“Add a screenshot to a PDF form” can mean three different jobs: place a fixed image on an existing page, let a respondent supply an image, or insert the screenshot in Word (or another source file) before exporting to PDF. The correct steps depend on which result you need. Word documents support a documented Picture Content Control; Acrobat supports creating and editing PDF form fields. Neither workflow, by itself, guarantees a universally compatible image-upload field in every PDF viewer.
Choose the workflow before you edit anything

Workflow 1: add a fixed screenshot in Word, then export
This is the most predictable route when you are authoring the form rather than modifying a finished PDF.
- Open the Word document and click where the screenshot should appear.
- Choose Insert > Pictures, then select the image from your computer. On a phone, Word can insert a picture from the image library or take one with the camera; Microsoft’s mobile guidance also explains adding alt text (Microsoft Support).
- Set the image layout to In Line with Text. Microsoft identifies this arrangement as the reliable choice for screen-reader reading in the documented Word workflow.
- Resize from a corner so the screenshot remains proportional. Keep labels and answer areas visible.
- Right-click the image, choose View Alt Text (or the equivalent accessibility command), and write concise text describing the screenshot’s purpose or evidence.
- Save the source document, then use Word’s PDF export command. Open the resulting PDF and inspect it at normal viewing size.
When the respondent must provide the image
In Word desktop, enable the Developer tab, place the cursor at the image location, and choose Developer > Picture Content Control. Microsoft documents this control for forms users can complete or print (Microsoft Support). Treat the PDF conversion as a separate compatibility question: the cited instructions do not establish that the control remains an interactive image-upload area after export. If image submission is essential, test the exported file in the exact viewer and workflow your recipients will use, or collect the image through a separate web form.
Workflow 2: prepare an existing PDF form in Acrobat
Use this route when your starting file is a scan, a PDF, or a document created in Word, Excel, or InDesign.

- Open the file in Acrobat and select All tools > Prepare a form.
- Choose the existing file or scan. Acrobat creates a form and attempts to identify fields automatically.
- Review every detected field. Resize, move, delete, or add components as needed.
- For respondent-entered text, use text fields with clear labels. For an image supplied by a respondent, confirm that the field type and your recipients’ PDF viewer actually support the intended behavior; do not assume that any generic field accepts an image.
- Save a working copy, then complete the form yourself using keyboard navigation and a mouse. Reopen the saved copy to verify that entries persist.
Adobe’s documented conversion workflow is described in Convert documents and scans into fillable PDF forms in Acrobat (page last updated September 23, 2025). Adobe Experience League also describes fillable-form creation from scanned paper and office documents (Create fillable forms, updated June 28, 2026). Some accessibility features are edition-dependent; Adobe notes that certain features require Acrobat Pro, Premium, or Studio (Create Accessible Forms, updated April 23, 2026).
Workflow 3: place a screenshot on a PDF that already exists
A fixed screenshot is artwork, not automatically a form field. The Adobe pages cited above document form preparation and field editing, but they do not provide a precise, universal sequence for placing a static screenshot directly onto an existing PDF page. In practice, use a PDF editor that explicitly offers image insertion, or return to the original Word, InDesign, or other source file, insert the image there, and export a new PDF. Keep the original PDF unchanged until you have checked the replacement.
Checks after placement
- At 100% viewing size, text in the screenshot is readable.
- The image does not cover captions, labels, fields, or signature areas.
- Aspect ratio is preserved and the page remains within its intended margins.
- The PDF opens in more than one viewer if recipients use different software.
- Printing does not clip the image or reduce critical details.
Make the form accessible and usable
Accessibility is part of the form’s design, not a final cosmetic step.

- Describe informative images: use concise alt text that tells a screen-reader user why the screenshot is present.
- Label fields: give each field a descriptive tooltip, not merely “Text field 1.”
- Tag the document: ensure fields and surrounding content have an understandable structure.
- Repair tab order: keyboard focus should move through the form in the same order a sighted reader sees it.
- Provide visible instructions: state accepted image format, maximum dimensions or size, and whether the image is optional.
WebAIM’s Acrobat guidance identifies four core tasks: add appropriate fields, give each a descriptive tooltip, tag the fields, and check or repair tab order (WebAIM: PDF Accessibility: Accessible Forms in Acrobat). Adobe’s accessibility guidance also recommends addressing alternate text and document structure in the source authoring application (Creating accessible PDFs in Adobe Acrobat).

Troubleshooting
The image is blurry
Use the original screenshot rather than a thumbnail, capture at a larger viewport or retina scale, and avoid enlarging a low-resolution image inside Word or Acrobat.
The screenshot covers form controls
Move it into its own labeled area, resize proportionally, or place it on a separate page. Recheck at 100% and in print preview.
The PDF field will not accept an image
A standard text field is not an image-upload control. Recheck the field type, viewer support, and recipient workflow. If compatibility is uncertain, collect the image separately and reference its filename or URL in the PDF.

Add descriptive tooltips, tag the fields, and repair tab order, then test with keyboard-only navigation and a screen reader.
The Word control disappeared after PDF export
That behavior is not resolved by the Picture Content Control instructions. Verify the exported file in the target viewer; if respondents must upload images, use a workflow designed and tested for image submission rather than assuming PDF conversion preserves the control.
Final delivery checklist
- Decide whether the screenshot is fixed or respondent-supplied.
- Keep an editable source and an untouched original PDF.
- Confirm image legibility, placement, alt text, labels, tags, and tab order.
- Test completion, saving, reopening, printing, and submission in the viewers your audience uses.
- State image requirements and provide an alternative submission route when PDF image upload is not dependable.
Frequently Asked Questions
Can I add a screenshot to a PDF form on a phone?
Microsoft documents inserting pictures and adding alt text in Word on a phone. That is a Word workflow; it does not establish direct editing of an existing PDF or guarantee an image-upload field after conversion.

Does Prepare a form automatically create an image-upload box?
No. Acrobat identifies and creates form components, but you must choose and test an appropriate field workflow. A generic PDF field is not universally an image-upload control.
What alt text should a screenshot have?
Describe the screenshot’s purpose and the relevant evidence in a short phrase, rather than transcribing every visible word.
