Before adopting agentic AI, define the outcome it must achieve and decide exactly which actions it may take without human approval. Then verify its access, governance, safety evidence and ongoing oversight. Treat these five questions as decision gates: if you cannot answer one clearly, limit the pilot or delay deployment.
1. What outcome justifies this agent, and how much autonomy does it need?
Start with a specific business or public-service result—not a general desire to “use AI.” Identify the process, the people affected, the current baseline and a measurable threshold for success. Without those, it is difficult to tell whether an agent is improving the work or merely adding a new system to manage.
Next, set an autonomy boundary. Divide possible actions into three categories: actions the agent may take automatically, actions that require human approval, and actions it must never take. For example, an agent might gather information and draft a response automatically, but need approval before sending it or changing a customer record. The right boundary depends on the consequences of an error, the reversibility of an action and the sensitivity of the system or data involved.
CISA and partner agencies recommend limiting agent autonomy and avoiding broad or unrestricted access, especially for sensitive data and critical systems. Their secure-by-design guidance is a useful starting point for making the boundary explicit.

2. What data, tools, identities and permissions will it use?
An agent’s effective capabilities are determined not just by its model, but by the data it can retrieve, the tools it can call and the identities or credentials available to it. Map those connections before the pilot begins, including data stores, APIs, plugins, memory, credentials and downstream systems.
- Use least privilege: grant only the access needed for the defined task, and scope it to the smallest practical data set and action set.
- Give agents distinct identities: do not rely on an employee’s broad user account to represent an agent. Separate identities make it easier to authorize, audit, disable and revoke agent activity.
- Scope credentials and actions: use limited tokens and explicit authorization for each action, rather than persistent, unrestricted credentials.
- Account for stored context: understand what the agent retains in memory, where it is stored and which connected systems can receive it.
- Inspect every tool path: document what each API, plugin or integration can read or change, including indirect effects in downstream systems.
NIST’s work on agent identity and authorization warns that weak identity, authorization and governance can contribute to data leaks, compliance failures, prompt injection and unpredictable autonomous behavior. 3. What governance and risk process will control adoption?
Name an accountable owner before the agent is connected to operational systems. That person should coordinate a documented risk assessment, affected-party analysis, privacy and security reviews, escalation rules and an incident process. Specify who can approve a change in scope, investigate a problem and stop the system.
Governance should follow the agent through design, development, use and evaluation—not end at procurement or launch. NIST’s AI Risk Management Framework is a voluntary framework for incorporating trustworthiness into those activities. It was released on January 26, 2023; the Generative AI Profile, NIST-AI-600-1, followed on July 26, 2024. For a pilot or vendor shortlist, ask each candidate to show how it supports these controls rather than relying on a general claim that it is “secure” or “responsible.” Compare options across the following areas:

| Monitoring and incident response | How issues are detected, escalated and handled. |
| Interoperability and vendor changes | How dependencies work and how changes to models, tools or service terms are communicated. |
| Operating cost | The expected cost of running, monitoring and maintaining the deployment. |
4. How will you test safety, security and reliability before launch?
Do not treat a strong benchmark score as sufficient evidence that an agent is safe for a particular workflow. Test it on realistic tasks and on the ways it could fail in its actual environment. Include scenarios involving prompt injection, unauthorized tool use, data leakage, privilege escalation, unsafe plans and unavailable or degraded dependencies.

Use a combination of methods appropriate to the system and its consequences: model testing, red-teaming and field testing. NIST’s ARIA program explicitly uses those three evaluation levels and examines technical and contextual robustness beyond simple accuracy. Its ARIA program information can help frame the evidence to request.
Before launch, document what was tested, the results, unresolved risks and the conditions under which the agent must stop or escalate. Make sure tests cover the permissions and integrations the agent will actually receive; a test in a restricted sandbox does not establish how the same agent will behave with production access.

5. Who is accountable after deployment, and how will controls adapt?
Assign a human owner who has the authority to pause the agent, revoke its credentials or remove a tool connection. Define how that owner will review activity and respond to errors, suspected misuse or unexpected actions.
Monitor the agent’s actions, permissions, outputs, incidents, drift and vendor changes. Review logs and reassess risk on a schedule, and whenever the model, connected tools, data access or operating context changes. CISA calls for continuous monitoring and regular security assessments in its secure-by-design guidance; NIST’s agent work also emphasizes identity, authorization, interoperability and security evaluation.
As part of operational planning, establish who receives an alert, how quickly access can be revoked, what evidence must be preserved and how the affected workflow will continue while the agent is disabled. These decisions turn accountability into a working control rather than a name on a policy document.
