Sleep apnea—particularly obstructive sleep apnea (OSA)—has been linked with cognitive impairment and dementia in observational studies, but the evidence does not show that OSA causes Alzheimer’s disease or that CPAP prevents it. Loud snoring, witnessed breathing pauses, gasping during sleep and daytime sleepiness are reasons to ask a healthcare provider about assessment. If OSA is diagnosed, treatment can address the sleep disorder and its established health effects; it should not be presented as a proven Alzheimer’s-prevention therapy.

Can sleep apnea cause Alzheimer’s disease?

It is not established that sleep apnea causes Alzheimer’s disease. Studies have found an association between sleep-disordered breathing or OSA and cognitive impairment or dementia, but observational findings cannot establish cause and effect or predict an individual’s risk.

A 2024 meta-analysis of 15 cohort studies reported a pooled hazard ratio (HR) of 1.52 (95% confidence interval [CI] 1.32–1.74) for cognitive impairment and/or all-cause dementia among people with sleep-disordered breathing or OSA compared with people without it. In the subset of studies that used polysomnography to diagnose sleep-disordered breathing, the pooled estimate for all-cause cognitive impairment was smaller: HR 1.32 (95% CI 1.00–1.74). The authors noted that many studies did not use objective polysomnography-based diagnosis. These are group-level associations, not a personal forecast.

A large Danish matched cohort published in 2025 illustrates why the findings should not be reduced to one risk number. It compared 62,928 people with OSA with 62,928 matched participants; 76% of the cohort was male and the median age was 52. The HR was 1.10 (95% CI 0.98–1.24) for all-cause dementia and 1.16 (95% CI 0.94–1.43) for Alzheimer’s disease. Both confidence intervals included 1.0, so the overall results did not establish an increased risk. The study also reported subgroup differences, but those do not resolve whether OSA causes dementia.

  • The mask is made of food-grade silicone and is designed to achieve a perfect facial fit using three sizes of high elastic straps for all-night comfort. The small size and lightweight (10g) make it easier to wear and carry. Convenient to use at home or while traveling.
  • IMPORTANT NOTES Before Purchasing: 1. Can not be used with C-P-A-P mask at the same time. 2. Designed to monitor nasal breathing, not recommended for mouth breather. 3. Not for SpO2 / Pulse Rate recording. 4. Monitor your sleep breathing data (flow, AHI, snoring, etc) only, not helping you to breathe. 5. Can not replace doctor's diagnosis.

Other research has reported associations in particular groups, including people with severe late-midlife OSA, and estimates have changed after accounting for cardiovascular risks. Results vary by age, diagnostic method and study design. The evidence supports taking possible OSA seriously, not assuming that a person with OSA will develop Alzheimer’s disease.

Does CPAP prevent dementia?

No. Current evidence does not establish that CPAP prevents Alzheimer’s disease, prevents dementia, reverses dementia or slows cognitive decline. The Danish cohort reported that associations were attenuated among people treated with CPAP, but an observational comparison cannot show that CPAP caused that difference. People who receive or consistently use treatment may differ in other ways from those who do not.

Small trials have examined CPAP in people who already had Alzheimer’s disease, but they do not answer whether treating OSA prevents dementia. A 2006 randomized placebo-controlled study of 39 community-dwelling older adults with mild-to-moderate probable Alzheimer’s disease and sleep-disordered breathing reported lower subjective daytime sleepiness during therapeutic CPAP.

A 2009 randomized study enrolled 52 people with mild-to-moderate Alzheimer’s disease and OSA. Its initial three-week comparison of therapeutic and placebo CPAP did not show significant cognitive improvement. Pre- and post-treatment comparisons after therapeutic CPAP suggested some improvement, but the study was underpowered to make definitive claims about individual cognitive domains. A separate preliminary follow-up compared only five people who continued CPAP with five who discontinued it; the authors described possible benefits and called for prospective randomized trials. These small findings are hypothesis-generating, not proof of long-term protection.

CPAP is a treatment for diagnosed sleep apnea. Whether it is appropriate, and which treatment to use, should be decided with a healthcare provider based on the person’s condition and circumstances.

What are the signs of sleep apnea?

Common clues to sleep apnea include breathing that starts and stops during sleep, frequent loud snoring and waking up gasping for air. A person may not notice these events; a bed partner or family member may be the first to observe them.

Other possible symptoms include:

  • Daytime sleepiness or tiredness, fatigue, or difficulty sleeping
  • Waking with a dry mouth or headache
  • Waking often to urinate
  • Sexual dysfunction or decreased libido

The National Heart, Lung, and Blood Institute (NHLBI) notes that fatigue, headache and insomnia are among the symptoms more common in women. Symptoms alone do not confirm OSA, and not everyone will notice or report the same signs. Share them with a healthcare provider rather than using a symptom list as a self-diagnosis.

How do doctors test for sleep apnea?

A healthcare provider typically asks about symptoms, risk factors and family history. They may refer the person to a sleep specialist or sleep center for a sleep study. The study helps determine whether sleep apnea is present and, if so, its type and severity. The provider or sleep specialist can explain which test is appropriate; the details of home or center-based testing depend on the test chosen.

  1. Describe what is happening. Note snoring, observed breathing pauses, gasping, sleep disruption and daytime sleepiness. A bed partner’s observations can help if the person is unaware of events during sleep.
  2. Discuss health history and risk factors. The provider will ask about symptoms, risk factors and family history as part of the assessment.
  3. Follow the referral for a sleep study, if recommended. A sleep specialist or sleep center can arrange testing to help identify the type and severity of apnea.
  4. Use a sleep diary if helpful. NHLBI says a diary can document sleep duration and quality, as well as daytime sleepiness; it supports the assessment but is not itself a sleep apnea diagnosis.

What treatments are available for OSA?

Treatment depends on the person and their OSA. NHLBI lists positive airway pressure (PAP), lifestyle measures, oral devices, orofacial therapy and surgery among possible approaches. A healthcare provider can discuss suitability, fitting or prescription, practical use and follow-up.

Approach What it involves How its use is determined
Positive airway pressure (PAP), including CPAP CPAP supplies steady pressure to help keep the airway open; other PAP types work differently. A clinician recommends and prescribes treatment as appropriate. NHLBI says equipment includes a mask or interface and tube.
Oral device A custom appliance may be an option for some people who do not want or cannot tolerate CPAP. It should be fitted and explained by a dental professional, with clinical guidance on whether it suits the person.
Lifestyle measures Depending on the patient, clinicians may discuss regular activity, healthy weight and sleep habits, limiting alcohol or caffeine, stopping smoking, or sleeping on one’s side. These measures are discussed in the context of the individual’s care; the appropriate plan varies.
Orofacial therapy or surgery These are among the other approaches listed by NHLBI. More invasive options are selected clinically; suitability depends on the individual.

People prescribed PAP use a mask or interface and tubing. If replacement supplies are needed, ask the clinician or equipment supplier to confirm that the parts fit the prescribed setup. Do not change machine pressure or substitute equipment based on general advice.

When should you seek an assessment?

Ask a healthcare provider about assessment if you or someone who sleeps nearby notices repeated breathing pauses, gasping or frequent loud snoring, especially when accompanied by daytime sleepiness or tiredness. The provider can decide whether referral and a sleep study are appropriate. Treating diagnosed OSA is worthwhile for its established sleep-related and health indications, even though it is not proven to prevent Alzheimer’s disease.
