A check stub—also called a pay stub or pay statement—shows how an employer calculated a payment: the work period, earnings, deductions, and net pay. Use a template to identify and organize those figures, not as proof that the figures or tax withholding are correct. Field names and legal disclosure requirements vary by payroll system and location.
What a check stub tells you
A check stub connects the pay you earned to the amount paid after listed deductions. To review it, first confirm the dates, then compare earnings and deductions with the totals. The IRS explains that current-period and year-to-date columns help employees track and reconcile withholding during the year (IRS guidance on pay statements).
What each field and section means
Pay period and payment date
The pay period begin and end dates identify the work span covered by the payment. The payment date, sometimes called the check date, is when the payment is issued; it may fall after the work period ends. The University of Oklahoma’s guide distinguishes the dates for the period being paid from the payday (University of Oklahoma paycheck-stub guide).
Employee and employer information
Names and identifying details associate the statement with the worker and employer. Templates may include addresses and other identifiers, but the exact fields depend on the payroll system and applicable rules. Avoid sharing a stub that exposes sensitive information such as a full Social Security number; mask it in examples and when sharing a copy unless a recipient specifically needs it.

Tax status and withholding information
A statement may show a filing status or other withholding information drawn from the employee’s Form W-4. Federal income-tax withholding depends on the employee’s current W-4 information and the employer’s calculation method. The IRS publishes year-specific methods in Publication 15-T. An older template’s “allowances” field should not be treated as a description of the current W-4 process, and a stub template is not a withholding calculator.
Hours, rates, and earnings
Hourly pay statements may break earnings into regular hours, overtime, bonuses, or other pay types, with hours and rates shown for each. Check that the listed categories make it possible to understand how the earnings total was reached. For covered, nonexempt workers, federal wage-record rules identify hours worked, pay basis and rate, straight-time earnings, and overtime earnings among the records employers must maintain. Some state requirements are more specific: Maryland, for example, says that where multiple rates apply, the rates and hours at each rate—including straight-time and overtime—must be reported under its law.

Gross pay
Gross pay is earnings before the deductions displayed on the statement. It may combine several earnings categories. Do not assume every payment is treated identically for every tax purpose; compare the categories and totals shown rather than inferring tax treatment from the gross-pay figure alone.
Deductions and withholdings
Deductions reduce the amount paid. Common lines include federal income-tax withholding, Social Security, Medicare, state withholding, insurance premiums, and other deductions, depending on the worker’s circumstances and employer benefits. A useful stub labels each item rather than combining unrelated amounts into one unexplained total. The IRS’s sample payroll ledger illustrates these categories (IRS sample payroll ledger).

Net pay
Net pay is the amount remaining after the deductions shown. Reconcile it against gross pay and the listed deductions: if the figures do not add up, ask payroll to explain the difference. A matching arithmetic total does not by itself establish that the underlying earnings, deductions, or tax withholding are correct.
Current period and year to date
Current-period figures describe this payment. Year-to-date (YTD) figures accumulate from the start of the year through the current payment. They answer different questions: a current-period amount helps explain this check, while a YTD amount helps track totals over the year. Do not compare a current-period figure with a YTD figure as if they covered the same span.

Leave balances and messages
A template may include leave balances or a space for messages, but these are not universal pay-stub fields. Maryland’s pay-statement FAQ says its pay-stub law does not require leave balances; a separate state sick-and-safe-leave rule may require a written balance statement. Check the rules that apply where the employee works before treating an optional template section as a legal requirement (Maryland pay-stub FAQ).
How to use a free check stub template
- Choose a layout that fits the pay arrangement. For hourly employees, look for separate hours, rates, and earnings by type; for any worker, make sure deductions and totals have clear labels.
- Enter the period and payment date separately. The dates worked and the date paid are not interchangeable.
- Keep earnings and deductions itemized. A single combined deduction amount makes it harder to understand the calculation.
- Use current-period and YTD columns only when you can keep them accurate. YTD amounts must accumulate consistently across payments.
- Protect personal information. Limit access to the finished statement and mask sensitive identifiers in public examples.
- Verify the applicable rules before using the form for payroll compliance. A sample layout is not a universal legal form.

Does federal law require this exact pay stub?
No single federal pay-stub layout is prescribed by the FLSA. The U.S. Department of Labor says: “There is no required form for the records, but the records must include accurate information about the employee and data about the hours worked and the wages earned.” That statement concerns recordkeeping by FLSA-covered employers for covered nonexempt workers—not a universal federal template mandate. The records listed by DOL include identifying information, hours, wage basis and rate, straight-time and overtime earnings, additions or deductions, total pay per period, payment date, and period covered (DOL Recordkeeping and Reporting guidance).
State pay-statement laws can impose separate disclosure duties. Maryland’s FAQ, for example, describes a voluntary template that satisfies that state’s stated requirements when properly completed; that does not establish the rules for other states. This is a U.S.-focused overview, not a survey of every state or territory. Employers should check the relevant state labor agency. DOL also notes that tax and W-2 matters fall under IRS authority.
How to choose between check stub templates
- Jurisdiction: Is the document intended as an employer’s internal wage record or as a statement provided to a worker? Which state or local requirements apply?
- Required versus optional fields: Does the template distinguish legally required disclosures from conveniences such as messages or leave balances?
- Period columns: Can it display current-period and YTD amounts clearly without mixing them?
- Pay detail: Can it show hours, multiple rates, overtime, and other earnings separately where relevant?
- Privacy: Can sensitive identifiers be omitted or masked in copies that do not need them?
Federal and state rules differ, and requirements can change. Use the template as a layout aid, then verify current requirements for the worker’s location and pay situation. For individual withholding questions, consult current IRS guidance or a qualified tax professional.
