A good candidate for orthokeratology (Ortho-K) has myopia, healthy eyes that can be safely fitted with the specific lens, and the ability to follow its overnight-wear, hygiene, and follow-up instructions. An eye-care professional must assess the person’s eyes and prescription; an online checklist cannot establish suitability. Ortho-K temporarily changes corneal shape to reduce myopia, and its vision effect wears off when lens wear stops.

What makes someone a potential Ortho-K candidate?

Ortho-K uses specially designed rigid gas-permeable contact lenses, commonly worn overnight, to temporarily change the cornea’s curvature and reduce myopia. The lenses require a prescription and professional fitting. They are intended for temporary vision correction, not permanent reshaping. The U.S. Food and Drug Administration (FDA) explains that “The vision correction effect is temporary.” FDA: Types of Contact Lenses

An evaluation may make sense if the person wants daytime correction without glasses or daytime contact lenses and understands that results vary: some people’s correction can wear off during the day, and prescribed lens wear is needed to maintain the effect. The clinician also needs to confirm that the person’s eye health and prescription suit the particular lens design.

Which factors affect suitability?

Eye health and corneal shape

The candidate’s cornea and ocular surface must be suitable for contact lens wear. FDA labeling for the devices reviewed identifies acute or subacute inflammation or infection, and disease, injury, or abnormality affecting the cornea, conjunctiva, or eyelids, among reasons not to use the labeled lens. Other device labels may add contraindications, so the clinician should consult the current instructions for the exact product.

Older FDA guidance also lists examples such as severe dry eye, reduced corneal sensitivity, certain systemic diseases, active corneal infection, relevant ocular allergies, and allergy to ingredients in lens-care solutions. These are not a complete universal checklist; the prescriber must assess the individual and the lens label.

Prescription and lens design

Prescription limits depend on the specific Ortho-K lens. For example, the FDA’s 2026 approval for the Dilight lens specifies temporary reduction of myopia up to 4.00 diopters (D), with astigmatism up to 1.50 D. Those limits apply to that device’s U.S. indication; they are not universal Ortho-K thresholds. FDA: Dilight PMA approval

Ask the eye-care professional whether the exact lens design is appropriate for the prescription and what vision to expect during waking hours and across different activities. The FDA indication concerns temporary refractive reduction; it should not be confused with a separate claim about slowing myopia progression.

Ability to manage overnight lenses

Ortho-K requires consistent insertion, removal, cleaning and disinfection, storage, and follow-up according to the prescriber’s and manufacturer’s instructions. Overnight wear raises the risk of serious eye infection, so hygiene and prompt attention to symptoms are central to safe use. Do not substitute generic care advice or products: use the exact regimen prescribed for the lens. The reviewed FDA Dilight approval specifies chemical disinfection.

Can children and teenagers use Ortho-K?

Children and teenagers may be considered, but age alone does not establish suitability. The assessment should include eye health and prescription, the child’s maturity and ability to handle lenses hygienically, caregiver support, and whether the family can keep up with scheduled professional checks.

Ortho-K may also be discussed as one option for managing myopia progression in children and adolescents. The American Academy of Ophthalmology’s (AAO) 2019 evidence assessment reported that included studies typically found about 50% less axial elongation over two years: average axial-length change was about 0.3 mm in Ortho-K groups versus 0.6 mm in control groups, corresponding to an approximate 0.5 D refractive difference. The studies varied, most were conducted in Asian populations, and the result is a group-level study finding—not a forecast for an individual child. The AAO also emphasized the risk of potentially vision-threatening microbial keratitis. AAO: Orthokeratology evidence assessment

Myopia control and temporary vision correction are distinct goals. Discuss with the clinician whether Ortho-K or another appropriate option best fits the child’s eyes, vision needs, and ability to follow the treatment plan. The AAO’s guidance addresses alternatives and the risks of overnight lens wear. AAO: Myopia Preferred Practice Pattern

Who should not use Ortho-K, or needs extra assessment?

A person with an active eye infection or inflammation, or a disease, injury, or abnormality affecting the cornea, conjunctiva, or eyelids, may be unable to use a lens whose labeling lists those conditions as contraindications. Other health or eye-surface issues may also affect suitability, depending on the device and the clinical assessment. Do not start Ortho-K until an eye-care professional has reviewed the relevant condition and the exact lens instructions.

Someone who cannot reliably follow overnight-wear, cleaning, disinfection, or follow-up requirements may not be a safe candidate. Contact lens wear during sleep can lead to serious infection, and microbial keratitis can threaten vision; the AAO identifies that risk as a significant safety concern.

How to compare Ortho-K with other options

If more than one approach could suit the patient, ask the clinician to compare options against the person’s actual needs rather than choosing by age or prescription alone. Useful points include:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • How well each option fits the refractive error and visual needs.
  • Whether the goal is vision correction alone or also myopia-progression management, and what evidence supports that goal.
  • Safety considerations, including overnight wear and infection risk.
  • Daily care, compliance, and follow-up demands for the patient and, where relevant, caregiver.
  • Expected vision quality in varied lighting and activities.
  • Total cost and the patient’s preferences.

For a child, the clinician can discuss other suitable myopia-control approaches, including daytime optical options, alongside Ortho-K. The decision should account for the child’s circumstances and the family’s ability to manage the selected treatment.
