In a small experiment, feeding an incident-response agent 104 real outage postmortems helped it match the root-cause category in nine of ten held-out cases, according to the author. The most distinctive benefit was recalling not just failure patterns but failed fixes—“trap actions”—that can make an outage worse. The result is promising, but it is a small, self-graded test, not proof that memory makes incident advice reliable.
What the experiment did
In a September 29, 2026, DEV Community article, Kudikala Saikeerthika describes building an incident-response agent around the OpenSRE incident dataset. The dataset contained 114 postmortems associated with Slack, Cloudflare, GitHub, AWS, Datadog, CircleCI, and LaunchDarkly. Each incident had a true_category root-cause label. The author retained 104 incidents for the memory bank and reserved ten for testing. Read the author’s account on DEV Community.
The article reports that Hindsight converted the 104 retained incidents into 759 world facts, five experiences, and 182 observations: 946 memories in all, connected by 7,135 links. Those are figures reported by the author, not independently verified telemetry.
What the agent could recall that a simple checklist might miss
The central idea is to preserve the history of attempted fixes, including cases where a seemingly sensible intervention deepened the problem. The author calls these failed remediations “trap actions.” Examples include a rollback that retriggers a failure, a restart that erases state needed for recovery, and scaling that sends more traffic to an already saturated dependency.

This is a useful distinction for incident response: identifying a plausible cause is only part of the job. An agent that retrieves a similar incident’s failed remediation may help responders avoid repeating a costly mistake. But similarity is not proof that the current incident has the same cause, and recalled advice must be checked against present-day telemetry and system behavior.
How trap actions were surfaced—and why it is not a safety system
The implementation used a simple lexical reranking rule alongside a prompt instruction. A memory’s ranking began at 0.5, gained up to 0.3 for query-term matches, and gained 0.2 if the recalled text literally contained “trap.” The prompt told the model to say “DO NOT do X” when retrieved context described a trap.

That approach can make relevant warnings more visible, but its keyword trigger is crude: a failed remediation described without the word “trap” may not receive the boost. Nor does a prompt guarantee that a model will identify, interpret, or follow every warning. The author characterizes the mechanism as a nudge, not a guarantee.
What happened in the rollback example
The author compared responses to the same hypothetical query: checkout errors near 12% after a 06:31 deploy, with the operator asking whether to roll back. The no-memory response allegedly invented a NullPointerException, a promoCode field, 112 log occurrences, and a nonexistent Helm revision, then recommended an immediate rollback.

The memory-backed response raised Redis or database connection-pool exhaustion as possibilities and warned that rollback could be a trap for that failure class. It also suggested checking BGP and systemd-networkd changes—unrelated retrieval bleed-through, in the author’s account. The example shows both sides of retrieval: it can surface a potentially useful caution, yet also add irrelevant leads. It is a hypothetical symptom-query comparison, not a live-incident trial.
How to read the 9-of-10 result
For ten held-out incidents, the author wrote symptom-only queries and compared the memory-backed condition with the same model and prompt after removing the memory block. The reported outcomes were:

The author graded answers against the dataset’s true_category, without a second grader. “Correct” therefore means category-level agreement, not a faithful reconstruction of the original incident or proof that the proposed diagnosis would guide a safe resolution. With ten cases, each outcome can move the apparent success rate substantially.
What the experiment does not establish
- General reliability: Ten self-graded cases do not establish that memory makes incident-response agents dependable across organizations, systems, or outages.
- Performance on genuinely novel failures: A held-out postmortem may still resemble incidents in the memory bank, because outages recur in recognizable classes.
- Real-time incident performance: Postmortems are curated after the fact; live incidents are less orderly and may have incomplete or changing evidence.
- Whether real data is better: The author did not measure real postmortems against synthetic or hand-written data, and did not compare other data sources.
- Safe remediation: The test evaluated category matches, not whether following recommendations would avoid harm in production.
Should you roll back after a deploy causes errors?
Not on the basis of a retrieved postmortem or an agent’s confident-sounding answer alone. Treat a similar incident as a hypothesis generator: establish what changed, inspect current logs and telemetry, check whether rollback is safe for the affected state and dependencies, and compare the expected risks of rollback with other recovery options. A recalled warning can prompt a check; it cannot determine whether the warning applies to this deployment.
