Healthcare can adapt a few engagement ideas associated with sports betting—especially personalized feedback, clear progress cues, and easy-to-find self-management tools—but it should not copy betting products’ goal of sustaining use. The useful test is whether an approach supports a patient’s health goals and autonomy, not whether it increases clicks, sessions, or time in an app. Evidence from gambling research offers design clues, not proof that those tactics improve healthcare outcomes.

What should healthcare borrow—and what should it avoid?

The comparison is about engagement methods, not equivalent goals. A betting product may seek continued commercial use; a healthcare tool should help people take meaningful steps toward their own health goals. That difference changes what counts as success and where personalization becomes risky.

Design question Healthcare priority Why the distinction matters
Goal Patient benefit and self-management More activity is not automatically better if it does not help the patient.
Outcome Meaningful health or protective behavior Clicks and time in an app are engagement measures, not proof of health improvement.
Personalization Relevance to an individual’s needs Targeting should support informed choice, not exploit vulnerability.
Safety and autonomy Clear choices, appropriate limits, and an easy way to stop Healthcare should not add friction merely to prolong use.
Delivery context Fit with clinical or community support and available resources A digital prompt cannot replace trust or practical help.

These are useful design questions, not a formally validated framework. The available studies do not directly test transferring commercial sportsbook retention mechanics into clinical care.

Can personalized feedback help people take useful next steps?

A randomized Canadian online trial offers a relevant, but bounded, example. Tabri, Wood, and Wohl studied 4,091 adults who had gambled online or at land-based casinos in the previous three months. Participants were assigned to personalized and normative feedback (1,940 people) or no feedback (2,151); outcomes were assessed after randomization and again at three months. The trial was about responsible gambling, not healthcare patients or sportsbook retention.

Compared with no feedback, participants receiving personalized and normative feedback showed higher gambling insight and stronger intentions and self-efficacy for learning how games and odds work and using operator-provided limit-setting tools. Reported effect sizes were d = 0.32 for gambling insight, d = 0.31 for intentions to learn how games work, and d = 0.30 for intentions to learn odds. The study did not find corresponding differences in intentions and self-efficacy for limiting time or money, being open with others, or balancing gambling with other activities. Behavioral increases at three months were not statistically significant. Read the trial abstract and record.

For healthcare, the reasonable design hypothesis is to show feedback that is relevant to a person and make a beneficial next step easy to find—for example, a self-management tool that directly supports the person’s plan. The trial does not establish that the same approach improves medication adherence, clinical outcomes, or other health behaviors. Those outcomes need to be measured in healthcare settings.

Why a well-written message may not change behavior on its own

A five-week experiment tested safer-gambling messages for sports and race betting. It found no significant differences across message conditions in commercial betting amounts, time spent gambling, gambling harm, urges, or risk-related beliefs. A prompt may attract attention without changing consequential behavior. See the experiment’s abstract.

Message preferences may also differ among groups. A review and focus-group study examined responsible-gambling message archetypes and reported variation across cohorts, including people who bet on skill-based games such as sports. This supports testing whether a message is relevant to its intended audience; it does not show that personalization will reliably change behavior. Read the review and focus-group study.

Why trust and delivery setting matter

The Football Fans and Betting (FFAB) report describes an eight-week early intervention delivered through community-trust coaches at Leicester, Preston North End, Accrington Stanley, and Blackpool football clubs. Its authors identify shame and stigma as barriers to recruitment. Smaller, locally rooted clubs made recruitment easier, but had fewer resources and greater retention challenges. They recommend public-health education before recruitment and more localized, grassroots delivery. Read the FFAB intervention report.

For healthcare, the practical implication is that engagement depends on more than interface design: the setting must feel credible, people need a reason to participate, and the delivery team needs capacity to support them. FFAB is an early intervention report, not definitive evidence that community delivery improves clinical outcomes.

Where gamification fits—and where the evidence stops

Healthcare gamification can include progress levels, points, rewards, narrative or theme, personalization, and customization. These are design options, not treatments with established effects. A systematic review of gamification in healthcare management found a shortage of systematic, structured quantitative evidence connecting these mechanics to motivation, engagement, and behavior change. Read the systematic review.

Use a game-like element only when it clarifies progress or makes a useful action easier. Before treating it as successful, assess whether it leads to the behavior or health outcome that matters—not merely repeated app use. The distinction is especially important when rewards or reminders could pressure someone to continue beyond what is useful.

How to evaluate a healthcare engagement idea

Because the transfer from betting to healthcare has not been directly tested, treat each borrowed mechanic as a hypothesis and evaluate it in the setting where it will be used.

  1. Define the patient-benefit goal. Specify the health-related action or outcome the tool is meant to support, rather than setting app activity as the end goal.
  2. Choose feedback that helps the person decide. Make information relevant and understandable, and connect it to a practical next step without implying that a prompt alone will change behavior.
  3. Keep the interaction voluntary. Make choices, limits, and ways to disengage clear; do not use retention friction to keep someone active.
  4. Fit delivery to the context. Consider trust, stigma, recruitment, and staff or community capacity alongside the interface.
  5. Measure the intended result. Track meaningful behavior and, where appropriate, health outcomes. Treat clicks, sessions, or time in app as secondary indicators rather than proof of benefit.

The responsible-gambling studies support limited lessons about feedback, messages, and delivery context. They do not establish that sportsbook-style engagement strategies improve healthcare outcomes, nor that points, rewards, or more frequent prompts are effective treatments.
