Retinal blood vessels may offer clues about cardiovascular health, but an eye photograph or routine eye exam is not established as a way to diagnose heart disease or replace standard risk assessment. Studies link some retinal vessel patterns with coronary disease, and AI tools that analyze retinal images are being investigated; their clinical usefulness for individual patients remains unsettled.

What retinal blood vessels may reveal

The retina—the light-sensitive tissue at the back of the eye—contains small blood vessels that can be photographed. Researchers study features such as vessel width, winding, branching and blockage because changes in the tiny vessels may reflect aspects of vascular health elsewhere in the body.

A 2020 systematic review of 42 publications reported associations between retinal vascular features and acute coronary syndrome, coronary artery disease, heart failure and conduction abnormalities. These findings show that the features have been studied in relation to heart conditions; they do not establish that a particular eye finding directly causes heart disease or diagnoses it in an individual.

A 2024 systematic review and meta-analysis by Zhang and colleagues, published in Retina, pooled 21 cohort studies. Retinal microvascular changes—including narrowed arterioles, widened venules and vessel occlusion—were associated with incident coronary heart disease. The pooled adjusted hazard ratio was 1.20 (95% confidence interval 1.13–1.27). This is a relative association across observational cohorts, not a 20% absolute chance that a person with one of these findings will develop heart disease.

Can an AI retinal scan predict heart disease?

Deep-learning systems can analyze retinal photographs for combinations of vessel and other image features. A 2024 scoping review in the European Heart Journal found 24 studies published from 2018 through 2023. The evidence base was still developing:

  • Twenty-three of the 24 studies (96%) were cross-sectional.
  • Eight (33%) included follow-up outcomes.
  • External validation was reported in 21% of studies.
  • Four studies (17%) compared models with commonly used clinical risk scores in a prospective setting.

The review called for more prospective research, comparisons with established risk scores, and models that combine image analysis with traditional risk factors. External validation matters because a model that performs well in one dataset may not work equally well in different populations or clinical settings.

What one diabetes and prediabetes study found

An American Heart Association News report described a UK Biobank analysis of 1,101 adults with prediabetes or type 2 diabetes. An AI system grouped retinal images into low-, moderate- and high-risk categories. Over a median 11 years, the reported cardiovascular event proportions were 8.2% in the low-risk group, 15.2% in the moderate-risk group and 18.5% in the high-risk group. After adjustment, the moderate- and high-risk groups had 57% and 88% higher likelihood of events, respectively, than the low-risk group.

Those results describe groups in a selected study population and the particular analysis used. They do not establish that a consumer retinal scan can diagnose heart disease or predict an individual’s outcome.

Why an eye finding is not a heart diagnosis

An association can help researchers identify patterns, but diagnosis and useful prediction require more. A test needs to work reliably in the people for whom it is intended, add meaningful information beyond established assessments, and lead to an appropriate clinical action.

An analysis of the ARIC cohort followed 9,155 adults without diabetes for a mean of 8.8 years; 700 coronary events occurred. Retinal vessel caliber was associated with coronary risk in women, but the added predictive value beyond the Framingham model was modest and judged unlikely to be clinically important. This illustrates the difference between finding a statistical relationship and improving decisions for patients.

Current evidence therefore does not establish that visible retinal vessels reveal a blocked heart artery, that retinal photographs diagnose cardiovascular disease, or that routine retinal imaging can stand alone as a heart-risk screen. Retinal patterns may eventually contribute to risk assessment, but the 2024 review’s limited external validation and prospective comparison data are important constraints.

Vision difficulty is a different kind of finding

Trouble seeing should not be confused with retinal vessel biomarkers. A 2024 American Heart Association News report covered an observational study of 11,332 adults in China aged 45 or older who had no known cardiovascular disease. After seven years, self-reported vision difficulty alone was associated with 24% higher cardiovascular disease risk than reporting no sensory issue. Participants reporting both vision and hearing problems had a 35% increase.

This study concerned reported sensory difficulty, not an AI retinal-image test or a finding of abnormal retinal blood vessels. It does not show that vision difficulty causes heart disease; the authors said the mechanisms were uncertain.

What to do if an eye-care professional notes vessel changes

Ask what the finding means for your eye health and whether it warrants a conversation with your primary-care clinician about cardiovascular risk. A retinal observation should be interpreted in context rather than treated as a diagnosis.

Conventional assessment focuses on established risk factors, including blood pressure, cholesterol, glucose when appropriate, and health behaviors such as smoking, physical activity and diet. The American Heart Association notes that high blood pressure often has no symptoms and must be measured. It recommends screening at regular health visits or at least annually when blood pressure is below 120/80 mm Hg.

If a clinician advises you to monitor blood pressure at home, a validated upper-arm monitor can measure it; it does not analyze retinal images or diagnose cardiovascular disease. Follow the clinician’s instructions about when and how to take readings. A single measurement does not establish your overall risk.
