Utah has not given AI a general license to examine patients or prescribe medication. Instead, the state uses temporary agreements for named products and narrowly defined tasks. Its October 5, 2026 announcement described pilots for acne care, pelvic-health physical therapy and chronic-medication refills, but the arrangements differ in how they use clinicians. In the refill process Utah describes, a physician reviews each recommendation in the first phase; direct AI-to-pharmacist refills would require the pilot to meet benchmarks and receive state approval.
What Utah’s AI healthcare sandbox allows
Utah’s Office of Artificial Intelligence Policy, within the Department of Commerce, coordinates regulatory-mitigation agreements with the agency responsible for a regulated field. For prescribing pilots, Utah says the Division of Professional Licensing must review and approve the arrangement. An agreement temporarily allows a named product to perform a defined task under specified conditions; it does not rewrite the underlying rules or grant an AI company a professional medical license. Utah also says participation is not state endorsement.
The October 5, 2026 announcement added healthcare as a pillar of the state’s Pro-Human AI initiative and named five agreements: two umbrella agreements that create expedited review pathways for future, individually vetted pilots, and three agreements describing specific clinical pathways. The umbrella agreements are with University of Utah Health and Intermountain Health. Details of any future pilot would depend on its own review and terms.
What the announced pilots cover
The October announcement describes three task-specific pilots. It does not provide enough operational detail to establish each pilot’s current phase, enrollment, or real-world outcomes, so the descriptions below reflect the stated scope rather than proof of autonomous operation or demonstrated safety.

The state says participating organizations are subject to multi-layered oversight, patient disclosure of AI involvement, specialty medical malpractice insurance, 24-hour incident-response plans and preservation of traditional legal remedies. Those are program-level safeguards described in the announcement, not evidence that any individual pilot has already met a particular safety or performance threshold. Zach Boyd, director of the Office of Artificial Intelligence Policy, said, “Our foundational approach is that technical innovation will never supersede patient safety.”
Can AI prescribe medication in Utah?
Utah’s prescription pilots are described as refill pathways for medicines previously ordered by a licensed physician, not a way for AI to issue a new prescription or change a treatment plan. The state’s description of the Doctronic pilot says it processes 30-, 60- or 90-day renewals for previously prescribed medications and excludes controlled or addictive substances. The Utah FAQ says pilot formularies vary and are published in their agreements.

How the refill stages work
- Initial phase: AI recommends whether a refill is appropriate, and a licensed physician reviews each recommendation before it reaches a pharmacist.
- Possible later phase: Only after the pilot meets its agreement benchmarks and receives state approval may AI send a refill directly to a pharmacist.
- Escalation: A pharmacist may refer a refill to a licensed physician. Utah also describes escalation when a case exceeds thresholds or information conflicts.
The later-stage pathway is conditional; the state’s description does not establish that a specific pilot has reached it. Utah says participating companies must employ a licensed physician whose name appears on AI-generated refills, and maintain malpractice coverage for AI-related risk or self-insure.

What if the AI gets a refill decision wrong?
Utah’s FAQ describes several controls intended to detect or limit problems: identity and prescription verification, exclusion of controlled substances in the pilots it describes, limits on the number of refills before a clinician visit, escalation rules, monthly company reporting, and state verification and audits. Utah says the agreements do not waive other legal obligations or affect patients’ legal remedies.

The state has agreements with six independent evaluator organizations that may review proposals and audit pilots. Utah describes evaluator work such as clinical-error testing, privacy review, sampling live system activity and checking company reports. It cautions that a tamper-evident receipt shows that a specified safety check ran; it does not prove that an AI system is safe or that a company complied with the law.

- Is care through the AI pathway not inferior to conventional care?
- Does the pathway cause substantial bodily or psychological injury?
- Could it improve clinical quality, geographic access or affordability for Utah residents?
The standards call for voluntary, informed participation, disclosure that AI is involved and disclosure that authorization is temporary under the sandbox. They are provisional and iterative, not a blanket finding that a particular product is safe or effective.
Why the Utah Medical Licensing Board objected to Doctronic
In an April 20, 2026 letter to the Office of Artificial Intelligence Policy, the Utah Medical Licensing Board said it learned of the Doctronic agreement only after implementation, when the system was already live. The board argued that even renewing an existing prescription can require reassessment of dosage, side effects, contraindications, interactions and treatment effectiveness. It wrote: “It is the strong recommendation of the Utah Medical Licensing Board that this program be immediately suspended pending further discussion.”

That quotation is the board’s formal position, not a neutral finding that the pilot caused patient harm. It also highlights a substantive policy concern: a refill may appear routine while still involving clinical judgment.
What is and is not established about current pilot status
The October announcement gives the new pilots’ stated scopes but not their precise operating phases, patient totals or real-world performance. Do not treat an announced stage gate as proof that a pilot has passed it, or a pathway description as evidence of safety outcomes.
Utah’s Doctronic page described an agreement term from October 2025 through October 2026. The available state information does not settle whether that agreement was renewed after its stated term. For any current eligibility or operating status, the signed agreement and subsequent official updates are the relevant records.
