For the short route, return the box. To unlock the full quest, inspect the cart evidence and challenge the supposed merchant. That starts a chase and leads to a choice about the deserter and the medicine: hand him over to the Nilfgaardians, let him leave with it, or keep it. The quest references do not establish a lasting effect on the wider war or game ending.
Should you return the box or challenge the merchant?
Near the Nilfgaardian garrison in White Orchard, a man claiming to be a merchant asks Geralt to recover a small box from a cart in a swamp. The arrow wounds and dead driver at the scene contradict his account.
- Return the box: Accept the man’s story and give him the box. This ends the quest on its short branch. Game8 reports a reward of 25 XP and 10 crowns for this outcome.
- Challenge him: Examine the driver or the arrow-struck horse, then choose the dialogue option “Seen through your lies.” The man flees on horseback, opening the longer branch and its later decisions.
How to stop the rider
Catch the fleeing man before he reaches his associates. The walkthroughs describe knocking him from his horse with a crossbow bolt, bomb, Aard, or a sword hit. If he makes it to the bandits, Geralt must fight them; failing to stop the rider can fail the quest.
What should you do with John Geermer and the medicine?
Once caught, the man identifies himself as John Geermer, a deserter associated with the Temerian Sixth Division. He says the box contains medicine taken from a Nilfgaardian transport. The final decision is a role-playing choice rather than a documented way to change the larger story.

- Trained from early childhood and mutated to gain superhuman skills, strength, and reflexes, witchers are a counterbalance to the monster-infested world in which they live.

- Take on the most important contract of your life: to track down the child of prophecy, the key to saving or destroying this world.
| Choice | What it means | Reported immediate outcome |
|---|---|---|
| Take Geermer and the medicine to the Nilfgaardians | Turn the deserter and the recovered medicine over to the garrison. | Immediate cash and XP rewards differ from the other outcomes; the reference does not state a specific amount here. |
| Let Geermer go with the medicine | Release him and allow him to keep the medicine. | Immediate cash and XP rewards differ by outcome; the reference does not state a specific amount here. |
| Keep the medicine | Take the medicine for Geralt rather than letting Geermer keep it. | Witcher.tools lists up to 50 crowns and five celandine plus white myrtle petals for this outcome. |
The available references do not establish that any of these decisions changes the wider war or the game’s ending. Choose based on how you want Geralt to treat a deserter and the medicine.

WitcherDB marks the quest as missable and reports that it can become unavailable when Geralt leaves White Orchard for Vizima after “Lilac and Gooseberries.” If you are completing the region’s quests, finish Precious Cargo before advancing out of White Orchard.
