FormAssembly is a web-based form builder and data collection platform for organizations that need structured submissions and workflow automation. It suits healthcare, higher education, financial services, government, nonprofits, and marketing teams that need more than basic field collection. The platform supports no-code form building and publishing, conditional rules, file uploads, electronic signatures, automatic PDF generation, templates, and an AI Assistant named Fai. Web, iOS, and Android support extend access across platforms.

Its strongest fit is approval-driven work. Conditional rules can show or hide fields, sections, groups, and pages, while multi-step workflows support approval steps with approved and denied paths. Payment collection is available through Stripe, PayPal, and Authorize.Net connectors. Integrations with Salesforce, HubSpot, Google Drive, Microsoft Excel, Dropbox, and Stripe make FormAssembly relevant for teams that want form submissions connected to existing business systems. The Basic plan includes one user, unlimited forms and responses, and a no-code builder; Essentials adds e-signatures, branding, themes, permissions, access controls, and data residency and localization options.

Larger or more regulated organizations can move to Team for workflows, SSO, respondent authentication, and multi-user support. Enterprise adds field-level masking and encryption, HIPAA and BAA compliance, plus a customizable SLA and contract. Government adds custom pricing, FedRAMP Moderate-Impact support, and government authentication requirements. That tier structure makes the product a strong choice for organizations with approval, identity, compliance, or integration requirements. It is less suitable for a single user who only needs a simple form and does not need workflow or system connections.
