Human-in-the-loop infrastructure automation uses software to prepare or carry out infrastructure changes while a person reviews, approves, rejects, or takes control at selected decision points. In infrastructure as code (IaC), the clearest example is reviewing a proposed plan before it is applied. For AI agents, human approval can add oversight to consequential actions—but it is not a substitute for narrow permissions and controls enforced by the infrastructure itself.

What human-in-the-loop means for infrastructure

Human-in-the-loop (HITL) infrastructure automation is a workflow design, not one standardized product or protocol. It combines automated work with deliberate points where a person can make or influence a decision. In an IaC workflow, the automation can calculate the resource changes implied by a configuration; a reviewer then checks whether those changes match the intended outcome before they are applied.

In newer agentic workflows, an AI agent may reason across tools and propose or perform actions. A human checkpoint can be useful for a consequential decision, but the agent should not be the authority that decides whether it is allowed to perform that action. NIST advises establishing a strong identity foundation for agentic AI, while AWS recommends deterministic controls outside the agent’s reasoning loop.

How an approval-gated infrastructure change works

  1. Author the change. A person updates the infrastructure configuration, typically as part of a version-controlled change.
  2. Generate a plan. The IaC tool previews the resource operations implied by the configuration, such as creation, update, or deletion.
  3. Run automated checks. Validate the configuration and apply policy checks before asking a reviewer to decide.
  4. Present the change with context. Show the proposed resource changes and relevant check results to an authorized reviewer, rather than asking for approval without showing what will happen.
  5. Approve or reject. Approval authorizes the planned change within the workflow; rejection should stop it and leave a clear record.
  6. Apply and record the result. Execute the authorized change and retain the outcome for later investigation.

HashiCorp describes Terraform plan review in team and pull-request workflows, including speculative plans for review and a concrete plan presented to a team before apply in HCP Terraform.

What a Terraform plan does—and does not—approve

A Terraform plan previews proposed infrastructure changes so a reviewer can compare the expected resource operations with the author’s intent. That makes the plan reviewable evidence of what Terraform proposes to do. It does not, by itself, establish that the person approving it has the right identity, that the change is safe, or that the artifact later executed is the one that was reviewed.

Terraform supports saved plans for automation. HashiCorp documents that applying a saved plan executes it without an additional interactive approval prompt. Therefore, the workflow must control access to the plan artifact and to the ability to apply it. A separate approval step is meaningful only if it authorizes the exact plan that is executed.

Where to put human approval gates

Place approval at boundaries where a mistaken or unauthorized action could have significant consequences. The right boundary depends on the operation’s likely impact and blast radius, not on a rule that every automated step needs a person’s click.

  • Match review to consequence. Reserve human decisions for actions whose impact warrants attention; routine, well-bounded steps may be handled by automated checks and scoped permissions.
  • Make the decision informed. Give reviewers the readable plan, policy results, and enough context to understand the change.
  • Preserve separation of duties. Use identity and authorization rules to determine who can approve and who can apply; do not rely on the agent’s own judgment to enforce access.
  • Define failure behavior. Decide what happens when a reviewer rejects, does not respond before a timeout, or an apply fails. Keep a record that supports investigation.
  • Watch reviewer workload. Approval prompts that arrive too often can become routine clicks rather than meaningful checks.

AWS recommends human final decisions for high-consequence actions and cautions that requiring approval for every action can overwhelm reviewers. NIST likewise warns that excessive approval requests can create consent fatigue and lead people to approve reflexively. Approval is not a security boundary

A human click should be one layer in a control design, not the only thing preventing an unsafe operation. NIST warns that agents with broad access may take unexpected paths and cause unintended damage. AWS similarly argues that security must be enforced through deterministic, infrastructure-level controls outside an agent’s reasoning; prompts and model behavior are not reliable authorization mechanisms.

  • Give the automation only the permissions it needs for its assigned work.
  • Enforce authorization outside the agent, using the identity and access controls of the infrastructure or workflow.
  • Avoid credential sharing, static tokens, and overly broad access, as highlighted in NIST’s guidance.
  • Keep a useful record of the proposed action, decision, actor, and execution result.

AWS states: “Organizations should enforce security through deterministic, infrastructure-level controls external to the agent’s reasoning loop, not through the agent’s own reasoning, internal guardrails, or prompt-based instructions.” (AWS Security Blog.)

Choosing an approval pattern

Before adopting an approval mechanism, compare how it handles the control questions that matter in your environment:

  • Consequence and blast radius: What could the operation change, and how difficult would recovery be?
  • Review evidence: Can the reviewer inspect a readable plan and policy results?
  • Artifact alignment: Is the plan approved the same artifact that will execute?
  • Identity and permissions: Are approval and apply rights scoped and assigned to the right people or systems?
  • Auditability: Can the team establish what was proposed, approved, and done?
  • Latency and workload: Will the review process delay urgent work or produce so many prompts that people stop evaluating them?
  • Fallback behavior: What happens on rejection, timeout, or failed execution?

Terraform and agentic workflows are different cases

Terraform plan review

Terraform plan review is a direct IaC example: the tool previews resource changes and the team can review the proposed plan before apply. HashiCorp’s automation workflow documentation describes speculative plans for review and HCP Terraform’s concrete plan approval flow. For automated execution, keep control of the saved plan and the authority to apply it, as described in Terraform’s apply command documentation.

Agentic intervention patterns

AWS Nova Act documents human intervention patterns for autonomous web workflows, including binary or multiple-choice approval and live UI takeover. Its documentation says the capability is implemented in the SDK rather than offered as a managed AWS service; teams can deploy a Human Intervention Service package in an AWS environment or build a custom interface. It also discusses timeouts, rejection handling, supervisor notifications, and interaction logs. This is an illustration of ways to design human intervention for autonomous workflows, not a Terraform or general IaC approval product. See AWS Nova Act’s human intervention documentation.

How much autonomy should the workflow have?

Start with review and controls appropriate to the operation’s consequence, then adjust autonomy based on ongoing evaluation of how the workflow performs. AWS recommends expanding autonomy deliberately while retaining durable constraints where the consequences justify them. A useful design does not maximize approval prompts or eliminate them; it assigns people decisions they can meaningfully assess and uses technical controls to enforce the boundaries that should not depend on judgment.
