If someone may be in immediate danger or seriously injured after sexual assault, call the emergency service for their location. If there is no immediate emergency, help them reach safety, listen without pressing for details, and ask what support they want. You can offer to stay with them while they contact a medical provider or sexual-assault support service.
1. Check for immediate danger or serious injury
If the person is in immediate danger or seriously injured, contact the local emergency service. RAINN advises calling 911 in the United States; UK government guidance says to call 999 in England and Wales. These numbers are location-specific, not universal.
If approaching the suspected assailant or the scene could put you or the survivor in danger, do not approach. Move to a safer place if possible and get emergency help.

2. Help them get somewhere safe and ask what they need
When there is no immediate emergency, offer calm, practical help without taking control. Ask simple questions such as, “Do you want me to stay with you?”, “Would you like me to contact someone you trust?” or “Would you like help getting somewhere safer?” Let them choose what feels right when they are able to decide.

Believe them and listen. They do not have to tell you details. Avoid asking why they did not resist, leave, or report. NHS guidance for supporting someone who has been sexually assaulted recommends listening without asking for details and respecting their decisions: NHS: Help after rape and sexual assault.
3. Offer medical care without diagnosing
Encourage them to speak with a qualified medical provider about injuries and other health concerns. Some injuries may not be visible, and medical care can address concerns such as infection or pregnancy. Do not try to assess or treat injuries yourself; if they may need urgent care, prioritize emergency medical help.

4. Explain specialist support and forensic options without pressure
A sexual-assault specialist service can explain what care is available locally, including whether a forensic exam is an option. In England and Wales, sexual assault referral centres (SARCs) can provide crisis care, medical and forensic examinations, emergency contraception, STI testing, and onward emotional support. A forensic exam may help collect evidence, but it is the survivor’s choice. Medical help should come first when it is urgently needed. If they have washed or changed clothes, do not tell them it is too late to seek help: NHS guidance says someone can still seek police help after washing. A specialist can explain local options and any relevant timing considerations. 5. Treat reporting as a choice, while recognizing local rules
Do not tell the person they must report. UK guidance says people in England and Wales can access SARC and independent sexual violence adviser (ISVA) support without reporting to the police. That does not establish the rules everywhere, and it is not a promise that every service or situation is confidential or exempt from reporting requirements. Local laws and circumstances matter.

A friend or relative may be able to contact a support service on the survivor’s behalf, but service rules vary. The UK government describes routes for third parties here: Support following a rape or sexual assault. If a child or another person may remain at risk, local safeguarding and mandatory-reporting rules may apply; check with the relevant local service rather than assuming a rule.

Offer to check in later, accompany them to an appointment, or help contact someone they trust. Ask what kind of follow-up feels welcome, and respect it if they prefer space. The World Health Organization’s clinical handbook describes care as addressing immediate emotional and physical needs, ongoing safety, and continuing support and mental-health needs; its guidance is framed for women and does not directly address men or people under 18: WHO, Health care for women subjected to intimate partner violence or sexual violence: a clinical handbook.
