A clean malware scan—or a normal-looking homepage—does not prove that a website is free of SEO spam. Spam may be stored in a place the scan did not inspect, added as new pages, hidden in markup, or shown only to certain visitors or crawlers. Whether a scanner finds it depends on what it checks and how it checks it; there is no evidence that every traditional scanner misses these attacks.

Why a clean scan may not settle the question

A scan result describes the areas and responses that a particular scan examined. If it checks only selected public URLs or one ordinary browser view, it may not reveal spam stored in a CMS database or files, present on an unvisited URL, or served only under a particular request condition. This is a scope mismatch, not proof that all scanners work the same way.

Google documents several forms of hacked content that may not be obvious during a casual visit: code injected into existing pages, newly created spam pages, hidden links or text, and redirects that vary by referrer, user agent, or device. A homepage that looks normal therefore cannot stand in for a check of the whole site.

Where SEO spam can hide

Injected code on otherwise familiar pages

Attackers can add malicious JavaScript or iframes to existing pages. The visible page may appear unchanged even though its code or content has been altered. Google includes these among examples of hacked content in its Spam Policies for Google Web Search.

New pages and database records

Spam may be added as new URLs rather than placed on the homepage. Sucuri’s Q2 2022 report describes fake posts inserted into CMS databases and doorway pages generated on the fly. A scan that does not discover or request those URLs, or does not inspect the relevant CMS data, may not surface them.

Hidden markup in files or page content

Sucuri also describes hidden HTML in plugin or theme files. Hidden content can use CSS or JavaScript; the report gives the example of links placed in a tiny div that ordinary visitors may not notice. Google likewise identifies hidden text and links as a hacked-content pattern. A visual check alone is poorly suited to finding content that is present in markup but not readily visible.

Content or redirects that depend on the requester

A site can return different content depending on who requests it or how the request arrives. Google defines cloaking as “presenting different content to users and search engines with the intent to manipulate search rankings and mislead users.” It also warns that hackers may use cloaking to make a hack harder for the site owner to detect. A redirect can vary by referrer, user agent, or device, so clicking through from a search result may behave differently from opening the same address directly.

What the available figures do—and do not—show

Sucuri reported that 44.82% of its SEO-spam detections in Q2 2022 were in its keyword-spam category, counting 66,445 infected websites in that category. These are vendor detections from one quarter, not a current or market-wide estimate of infected sites or scanner performance.

Google says it finds 40 billion spammy pages every day. That figure concerns Google’s broad search-spam detection, not hacked websites specifically and not malware-scanner misses. A 2015 study by Google researchers reported 760,935 hijacking incidents, but its authors cautioned that the dataset was biased toward threats known to Google’s pipelines, was not exhaustive, and had unknown false negatives. Neither figure establishes how often a particular scanner misses SEO spam.

How to investigate a site that looks clean

Use the scan as one signal, then check the different surfaces and request contexts suggested by the attack patterns. These checks are investigative guidance, not a universal forensic procedure or a certification of any scanning product.

  1. Check Search Console. Review any security or hacked-content warning and confirm the affected issue. Google says registered site owners can receive suspected-hack notices. Its hacked-site help describes the verification, cleanup, and review workflow.
  2. Look beyond existing pages. Check for unexpected indexed URLs and newly created pages, including URLs that may not be linked from the homepage. Compare search-result behavior with direct visits where a suspicious URL is known.
  3. Inspect site files and CMS content. Look for unexpected code in relevant plugin or theme files and suspicious records or posts in the CMS database. A public-page scan and a review of stored content cover different surfaces.
  4. Examine rendered pages and underlying markup. Check for hidden text, links, scripts, or iframes that may not be apparent in the visual layout.
  5. Compare request contexts for suspicious URLs. Where practical, compare responses when reached directly and through a search result, and consider whether behavior changes with referrer, user agent, or device. A difference warrants investigation; it does not by itself prove malicious cloaking.
  6. Fix the underlying issue, then request review when appropriate. Follow Search Console’s instructions for the specific warning and verify that it is cleared. Google notes that programmatically detected content may return after it is found clean on a regular recrawl; this does not guarantee an immediate return.

How to judge a scanner’s coverage

When evaluating what a scan result means, focus on its stated coverage rather than assuming the word “clean” means every possible surface was checked. Useful questions include:

  • Did it request a broad set of public URLs, or only a small selection?
  • Does it inspect files and CMS data, or only pages served publicly?
  • Does it examine markup beyond what an ordinary visitor sees?
  • Does it test one response context, or account for conditional content and redirects?
  • Does a finding identify the affected URL or file clearly enough to verify and clean up?

These are practical comparison axes derived from documented attack patterns. They do not rank products or establish that any named scanner has a particular capability.
