First, reduce the risk to users and the business: confirm what is failing, scope its impact, and use a prepared containment option such as rollback, isolation, disablement, or a safe fallback. Then investigate the whole production system—not just the model—restore service in a controlled way, and document what happened. The right response depends on the harm, affected dependencies, and whether a fallback is safe for the task.

1. Confirm the incident and scope its impact

Unexpected behavior is a symptom, not a diagnosis. A poor or unsafe result could involve changing inputs, a model or application change, a dependency, a security issue, or ordinary serving failures. NIST’s AI Risk Management Framework (AI RMF) recognizes that AI system performance and trustworthiness can change; AWS monitoring guidance distinguishes model and data signals from service-health metrics.

Start with concrete examples and establish what changed, who or what is affected, and how serious the consequences could be. Capture the affected tasks, user groups, regions, model and application versions, and time window. Classify the main risk so the response is proportionate:

  • Potentially harmful output or decisions: Could the system cause physical, financial, legal, or other meaningful harm?
  • Security or privacy exposure: Is there evidence of unauthorized access, data disclosure, or malicious input activity?
  • Degraded task quality: Are results incorrect, biased, off-topic, malformed, or otherwise failing the intended task?
  • Service availability: Are requests failing, slowing down, or exhausting capacity?

Escalate high-impact or potentially harmful events through the organization’s established safety, security, legal, and business procedures. Google Cloud’s AI/ML Security guidance recommends AI-aware response procedures, clear notification channels, and coordination among AI/ML, MLOps, security, data science, legal, and compliance teams.

2. Contain exposure without creating a larger outage

Choose a prepared action for the affected component and failure mode. Do not assume that rollback is always the safest or fastest choice: a model may be a dependency for several business functions, and isolating a component can itself take a production function offline. AWS-authored incident-response guidance dated May 27, 2026, recommends mapping AI components to business functions, documenting cascading effects, clarifying decision authority, and rehearsing response with incident responders, ML engineers, and business owners. It does not prescribe a universal priority order.

Containment option When it may fit Trade-offs to check
Revoke access There is a credible access or security concern and restricting permissions can reduce exposure. Check which users, services, or workflows depend on that access, and preserve logs and other evidence under applicable rules.
Roll back A recent model, application, or configuration change is a plausible cause and a known stable state is available. Check compatibility and downstream dependencies. A rollback can restore one component while disrupting services that rely on its newer interface or behavior.
Isolate Separating a component can limit a suspected failure or compromise while keeping unrelated systems available. Isolation may disable a production function or interrupt dependent services. Confirm what will stop working before applying it.
Disable Continuing to serve the affected function presents more risk than temporarily making it unavailable. Assess user and business impact, communicate the interruption, and define how the function can be safely restored.
Use a fallback A simpler model, cached result, or alternate path can safely handle the task while the primary system is investigated. Verify that fallback behavior is appropriate for this use case; availability alone is not enough if it produces unsafe or inadequate results.

Before acting, weigh how quickly the option reduces harm against its availability impact, dependencies, reversibility, security and privacy implications, and the task quality it preserves. Keep the authority to make urgent containment decisions clear in advance. Where permitted by privacy, security, and retention rules, preserve the relevant model and application versions, configuration, input or prompt context, timestamps, quality and error measurements, and action timeline. Minimize sensitive data in incident records and restrict access appropriately.

3. Diagnose with evidence from the whole system

Compare the incident period with a known baseline and, where relevant, a recent stable release. Separate evidence about service health, data, model behavior, application behavior, and operational changes; a change in one signal does not by itself prove that the model has degraded.

  • Inputs and data: Check schema violations, missing or invalid values, anomalous inputs, and changes in input or feature distributions.
  • Model behavior: Examine prediction distributions and confidence changes where confidence is meaningful. Evaluate model quality against ground-truth labels when those labels are available.
  • Application outputs: For generative systems, inspect task-specific failures such as unsafe, biased, off-topic, malicious, or malformed content. Check application-level validation for required formats, ranges, or constraints.
  • Service health: Review request volume and traffic patterns, latency, error rates, and relevant capacity measures such as CPU, GPU, memory, or disk utilization.
  • Operational and security context: Check recent version, configuration, permission, and pipeline changes, as well as suspicious request patterns and dependency failures.

A distribution shift is a reason to investigate, not proof that users are receiving worse results. Determine whether the shift affects the application and user outcomes. For some model-quality measures, AWS notes, ground-truth labels arrive only after inference; those measures cannot necessarily detect a live incident immediately. Generative outputs also vary and can be subjective, so define task-specific checks and use human review where the risk warrants it rather than relying on one generic score.

4. Restore service gradually and verify the result

After the cause is understood—or exposure is adequately controlled—test the candidate recovery state before returning normal traffic. Verify that the serving interface works and that representative outputs meet the task’s requirements. If controlled traffic deployment is available, use a limited or staged rollout and retain a fast path back to the previous stable state.

During recovery, watch both operational alerts and the business-specific quality or safety measures that matter for the affected task. Google Cloud’s AI/ML Reliability guidance recommends monitoring during deployment and automated rollback to a previous stable version when alerts fire or performance thresholds are missed. A simpler model or cached data may serve as a fallback for some services, but use either only when its limitations are acceptable for the task. NIST’s AI RMF calls for planning recovery, change management, and the ability to fail safely.

5. Review the incident and strengthen readiness

Keep an incident record covering impact, timeline, investigation, containment, recovery, causes, and follow-up work. Review whether alerts surfaced the problem promptly, whether containment caused secondary effects, and whether decision authority and escalation paths were clear. Google Cloud’s postmortem guidance describes the purpose as improving technology and future response rather than assigning guilt.

Close the loop with relevant teams and, where appropriate, affected users or communities. NIST AI RMF Manage 4.3 states: “Incidents and errors are communicated to relevant AI actors, including affected communities.” Track follow-up actions to completion, including monitoring changes, safer fallback behavior, clearer runbooks, or additional testing.

Production monitoring and response-readiness checklist

Use this as a starting point, then set thresholds and escalation rules based on the system’s risk, baseline, user impact, and operational objectives. There is no universal drift threshold or response-time target established here.

  • Service health: Request rate, latency, error rate, traffic patterns, and relevant CPU, GPU, memory, disk, or other capacity measures.
  • Inputs and data: Schema violations, missing or invalid values, anomalous inputs, and input-distribution changes relative to a baseline.
  • Model behavior: Prediction-distribution changes, low-confidence spikes when confidence is meaningful, quality against labels when available, and relevant changes in feature relationships.
  • Generative application outputs: Task-specific checks for unsafe, biased, off-topic, malicious, malformed, or otherwise failing content, with human review where appropriate.
  • Operational context: Version and configuration changes, permissions, pipeline failures, and suspicious request patterns.
  • Response readiness: Actionable alert routing, named owners, escalation thresholds, permitted containment options, documented dependencies, and rehearsed recovery steps.

What the NIST AI RMF does—and does not—provide

The NIST AI RMF 1.0 is voluntary guidance, not a universal mandatory incident runbook. Its core framework was released January 26, 2023; NIST’s current overview says it is being revised and notes that a concept note for a critical-infrastructure profile was released April 7, 2026. The NIST Playbook offers implementation guidance based on AI RMF 1.0. These materials can help structure risk management and production monitoring, but they do not determine which legal, contractual, or sector-specific requirements apply to a particular organization. Assess those obligations separately.
