Choose washable incontinence underwear when leakage is light or mild to moderate, the product is rated for your needs, it fits comfortably, and you can manage washing and drying. Disposable briefs or pads may be more practical when you need stronger protection, easier changes away from home, or less laundry. There is no one best option for everyone; using different products in different situations can be reasonable.
Will washable underwear manage my amount of leakage?
Start with how much and how unpredictably leakage occurs, then compare that pattern with the product’s stated capacity. Underwear designed for light leakage should not be assumed to manage larger or unpredictable losses. Consider how long you will wear it and whether you can change when needed, especially overnight or when away from home.
The available studies do not establish a universal capacity threshold at which washable underwear stops being suitable. If your leakage exceeds a product’s stated use, or it does not keep you dry in your ordinary routine, consider a different level or type of protection rather than relying on the word “washable.”

| Consideration | Washable underwear | Disposable briefs or pads |
|---|---|---|
| Use pattern | Can suit light or mild-to-moderate urinary leakage when the stated capacity matches the wearer’s needs. | May be more practical when stronger protection or convenient changes are priorities. |
| Changing and care | Requires a washing and drying routine and enough clean pairs. | Avoids laundry, but requires resupply and disposal. |
| Fit and comfort | Fit and comfort matter; one short crossover study found favorable ratings for one reusable product on several measures. | Product performance varies; a 2007 review found disposable insert pads generally performed better than the washable designs studied in its one eligible trial. |
| Skin health and odor | No category-wide advantage is established by the cited evidence. | No category-wide advantage is established by the cited evidence. |
| Cost-effectiveness | Depends on purchase price, useful life, and laundry costs; no universal savings amount is established. | Depends on purchase price and replacement frequency; no universal cost advantage is established. |
The evidence is limited and product-specific. A 2007 Cochrane review of absorbent products for light urinary incontinence in women found only one eligible trial; disposable insert pads generally performed better for leakage prevention, preference, and acceptability than the washable designs studied. It found no strong evidence that either design was better for skin health, and neither was clearly dominant on cost-effectiveness. The review does not rank every product sold today. In a 2018 randomized crossover study of Icon reusable underwear versus disposable pads for women with mild-to-moderate urinary incontinence, 70 women were randomized and 52 completed the study. Quality-of-life scores did not significantly differ. Ratings favored Icon on several measured outcomes, including fit, discretion, comfort, and perceived skin dryness; there was no significant difference in reported ability to hold urine without leakage or prevent odor. The study evaluated one product over a short period, and excluded participants with fecal incontinence or an active urinary tract infection, so its results should not be generalized to all products or users. See the 2018 study.
What should I consider if I have heavier or overnight leakage?
Do not treat washable underwear as a substitute for a product whose capacity matches your leakage. Consider how much protection you need, how long it must last, and whether you can change during the night or while out. The available evidence does not establish that washable underwear reliably handles heavy or unpredictable leakage across products.

Choose a close, comfortable fit and pay attention to how the product feels both dry and wet. A product that shifts, rubs, or feels uncomfortable may not work well in everyday use. Evidence does not establish that washable underwear or disposables universally prevent odor or protect skin better.

It also mentions skin cleansers and creams that may help with irritation. Read NIDDK’s treatment guidance.
What does the laundry routine and total cost involve?
Reusable underwear means planning for washing and drying, and having enough clean pairs between loads. Disposable products avoid that laundry, but need regular resupply and disposal. Specific wash temperatures, drying times, and product lifespans are not established here; follow the care instructions on the product label.

To compare cost, use the actual price you would pay, how often each product needs replacing, the expected useful life of washable pairs, and any added laundry costs. The Cochrane review did not find a universally dominant design for cost-effectiveness, so savings depend on an individual’s use and costs rather than the category alone.
Can I use both washable and disposable protection?
Yes. One practical arrangement is washable underwear at home and disposables for travel or situations where changing and laundry are difficult. This is a personal convenience choice, not a clinical rule; match the product to the wearer’s leakage pattern and circumstances.
