Coding agents fail in the outer loop when the system around the model does not reliably turn a request into a verified, acceptable change. Here, “outer loop” means the engineering and evaluation around repeated agent work: framing the task, providing a usable repository and environment, collecting execution feedback, checking the result, deciding when to stop, and reviewing the final change—not just the agent’s sequence of tool calls.
The failure is rarely explained by code generation alone. A relevant file can be found without the right behavior being changed; selected tests can pass while important requirements remain unchecked; and a benchmark result can be mistaken for a guarantee about a different repository or workflow.
Why is the outer loop where coding agents fail?
A coding task is a chain of dependencies. The request must describe an observable outcome; the agent must work in a sufficiently representative repository and environment; its edits must respond to evidence; and verification must reflect what “done” means. A weak link can make a plausible patch unusable, even when the model produces convincing code.
That is why a result should be attributed to the complete setup, not casually to a model alone. SWE-bench, for example, gives an agent a repository snapshot and issue, then evaluates its proposed patch in Docker by running repository tests. The resulting score is conditional on the benchmark task set, environment, agent harness, and test suite. It is useful evidence about performance under those conditions, not a direct certification of integration quality or performance in another workflow. See the SWE-bench benchmark description.

Where does the work system break down?
1. The task is not framed in checkable terms
An issue can describe a symptom without specifying the intended behavior, relevant edge cases, or acceptance conditions. If those details are unclear, an agent may make a locally plausible change that does not satisfy the requester. An evaluator can only check what the task and its tests make observable.
Ambiguity is a mechanism to inspect, not a measured explanation for a known share of production failures: the studies cited here do not quantify how often unclear requests cause deployed-agent failures. Before delegating, state the expected behavior and the conditions that would demonstrate it.

2. The repository or environment does not match the work
Dependencies, runtime versions, configuration, and integration context shape whether a change works. A reproducible container is valuable because it makes evaluation repeatable, but success inside a fixed benchmark container still says nothing conclusive about an environment the benchmark did not exercise. Record the repository snapshot and execution conditions, and check whether they represent the workflow where the patch will land.
3. Finding the right code is mistaken for solving the problem
Localization is progress, not completion. In a 2025 study of OpenHands, SWE-agent, and Prometheus trajectories on SWE-bench, Majgaonkar and coauthors reported that failed trajectories were consistently longer and more variable than successful ones. They also found that 72–81% of failed trajectories identified the problematic files in that study’s benchmark setup. The result illustrates a practical distinction: an agent may reach relevant code yet still misread the evidence, make an ineffective change, or fail to converge. Useful diagnosis therefore looks beyond whether the agent opened the right file. Inspect whether it connected the issue to the observed failure, made a suitably scoped change, used test and tool output to revise its approach, and reached a verifiable result.
4. Passing tests is treated as proof of correctness
A green test run establishes that the selected checks passed in the tested setup. It does not establish that every requirement was captured, that untested cases work, or that the change is maintainable. Chen and Jiang’s 2024 analysis examined 4,892 patches from ten agents on 500 SWE-bench Verified issues. Their abstract reports that some test-passing patches changed different files and functions from the maintainer’s gold patch, which the authors point to as evidence of test-coverage limitations. They also found no single agent dominated and that agents did better on simpler codebases; these observations describe that sample and setup, not a universal ranking. Generated tests can add another filter, but they are not a guarantee. The SWT-BENCH paper studies test generation as its own task and reports that generated tests can filter proposed fixes. Such tests are additional evidence, not proof that the full requirement or every regression has been covered. See “Code Agents are State of The Art Software Testers”.
5. The loop stops before completion is demonstrated
An agent can stop after an edit, after a tool call, or after a test run without establishing that the task is complete. Define completion through observable checks, then inspect the final diff for scope, edge cases, integration, and maintainability. The sources discussed here do not compare stopping policies well enough to establish one universally best rule; the practical requirement is that the stopping condition be tied to the task’s acceptance criteria.

6. A correct patch is confused with safe operation
Patch correctness and execution safety are separate questions. Running repository code or agent-selected commands can create risk even when the eventual change is correct. RedCode frames risky code execution and generation as a deployment concern and evaluates agents in a Docker sandbox. Use permission boundaries and isolation appropriate to the commands and code being run; a passing task result does not answer whether execution was safely constrained. See RedCode.

Compare the setup on the work it needs to handle, rather than treating a leaderboard percentage as a complete verdict. Public benchmarks provide useful context, while internal evaluation checks whether an agent can meet a team’s repositories, constraints, and acceptance criteria.

Fresh tasks matter because a fixed public set cannot represent every future issue. SWE-rebench describes a continuous pipeline for collecting fresh tasks with the goal of supporting contamination-aware evaluation. Teams can apply the same principle by periodically adding representative, previously unseen work and retaining enough task and environment detail to reproduce results. See SWE-rebench.
What should a reviewer check before accepting an agent’s change?
- Match the change to the request. Confirm that the patch implements the stated behavior, including relevant edge cases and acceptance conditions.
- Read the execution evidence. Check which commands and tests ran, what they covered, and whether failures were resolved rather than ignored.
- Inspect the final diff. Look for unrelated edits, unexpected files or functions, regressions, and changes that make the code harder to maintain.
- Check the target context. Verify dependencies, runtime, configuration, and integration assumptions against the environment where the change will be used.
- Review execution boundaries. Confirm that commands and untrusted code ran with suitable isolation and permissions.
These checks separate “the agent produced a patch” from “the task has been completed and the change is ready to accept.”
