In a project account published September 29, 2026, developer Kunduru Bhavi describes pairing a live queue with a separate memory service for visit summaries—and finding that a text-sanitization step had silently changed a patient’s recorded words before they were saved.
The implementation lesson is broader than the specific tools: keep workflow state separate from longitudinal context, preserve the source wording, and treat recalled history as supporting information that people must verify and interpret—not as a diagnosis or proof of clinical benefit.
What the queue tracks—and what it cannot remember
Bhavi describes a web application built with React, an Express API, Mongoose, MongoDB, and Hindsight. MongoDB holds the live queue records and workflow state. Hindsight is used separately to retain and recall visit context. When the doctor stage is completed, the application builds a dated, labeled visit summary for retention; when a returning patient reaches the doctor, the app requests earlier context for that encounter.
That division matters. The queue answers operational questions such as who is next and where a patient is in the process. The memory service addresses a different question: “What happened last time, and is any of it relevant now?” Bhavi’s example of asking what happened “last spring” illustrates the value the feature is meant to offer, but the account is an implementation description, not evidence that memory improves care.

The distinction is architectural, not a claim that the two systems are inherently safe or clinically validated. The application still needs sound identity, access, retention, and review practices around any health-related information it handles.
Why “chest pain & dizziness” became “chest pain & dizziness”
The reported defect was not a failed queue operation or a failed memory lookup. Bhavi says an intake sanitizer called validator.escape encoded the ampersand in the complaint “chest pain & dizziness” before the text was stored. The altered version then appeared in stored and recalled content as & in HTML-escaped output, rather than preserving the patient’s original wording.
The safer principle is to preserve the submitted text as the source record, while validating it and encoding it for the specific output context when rendering. HTML encoding is still important where HTML is produced; it should not be used as a permanent rewrite of the source text at intake. The account also mentions checking input type and length and guarding against MongoDB operator injection—separate concerns from output encoding.

- At intake: validate expected type and reasonable length; do not mutate the recorded wording merely to prepare it for HTML.
- At output: encode for the destination and context, such as HTML text, rather than assuming one encoding is safe everywhere.
- For existing altered records: repair only when a trustworthy original is available. Blindly decoding stored strings can create a different integrity error if the text was intentionally encoded or has passed through multiple transformations.
This is a data-integrity issue as well as a display issue: once a transformed string is treated as the source, later summaries and recalls can carry forward the distortion.

Bhavi says the implementation derives a separate Hindsight memory-bank name for each patient from the patient’s database ID. This makes the intended retrieval scope explicit: a request for one patient’s history can target that patient’s bank rather than a shared undifferentiated store.
That boundary is useful, but it is not a complete privacy guarantee. A bank name does not establish that the caller is authorized to access the requested patient, that identifiers are protected, or that the system has appropriate retention and deletion procedures. Identity matching also remains critical: a newly assigned ID can hide a returning patient’s prior history, while a reused ID can mix records across people even when banks are otherwise separated.

For health-data systems, the U.S. Department of Health and Human Services summarizes the HIPAA Security Rule as requiring regulated entities to implement reasonable and appropriate administrative, physical, and technical safeguards for electronic protected health information (ePHI). Its overview discusses controls including access control, authentication, audit controls, transmission security, and protections against improper alteration or destruction. Those requirements cannot be inferred from the architecture in this project account; its jurisdiction, legal status, full controls, and compliance assessment are not established.
HL7 FHIR R5’s Security and Privacy Module is another useful design reference for health-data systems. It discusses building blocks such as access control and authorization, consent, audit logging, and provenance without prescribing one technical approach. It is a checklist context here, not evidence that this application uses FHIR or conforms to it.
Recalled history is context for a clinician, not a diagnosis
The described default recall asks broadly for past visits, symptoms, and treatment, with a token limit selected as a user-interface tradeoff. A limit can affect how much context fits on screen or in a request; it does not make the returned material complete or authoritative.

The account’s example of an earlier headache followed by later blurred vision is illustrative, not captured production output. In practice, a remembered summary may be incomplete, imprecise, stale, or associated with the wrong identity if other safeguards fail. It should be considered alongside the current encounter and interpreted by a clinician, not treated as independent verification or a system-generated diagnosis.

In Bhavi’s wrapper, failed retain calls are caught and logged, while a failed recall returns an empty array. That approach is intended to let the visit flow continue if the memory service fails. But an empty result is ambiguous: it could mean the patient has no prior visits in memory, or that the service could not provide them. A logged write failure can also leave a missing history item without making the live queue fail.
The completion route is described as awaiting the retain operation, so memory availability can still add delay even when the queue remains usable. Bhavi proposes retryable background work and a safe availability indicator as future improvements; these are proposals, not verified features of the described application.

A useful design distinction is to preserve visit completion while making the state of context visible in a way that does not mislead staff:
- Show “no prior history found” only when the lookup succeeded and returned no history.
- Represent a failed or timed-out lookup as unavailable, rather than as an empty history.
- Make a failed retention operation observable and retryable where appropriate, so an error does not silently become a permanent gap.
- Keep any availability message clear about what is known, without implying that the recalled content has been verified.
Bhavi summarizes the intended separation: “The memory service doesn’t decide who is next, and its availability shouldn’t determine whether a patient can finish a visit.”
Implementation checks worth prioritizing
Bhavi identifies several useful end-to-end checks, while noting that they are priorities rather than a claim that all have already been automated. They test the boundaries between original data, patient identity, memory retrieval, and the clinician-facing experience:
- Enter notes with ampersands and other punctuation; verify the stored source wording and the correctly encoded rendered display.
- Test missing notes and incomplete visit summaries so absent data does not become misleading history.
- Attempt patient-isolation and API-authorization failures, including requests for another patient’s history.
- Repeat a visit using a stable patient identity, then test what happens when identity is missing or changes.
- Simulate memory-service outages and distinguish unavailable history from a successful lookup with no prior visits.
- Review the recalled material for readability and whether clinicians can tell what it represents and where it came from.
There is also an environment-specific Docker detail in the account: from inside an API container, localhost refers to that container rather than the host. The described setup uses host.docker.internal with a Linux host-gateway mapping. This is relevant when configuring connectivity in that deployment shape, not a general privacy or clinical-safety measure.

Bhavi’s September 29, 2026 DEV Community article provides a first-person account of an architecture, a specific escaping bug, and handling of memory-service errors. It does not independently audit the code, establish production performance, demonstrate clinical safety, or determine legal compliance. The useful takeaway is therefore an implementation discipline: keep queue state and remembered context distinct, protect the integrity of the source text, make identity and access controls explicit, and design failures so a visit can continue without disguising missing or unavailable history.
