewer dollars could remain for local work. Van Liew warned: “The fact is that less local capacity from less dollars means fewer services for Iowa residents.”
What the estimates do not establish
- They do not show that Polk County vaccination clinics have closed or reduced hours.
- They do not establish that school immunization audits have stopped or that student vaccinations have been reduced.
- They do not quantify vaccine doses that could be lost.
- They do not settle the final administrative allocation or how any funding pressure would be distributed across Region 5.
The school-audit and student-vaccination concerns appeared in a Des Moines Register report surfaced through a Bluewater Healthy Living republication, but that report could not be opened for verification. Iowa Public Radio’s reporting supports the broader concern about funding and immunization capacity; it does not confirm a specific change to audits or school vaccinations.

What Polk County’s current childhood-vaccine service includes
Polk County’s childhood-vaccine service page says the health department provides recommended childhood immunizations to uninsured or underinsured families and participates in the federal Vaccines for Children program. The page directs families to call for appointments. Nothing in the reporting establishes that regionalization has changed eligibility, appointment procedures, or vaccine offerings.
Why local officials say one regional plan may not fit every county
Regional coordination could make services more consistent across county lines, but local health departments serve communities with different needs. Wayne County Public Health Director Susan Moore said, “The services that are needed in Polk County just are not the same as Wayne County,” and raised concern that needed local services might not be available.

Van Liew has argued for consistency in the system while keeping decisions about implementation local: “So, making sure that there’s consistency across the system, but the methods for how we implement a strategy or what that looks like — those should be different, and those should be designed and driven by those local health department leaders.” The policy question is whether regional administration can coordinate funding without weakening local capacity to choose services suited to their communities.

Iowa Public Radio reported that an initial phase was planned for January, followed by implementation of the regional system in July 2028, when oversight would shift to each region’s lead agency. The report does not provide final Region 5 grant allocations, the completed district service plan, or a vaccination-specific decision about clinic schedules, staffing, school audits, or student vaccinations. Those details are needed to determine whether the projected funding pressure would affect particular services.
