Most built-in Apple Watch and Pixel Watch health tracking does not require a paid subscription. Apple’s reviewed guidance describes features such as sleep tracking, heart-rate alerts, cycle tracking and ECG without identifying a recurring Apple health subscription as a prerequisite. Pixel Watch also records core health and activity data; Google Health Premium is for personalized coaching and certain advanced insights. A separate service, Safety Signal, covers connectivity for some Pixel Watch LTE safety features.
Availability depends on the watch generation, country, software, account setup and eligibility. The distinction to keep in mind is between collecting health data and paying for added interpretation, coaching or connectivity.
Apple Watch: built-in health features and subscriptions
Apple’s reviewed health overview and support guides describe heart-rate variability, high- and low-heart-rate alerts, irregular-rhythm notifications, sleep tracking, cycle tracking, overnight vitals, ECG and AFib History as Apple Watch capabilities. Those guides do not identify a recurring subscription as a requirement for these built-in features. That does not rule out charges for third-party apps or other services.
ECG
Apple says ECG is available on Apple Watch Series 4 or later and all Apple Watch Ultra models, subject to country availability and software. It is not intended for people under 22. Hypertension notifications
Apple’s guidance says supported watches evaluate patterns over a 30-day period to determine whether to send a hypertension notification. This is a notification about a possible pattern, not a blood-pressure reading: the watch is not a cuff or blood-pressure measuring device. Apple recommends discussing an alert with a health professional and collecting blood-pressure readings with a third-party cuff for seven days. Eligibility includes Apple Watch Series 9 or later or Ultra 2 or later, compatible iPhone and current software, wrist detection, a minimum age, no active pregnancy, no previous hypertension diagnosis, and availability in the user’s country or region. Regulatory clearance for some new models may still be pending in certain regions. Check Apple’s hypertension notification guidance for the current requirements.
Regional availability
Do not assume every feature appears on every watch sold in every market. Apple notes that Blood Oxygen is unavailable in the Vitals app for specified U.S.-purchased watches with part numbers ending LW/A. Verify the exact model and region in Apple’s health-feature information.

Pixel Watch: what Google Health Premium adds
Google documents heart-rate tracking, activity metrics, sleep tracking and ECG through the Google ECG app where available as Pixel Watch capabilities. Google Health Premium adds personalized coaching and certain advanced insights and recommendations; it is not a blanket fee to enable all health tracking. Google’s setup guidance describes a three-month Premium trial for eligible new users, but the trial is not guaranteed to everyone. See Google’s Pixel Watch setup and Premium information.
Sleep tracking versus sleep coaching
Recording sleep and receiving a sleep score are distinct from additional sleep insights or a sleep coach. Google’s help materials describe extra sleep insights and coaching for Premium members, while feature availability and requirements can vary by device, region and app surface. Google is transitioning plan terminology, so check the current Google Health app for the feature label and account-specific access rather than assuming that all sleep tracking requires payment. Safety Signal is a connectivity service, not a sensor unlock
Google describes Safety Signal as a Google Health Premium subscription service that provides connectivity for safety features when the wearer is away from their phone and does not have a carrier plan on the watch. It applies to Pixel Watch 2 and 3 LTE in the United States, Canada, United Kingdom and Germany; it is not available on Wi-Fi models. If the watch already has carrier service, Safety Signal is unnecessary for that connectivity use case. This is a safety/connectivity subscription, not a requirement for heart-rate or sleep tracking. Google says Fitbit Premium transitions to Google Health Premium in the redesigned Google Health app, with Google Health Coach for subscribers in supported countries. Depending on the app and account, you may still encounter the Fitbit Premium name during the transition. What to check before relying on a feature
- Exact watch generation: ECG and hypertension notification eligibility differ by model.
- Country and region: ECG, blood oxygen, hypertension notifications and Premium features may not be available everywhere.
- Software, phone and app setup: Keep the compatible phone, watch software and health app requirements in view.
- Personal eligibility: Age and health-history conditions apply to some features, including Apple’s hypertension notifications.
- Plan and connectivity status: For Pixel coaching or Safety Signal, confirm the current Premium plan, LTE model and carrier setup in the Google Health app.
