Plague can cause fever and severe illness, with symptoms that vary by form. A painful, swollen lymph node is common in bubonic plague, but it may be absent in septicemic or pneumonic plague. If you have compatible symptoms after a possible exposure, seek medical attention immediately; do not wait for a test result.
What are the symptoms of plague?
Plague has three main forms: bubonic, septicemic, and pneumonic. Fever, chills, headache, and weakness can occur across forms, but the most noticeable symptoms differ. Symptoms alone cannot confirm or rule out plague; possible exposure and prompt medical evaluation matter.

| Form | Typical symptom pattern | How it may occur |
|---|---|---|
| Bubonic | Fever, headache, chills, weakness, and one or more swollen, painful lymph nodes called buboes. They commonly appear in the groin, armpit, or neck. | Often follows an infected flea bite. CDC gives a typical incubation period of 2 to 8 days after the bite. |
| Septicemic | Fever, chills, extreme weakness, abdominal pain, shock, and possible bleeding into the skin or organs. Tissue, especially on fingers, toes, or the nose, may turn black and die. | Can begin as this form or develop from untreated bubonic plague. A bubo may not be obvious. |
| Pneumonic | Fever, headache, weakness, rapidly developing pneumonia, shortness of breath, chest pain, cough, and sometimes bloody or watery mucus. | Can follow spread from untreated bubonic or septicemic plague, or inhalation of infectious droplets. CDC describes an inhalation incubation period as short as one day on its public symptom page and usually 1 to 3 days in clinical guidance. |
Pneumonic plague is the most serious form and the only form that can spread from person to person, through respiratory droplets.
When should you seek medical care?
Seek medical attention immediately if you have symptoms that could fit plague, particularly if you live in or recently traveled to an area where plague occurs, including parts of the western United States, or have had a recent flea bite, been in rodent areas, or handled a sick or dead animal. CDC’s symptom guidance says to seek immediate medical attention for symptoms like those it lists.

Plague is serious but treatable. CDC advises starting appropriate therapy as soon as plague is suspected, without waiting for laboratory results. Clinicians decide whether plague is likely, which tests and treatment are appropriate, and which antimicrobial to use based on illness severity and individual factors. CDC’s clinical testing guidance, dated April 6, 2026, emphasizes not waiting for diagnostic test results when plague is suspected.
