Poor sleep and sleep disorders are associated with cognitive decline and Alzheimer’s-related brain changes, yet researchers have not established whether sleep problems cause those changes—or whether treating them reduces Alzheimer’s risk.
What the evidence can—and cannot—tell us
The National Institute on Aging (NIA) describes a possible two-way relationship: changes associated with Alzheimer’s may disrupt sleep, while disrupted sleep may also be related to disease processes. The direction of cause and effect in people remains unresolved. An association between sleep and Alzheimer’s-related biomarkers or cognitive outcomes is not proof that poor sleep causes Alzheimer’s, or that improving sleep prevents the disease. NIA’s overview of sleep and Alzheimer’s risk explains this distinction.
For example, the NIA’s 2016 explainer summarized a five-year study of older women in which sleep-disordered breathing was associated with a nearly twofold increase in the risk of mild cognitive impairment or dementia. That combined outcome was not Alzheimer’s disease alone, and the observational finding does not establish that breathing problems caused the outcome or that treating them would prevent it. The same NIA explainer described a study of older men without dementia in which poor sleep, including more time awake at night, was associated with cognitive decline over more than three years. That, too, was an association, not proof of cause and effect.
Sleep quality is among the approaches researchers continue to explore, but the NIA says no intervention has yet been proven to prevent Alzheimer’s disease. Its prevention overview places sleep interventions among areas with less evidence than some other strategies. Read the NIA’s overview of what is known about Alzheimer’s prevention.

What this means for insomnia and possible sleep apnea
Persistent insomnia
If you regularly struggle to fall asleep or stay asleep, discuss the problem with a health care professional. The goal is to evaluate and address the sleep difficulty and its effects on daily life—not to promise Alzheimer’s prevention. NIA lists cognitive behavioral therapy for insomnia (CBT-I) among approaches being studied in older adults with insomnia symptoms; the study listing is not evidence that CBT-I prevents Alzheimer’s.
Possible sleep-disordered breathing
Loud snoring, witnessed pauses in breathing, or other concerns about nighttime breathing are reasons to seek medical advice. Sleep-disordered breathing and low oxygen have been linked in observational research to cognitive impairment risk, but that does not show that treating sleep apnea prevents Alzheimer’s. NIA’s active-study listing includes research on positive airway pressure treatment for obstructive sleep apnea and cognitive and vascular outcomes in American Indian participants. A study’s presence on a listing does not establish a prevention benefit. See NIA’s listing of active Alzheimer’s and related-dementia studies.

Sleep treatments are not proven Alzheimer’s prevention
The NIA states that there is no scientific evidence that sleep medications or other sleep treatments reduce Alzheimer’s risk. That does not mean sleep problems should be ignored: better sleep can matter for health, thinking, and daily function, even when a long-term prevention effect is unproven. Before using sleep medication or supplements, consult a health care provider; NIA notes potential medication risks, including dependence and next-day effects. NIA’s guidance on sleep and older adults discusses sleep concerns and when to seek advice.
Sleep trackers, journals, white-noise machines, CPAP devices, and supplements should not be presented as Alzheimer’s-protective products. The evidence described by NIA does not establish that these products reduce Alzheimer’s risk. A device or treatment may have a role in managing a specific sleep problem, but that is different from proving disease prevention.

If someone already has Alzheimer’s disease
Sleep changes can include sleeping too much or too little, waking often, or wandering at night. Poor sleep may worsen dementia symptoms and affect caregivers. In this situation, the priority is clinical evaluation, symptom management, and nighttime safety—not prevention. NIA offers guidance on addressing sleep problems in people living with Alzheimer’s.
