AI coding failures are hardest to catch when the code looks plausible, passes the tests that were run, or behaves differently in its real deployment environment. A green test run shows that the tested behaviors passed; it does not prove that untested cases work, that a change is minimal, or that the code is secure. No available study establishes one defect type as universally hardest to detect.
Why plausible code can still be wrong
Generated code may be syntactically valid and appear to implement the requested feature while making a subtle mistake in an assumption, edge case, or security-sensitive operation. Those failures can be harder to notice than a crash because the program may keep running and produce believable results.
Evidence from different studies points to several ways defects can evade notice, but the studies use different models, prompts, code samples, and evaluation methods. Their percentages describe those particular settings, not a general failure rate for AI-written software.
What test results do—and do not—tell you
Tests only exercise the behaviors and conditions they contain. A passing suite is useful evidence for those cases, but it cannot establish that untested inputs, error paths, system interactions, or security properties are correct.

Microsoft Research’s Precise Debugging Benchmark illustrates another limitation: passing tests and making a precise change are separate outcomes. In the benchmark’s defined tasks, evaluated frontier models had unit-test pass rates above 76% while edit-level precision remained below 45%, even when models were instructed to make minimal debugging changes. That result does not measure how often this happens in production, but it shows why a patch can pass tests and still include unnecessary or problematic edits.
Security weaknesses are not one universal percentage
CSET’s evaluation of five language models
The Center for Security and Emerging Technology (CSET) reported that an average of 48% of outputs from five tested language models contained at least one bug that could potentially enable malicious exploitation under its evaluation conditions. Each tested model produced buggy code in at least 40% of the prompts. CSET explicitly described the evaluation as limited in scope and not representative of average software-development workflows, so these figures are evidence that insecure outputs can occur—not a general rate for AI-generated code.

A sample of Copilot-generated snippets in GitHub projects
The empirical study Security Weaknesses of Copilot-Generated Code in GitHub Projects: An Empirical Study examined 733 snippets and reported security weaknesses in 29.5% of the Python samples and 24.2% of the JavaScript samples. The authors identified weaknesses spanning 43 CWE categories, including insufficiently random values, improper code generation, and cross-site scripting. The arXiv page notes that the preprint was accepted for publication in ACM Transactions on Software Engineering and Methodology in 2025. These figures describe that sample and method; they should not be read as rates for all AI-generated code.
The studies do not provide a controlled, apples-to-apples ranking of which defect class is hardest to catch. A useful way to compare a test suite, review process, or scanner is to ask what weakness classes it covers, which languages and frameworks it supports, whether it needs realistic inputs or deployment conditions, and whether its findings lead to focused, actionable fixes.

Why local success may not survive deployment
A program’s behavior can depend on its runtime, configuration, dependencies, platform, and connections to other systems. Code that works in a developer’s local setup may fail when those conditions change, or may behave incorrectly only when interacting with a service or platform.
Microsoft Research’s 2020 study of 4,960 real failures in deep-learning jobs found that 48.0% involved interaction with the platform rather than code logic, often because local and platform environments differed. This was not a study of AI-generated code, so it cannot show how often generated code has environment-related defects. It does illustrate why a successful local run may not reveal problems that emerge in deployment.

What static analysis and AI review can catch
Static-analysis tools and security scanners can help find defects, but their effectiveness varies by bug class, test case, and complexity. NIST’s 2023 SATE VI Report: Bug Injection and Collection found that higher-complexity bugs were harder for tools to find than lower-complexity ones. It concludes that static analysis can help find real security bugs in large codebases, while recommending that users evaluate tools on their own codebase before production use. As NIST puts it, “The right set of tools, used properly, can help increase code quality and security.”
A 2026 study in Empirical Software Engineering examined developer-AI interactions using multiple scanners and manual review. In its later experiment, the evaluated models found and fixed many, but not all, identified vulnerabilities. The authors also note that vulnerabilities outside the scanners’ detection capabilities could remain undetected. AI review and scanners are therefore aids, not proof that code is safe.

A practical review for AI-generated code
Use checks that target the ways a defect could hide, rather than treating one green result as a complete review:
- Exercise boundaries and failure paths. Add or run tests for boundary values, invalid inputs, error handling, and interactions with dependent systems, not only the happy path.
- Read for behavior and assumptions. Check what the code actually does, including how it handles inputs and failures. A plausible explanation of a patch is not evidence that its behavior is correct.
- Check the real execution context. Compare runtime versions, dependencies, configuration, and deployment conditions when code works locally but fails elsewhere.
- Use tools matched to the repository. Choose static-analysis and security tools suited to the languages and frameworks in use; review their findings and validate their performance on the codebase where they will be used.
- Review beyond the immediate feature. Consider security and maintainability as well as whether the requested behavior works, and look for unnecessary changes in the patch.
- Do not treat another AI review as independent assurance. Models may miss vulnerabilities or fail to repair them, just as scanners have blind spots.
This is a practical, evidence-informed workflow, not a method shown by the cited studies to catch every defect. The appropriate checks depend on the codebase, risk, and execution environment.
