In the United States, going without health insurance usually means you are responsible for more of your medical costs yourself. It may also make routine or preventive care harder to afford. You generally cannot be turned away from emergency screening and stabilizing treatment at a covered hospital emergency department because you lack insurance, but you can still be billed afterward. The federal tax penalty for being uninsured is gone; some states have their own coverage rules.

What being uninsured changes

Without an insurer paying part of covered costs or applying negotiated plan rates, you may have to work directly with providers to understand prices and arrange payment. The amount you owe depends on the care, provider, location, and any financial assistance available. Uninsurance does not automatically prevent you from receiving routine care, but cost can make it harder to obtain it.

In 2024, 8.2% of people in the United States—27.2 million people of all ages—were uninsured; the rate among adults aged 18–64 was 11.6%, according to the National Center for Health Statistics’ 2025 report. Can an emergency room turn you away without insurance?

At most hospital emergency departments that receive Medicare funding, federal EMTALA rules require an appropriate medical screening exam regardless of insurance or ability to pay. If the hospital finds an emergency medical condition, it must provide stabilizing treatment or arrange an appropriate transfer. This does not make the visit free: the hospital can bill you afterward. Do not delay emergency care because you are uninsured. EMTALA applies to screening and stabilization at covered hospital emergency departments; it is not a promise that every type of provider or non-emergency service is free.

Will you get fined for not having health insurance?

The federal tax fee for not having health coverage no longer applies. Some states have had their own coverage requirements or fees, so check the rules for your state and the tax year in question. HealthCare.gov: fee for not having coverage State coverage mandate overview

How to manage a medical bill without insurance

For scheduled care, ask for the price in writing before treatment. Under CMS guidance, uninsured or self-pay patients can usually request a good faith estimate, and providers generally must give one when care is scheduled at least three business days in advance. An estimate is not a bill. It currently covers expected charges for a single provider or facility, so request separate estimates from each provider and the facility involved.   1. Before non-emergency care: Tell the provider you are uninsured or will pay without using insurance. Ask for a written estimate and whether financial assistance or a self-pay rate is available.
  2. Request estimates from everyone involved: Ask separately for the facility and each provider. Scheduling at least three business days ahead generally triggers the provider’s estimate requirement.
  3. Check the bill: Compare it with the estimate, confirm that the services and quantities are correct, and contact the billing office about errors or assistance options.
  4. Check whether a dispute process applies: If a provider or facility bills at least $400 more than its estimate, you may be able to use CMS’s patient-provider dispute process. CMS says you must start within 120 days of the initial bill. Eligibility depends on details including whether you told the provider before care that you would not use insurance and received the estimate at least three days before the scheduled appointment. What happens to routine and preventive care?

Being uninsured does not mean you cannot get routine care, but paying the full charge may lead some people to delay or go without needed services. Community health centers are one place to look for care at a cost tied to income. HealthCare.gov says these centers offer services on a sliding-fee scale based on income. In 2020, preventive care accounted for 33.4% of community health center visits, visits for new problems for 32.8%, and visits for chronic problems for 32.1%. How to look for coverage

Check more than one route. Eligibility, enrollment timing, monthly cost, deductibles, out-of-pocket exposure, and provider and prescription networks can differ. Compare the options available to your household rather than assuming one route will fit.

  • Marketplace coverage: Apply through the Marketplace to see which plans and financial assistance may be available to you. Plan options and enrollment eligibility depend on your circumstances.
  • Special Enrollment Period: Certain life changes can let you enroll outside the annual open enrollment period. HealthCare.gov says losing qualifying coverage within the past 60 days, or expecting to lose it within the next 60 days, may qualify.   • Employer or family coverage: Ask whether a job-based plan or a family member’s plan is available and when you could enroll. If you have recently lost job-based coverage, investigate whether continuation coverage applies to your situation.
