Before using an online acne service, ask who will assess you, how they will evaluate your skin and medical history, what treatment they recommend and why, and how follow-up and prescriptions work. In the United States, check that the clinician is qualified and licensed where you are located, and ask how the service will arrange in-person care if an online assessment is not enough.
Who will review your case?
Ask for the clinician’s name, professional role, credentials, and state license. Find out whether a board-certified dermatologist will assess your case or direct and supervise your care. The American Academy of Dermatology (AAD) recommends consulting a board-certified dermatologist licensed in your state; requirements and service availability can vary by state. See the AAD’s guidance on getting quality telemedicine care and its teledermatology standards.
- Who makes the diagnosis and treatment decision?
- What are their credentials, and where are they licensed?
- Is a board-certified dermatologist involved in directing or supervising care?
How will the service assess your skin?
Ask whether a clinician will review clear photos, speak with you live, or use both. Find out what happens if the images are unclear or the information provided is not enough to assess you safely. The AAD says that without photos, it can be difficult for an online clinic to determine what is happening and make an accurate diagnosis. Its consumer guidance specifically suggests asking whether you can submit photos.
Photos are only one part of an assessment. Acne can resemble other conditions, including hidradenitis suppurativa, perioral dermatitis, and folliculitis, which may need different treatment. Ask how the clinician handles uncertainty and decides whether an in-person examination is needed. The AAD describes diagnosis as an examination of breakouts and discusses conditions that can look like acne in its acne diagnosis and treatment guidance.

What history will the clinician ask about?
A useful intake should give you room to explain your symptoms and relevant health history, not just upload photographs and select a treatment. The National Institute of Arthritis and Musculoskeletal and Skin Diseases says clinicians may ask about symptom duration, family history, menstrual cycles where relevant, current or recently stopped medicines, and examine the skin to identify lesion types. Sometimes they may order laboratory tests to check whether another condition is contributing.
- How long have the symptoms been present, and have they changed?
- What treatments have you tried, and what happened?
- What medicines do you take or have you recently stopped? What allergies or relevant health conditions should the clinician know about?
- How will the clinician distinguish acne from another condition?
Ask whether you can obtain a copy of the consultation record to share with your primary care clinician or dermatologist. The NIAMS acne diagnosis and treatment guidance explains the kinds of information a clinician may consider.

What treatment is proposed, and why?
Ask what the recommended treatment is intended to address, why it suits your case, and what side effects, precautions, or interactions matter for you. Treatment is individualized: the option that works for one person may not be appropriate for another. A clinician may discuss topical or oral treatments; adapalene, for example, is a nonprescription retinoid available online and in stores, but that does not make it the right choice for every person.
The AAD says it can take at least 6 to 8 weeks to start seeing fewer breakouts with an acne treatment plan. That timeframe is not a reason to wait through worsening symptoms or a possible adverse reaction. Ask when to expect a check-in, what changes should prompt you to contact the clinician sooner, and how to get help if treatment causes a problem.

How do prescriptions, pharmacies, and costs work?
Before paying, ask what the service charge covers and whether medication or pharmacy costs are separate. No single price or billing arrangement applies to all online services, so request the total expected cost and clarify what happens if the clinician does not prescribe medication.
- Where will the prescription be sent, and can you choose your pharmacy?
- How can you speak with a licensed pharmacist about directions, side effects, or other medication questions?
- What record of the prescription and consultation will you receive?
The Federal Trade Commission (FTC) advises consumers to check unfamiliar online sellers. Its consumer guidance, which displays a September 2011 date, says reputable U.S. pharmacy websites should require a prescription, make a licensed pharmacist available to answer questions, and provide a physical business address and phone number. You can also check with the board of pharmacy in the state where the pharmacy is based. Read the FTC’s guidance on buying health products and services online.

What follow-up and in-person care are available?
Ask what follow-up is included, how quickly you can contact the clinician, and what to do if your skin worsens or you have a problem with treatment. Find out how the service will arrange in-person care if the online evaluation is inconclusive, your condition needs an examination, or you prefer to be seen in person. Not every online clinic offers in-person follow-up; the AAD recommends that teledermatology platforms provide access to in-person dermatology when needed or preferred.
The AAD’s teledermatology standards say adequate history and physical examination should be collected before medication is prescribed. Ask the service how it meets that standard in its process, especially if the evaluation is asynchronous or based mainly on images.

What if isotretinoin is suggested?
Ask why isotretinoin is being considered, what monitoring applies, and what pregnancy-prevention safeguards and program steps are required for you. The medicine is used for severe nodular acne that has not cleared with other treatments and has serious birth-defect risks. It is subject to the FDA-required iPLEDGE Risk Evaluation and Mitigation Strategy (REMS).
The FDA approved changes to iPLEDGE on February 9, 2026. These include options affecting where some pregnancy tests may be performed, removal of a waiting period in a specified repeat-test situation, and removal of monthly counseling documentation for patients who cannot get pregnant. Requirements depend on individual circumstances and can change, so confirm your current obligations with the prescriber and the official FDA iPLEDGE questions and answers.
