A green test summary proves only that the checks a harness actually collected and ran passed. It does not prove that the intended checks ran—or that any assertion examined the behavior you care about. In three cases described by developer Debashish Ghosal, tests passed while assertions were missing, files produced zero assertions, or an HTTP authentication guard was bypassed by an import failure.
How can tests pass when they do not test anything?
A test runner’s success status can conceal gaps between what a test appears to check and what the test system actually executes. Ghosal’s September 19, 2026 article describes three examples in the planner-critic-engine project and CauterRule v0.3.0. These are project-specific incidents, not evidence of how often such failures occur across software projects.
A test function that asserted nothing
In planner-critic-engine, a test named test_all_adapters_importable had a body containing only pass. The name suggested that adapters were checked, but the function made no assertion. Ghosal says code review caught it before CI and before an LLM sweep. As he put it, “A test named test_all_adapters_importable asserted nothing. A harness that reported zero assertions
In the same project, 57 of 65 assertion files were reportedly in the wrong format. The harness parsed them but found no assertions to run, then returned 0 / 0 as success. A green status in this case meant the suite had executed nothing—not that its intended checks had passed. Ghosal’s article on DEV Community

1,558 passing tests and an untested HTTP guard
In CauterRule v0.3.0, the suite reportedly showed 1,558 tests green, yet an MCP HTTP bearer-authentication guard did not run as intended. The project’s field-test report says the helper _request_headers() imported fastmcp.server.dependencies inside a broad try/except Exception. When that import was unavailable, the helper returned empty headers, and the guard treated HTTP requests as local transport. An unauthenticated list_rules request from outside the container then returned rules.
The unit tests had monkeypatched _request_headers, so they did not exercise the real request path where the import and transport decision occurred. The field-test report says the issue was found in a Docker field test and fixed by switching to the official mcp SDK Context API. It reports that unauthenticated calls then returned 401 and authenticated calls succeeded. CauterRule v0.3.0 field-test report

The report also says 157 of 159 Docker field-test checks passed. That count is the project’s report of this evaluation; it does not establish that all tests or security properties were correct. CauterRule v0.3.0 field-test report
What should a green test summary tell you?
Treat the summary as evidence about the checks that were collected and executed, not a blanket guarantee that the application works. A useful result should make it possible to tell whether tests were discovered, assertions were parsed, and the relevant behavior was exercised. Names, file counts, and a green badge cannot answer those questions on their own.

How can CI fail when no tests or assertions run?
Add a check at the harness boundary so that a run with zero collected tests or zero parsed assertions exits unsuccessfully. In the reported harness failure, parsing succeeded but assertion collection did not; a check only for parse errors would therefore have missed the empty run.
- Have the harness report both the number of executed tests and the number of parsed assertions.
- Make CI fail if a module produces zero results; treat
0 / 0as an error state, not a passing run. - Make skipped modules and swallowed imports visible as failures rather than silently converting them into empty results or fallback behavior.
These safeguards address collection and execution failures. They do not prove that a nonzero set of assertions checks the intended behavior.

How do you test an authentication guard that depends on HTTP wiring?
Exercise the real request path at the boundary where the security decision is made. If the defect involves HTTP headers, transport detection, imports, or middleware wiring, a unit test that replaces the relevant helper can bypass the very behavior under test.

- Send an authenticated request through the same path and verify that the intended operation succeeds.
- Run the check in an integration or deployment-like environment when container configuration or runtime imports affect the request path.
In the CauterRule account, the Docker field test exposed a problem that the monkeypatched unit tests did not. This is a lesson from that incident, not a guarantee that deployment-level tests will catch every security defect. CauterRule v0.3.0 field-test report

Why test counts are useful—and insufficient
Minimum counts can reveal that a suite disappeared, a module was skipped, or a parser produced no assertions. They are a useful alarm, but not a measure of test quality: a test can run and still assert the wrong subject or fail to encode the behavior that matters.
Review test bodies and outcomes, not only names and totals. Confirm that each important test would fail if the behavior it is supposed to protect were broken. Comments on Ghosal’s article raise examples of tests that run but assert the wrong thing; those comments are anecdotal, rather than evidence establishing how common that failure is. What are the limits of meta-tests?
A meta-test can verify that a test harness reports nonzero execution and assertion counts, but it creates another check that must remain meaningful. Ghosal warns: “Meta-tests add process, and process can rot — a meta-test that stops checking is just another green checkmark.” Keep the meta-test tied to observable harness output and review it when the harness changes.
