The performance chart looked plausible in code, but the browser rendered the same data series twice. The coding agent had already marked the task complete; the defect became obvious only when Abhinav Pangaria opened the page. His account makes a useful distinction for anyone building interfaces with AI: code that appears sound is not proof that the rendered result works.
What went wrong with the chart
Pangaria reports that the chart pipeline and rendering call looked reasonable during code review. In the browser, however, overlapping traces with different visual weights revealed that one series had been drawn twice. Related layout problems were also visible in the rendered page, rather than in a quick review of the diff.
The failure was not simply that an agent wrote imperfect code. It was that the task was treated as complete before anyone had verified the interface against its requirements. A successful build or a plausible-looking implementation can coexist with a broken chart.

How the verification loop was designed
Pangaria describes GuardianKane as a task-tracking and verification workflow for Claude Code. It distinguishes an agent’s claim of completion from a verified status: tasks can move through IN_PROGRESS and CLAIMED_DONE, while KANE_VERIFIED is intended to be reserved for work that passes checks.

When the coding agent tries to stop while a task is claimed done, a Stop hook runs Kane CLI against a running development server in headless Chrome. The reported workflow runs two checks for each task:
- Scripted replay: The browser replays a test written for that task, checking the interactions and assertions the test author specified.
- Visual defect sweep: A browser agent is asked in free text to inspect for visual defects, layout problems, missing elements, console errors, and mismatches with the requirement.
If a check fails, the hook blocks the stop and tells the coding agent to return the task to IN_PROGRESS, fix the issue, and claim completion again. This is Pangaria’s description of the workflow; it is not an independent audit of GuardianKane or its implementation.

Why use both scripted and visual checks?

These checks address different gaps, so the account presents them as complementary rather than as a measured contest. A script can reliably confirm a known behavior; a visual sweep can look for problems the script was not designed to notice. Neither is a guarantee that every possible defect will be found.

Anthropic’s official prompting guidance also recommends giving agents tools to verify UI work, including computer-use, browser-use, or browser automation tools. That supports the general value of browser-based verification, but it does not validate GuardianKane or establish what caused the results Pangaria reports.

What the project comparisons show—and do not show
Pangaria describes four paired builds. Each pair used the same PRD for an unassisted Claude Code run and a run using the GuardianKane loop. The projects included a todo app, two booking projects, and ORBITAL, a dense portfolio dashboard. In the unassisted ORBITAL build, he reports a duplicated chart series and differences in the alerts-panel layout.
For the gated ORBITAL build, Pangaria reports twelve tasks reaching KANE_VERIFIED and one visual-sweep failure on the alerts panel despite passing scripted tests. He says that was the only failure in that build that represented a real app problem. The log did not record enough detail to establish precisely what the sweep detected. In particular, it does not show that the gate caught the duplicated chart and forced a fix.

The comparison also changed more than the verification loop: Pangaria says the gated build’s PRD was clarified before coding and tasks were scoped to PRD sections. The four pairs cannot isolate whether those changes, the browser gate, or another factor explains the differences. They are project-specific observations, not a controlled or large-scale benchmark, and they do not establish a general improvement rate.

What developers can take from the incident
- Treat completion as a claim until the output is checked. A status from the coding agent does not establish that the page matches the requirement.
- Verify the rendered interface, not just the source. The duplicated series was visible in the browser even though the chart code looked plausible.
- Pair repeatable tests with broader inspection. Scripted assertions cover known scenarios; a visual review can examine conditions outside those assertions, though it is less deterministic.
- Return failures to the agent while it can still act. The described hook runs checks before the coding agent yields control, so a failure can prompt another fix-and-check cycle.
- Keep claims proportional to the evidence. Pangaria’s account illustrates a verification approach and its reported outcomes; it does not prove that the gate caused a clean chart or that the method improves every project.
