Use single-modality segmentation when one image type shows the target clearly and is reliably available. Use multimodal segmentation when aligned inputs contribute distinct, relevant information—such as anatomical detail plus PET’s metabolic signal—and your workflow can handle alignment, input quality, compute, and missing or degraded data. More images do not automatically mean better segmentation, and the available evidence does not establish one approach as universally superior.

When should you use multimodal medical image segmentation?

Choose based on the target and the information needed to delineate it, not on the number of images available. Start by specifying the structure, lesion, or region to label and what counts as its boundary. Then ask whether one modality shows that target with adequate contrast for the task.

  1. Define the target. State which anatomy or pathology must be segmented and what boundary or label is clinically or scientifically meaningful.
  2. Assess the available image. If a single modality makes the target visible with sufficient detail and is consistently available at inference time, a single-modality method may be the more dependable choice.
  3. Add a modality only for complementary evidence. For example, PET may contribute metabolic information while CT or MRI supplies anatomical context; different MRI sequences can also show complementary tissue characteristics.
  4. Verify joint availability and alignment. Confirm that the inputs will be present together and registered well enough for the intended use.
  5. Validate under the intended workflow. Compare candidates on the same target, data split, annotation protocol, and metrics, and account for compute, inference latency, and deployment constraints.

A 2020 review notes that segmentation results are difficult to compare across studies when datasets and reported measures differ. A result from one task or dataset therefore should not be treated as proof that a modality combination will help another.

What does each imaging modality contribute?

The following are broad tendencies, not a ranking. Suitability depends on anatomy, pathology, acquisition protocol, and the target being segmented.

Modality Potentially useful signal Tradeoffs to consider
CT Anatomical and bone detail; it can provide anatomical context alongside PET. Weaker soft-tissue contrast than MRI and exposure to ionizing radiation.
MRI Strong soft-tissue contrast. Complementary sequences can help characterize different features; a segmentation review describes T2 and FLAIR as useful for tumor- and edema-related appearance, and T1/T1c for anatomy and tumor core. The benefit of multiple sequences depends on whether each adds useful information for the target and is available and reliable in the workflow.
PET Metabolic or functional information that can complement anatomical imaging. Limited anatomical detail and lower spatial resolution; interpretation and segmentation commonly benefit from CT or MRI context.
Ultrasound Accessible, real-time imaging without ionizing radiation. Operator dependence and acoustic-window limitations can affect image quality and segmentation stability.

These characteristics are summarized in reviews of imaging modalities and segmentation; they should not be read as universal performance specifications for every scanner, protocol, or clinical task.

Is multimodal segmentation always better?

No. A second image can help only if it supplies relevant information and the method can use it reliably. Registration errors, poor-quality inputs, or a modality that is missing at inference time can undermine a multimodal system. A simpler single-modality model may be more appropriate when one image already captures the required boundary or when the additional input is not dependable.

A 2017 study by Guo et al. on soft-tissue sarcoma imaging reported better results for fusion schemes than single-modality schemes in its experiments combining MRI, CT, and PET. The authors also reported that feature-level fusion could be less robust when one modality contained large errors. This is evidence from a specific research setting, not a clinical guarantee or a universal comparison across organs and tasks.

Fusion describes where information from multiple modalities is combined in a segmentation pipeline.

  • Input or early fusion: modality images are provided together as channels before a shared segmentation network processes them.
  • Feature or layer fusion: modality-specific features are learned first and combined later in the network.
  • Classifier or decision-level fusion: downstream predictions from separate processing paths are combined.

A 2020 review describes these strategies and notes that later fusion can improve results when the fusion method is effective, but the suitable design depends on the problem. The sarcoma experiment by Guo et al. illustrates a further tradeoff: feature-level fusion performed well in that study, but errors in one input could reduce robustness.

When is one MRI sequence enough?

One sequence is enough when it shows the target and its relevant boundary adequately for the task, and the model can rely on that sequence in the intended setting. Multiple sequences are worth considering when they provide complementary evidence—for example, the 2020 review discusses T2 and FLAIR for tumor- and edema-related appearance and T1/T1c for anatomy and tumor core.

Do not add sequences solely because they exist. Check whether the added sequence improves target-specific segmentation under a consistent evaluation protocol, and whether it will be reliably available in deployment. If a model requires several sequences but one is often absent or degraded in the actual workflow, that requirement is part of its practical cost.

What are the tradeoffs of PET/CT or PET/MRI segmentation?

PET can add functional or metabolic signal, while CT or MRI can provide anatomical context. That complementarity can be useful when both kinds of information matter to the target. The combination also makes alignment and input reliability important: the system must be able to relate the functional signal to the anatomy, and substantial errors in one modality can affect fused results.

The reviewed evidence supports evaluating such combinations for the particular task; it does not establish that PET/CT or PET/MRI is always better than segmentation from one modality. The 2017 soft-tissue sarcoma study is a task-specific example involving MRI, CT, and PET, not a cross-organ clinical comparison.

How should you compare candidate methods?

Make the comparison specific to the target and intended deployment rather than relying on a headline metric from unrelated studies.

  • Segmentation quality: evaluate the same target with the same data split, annotations, and metrics.
  • Information gain: establish what distinct signal each input contributes and whether that signal is relevant to the target.
  • Registration: inspect alignment quality and determine how misalignment affects the output.
  • Input robustness: test performance when a modality is noisy, degraded, or missing if those conditions are plausible in use.
  • Operational fit: account for compute, inference latency, data availability, and integration into the intended clinical or research workflow.

Local validation matters because the reviewed literature includes modality reviews, a segmentation-fusion review, and a focused sarcoma experiment rather than a controlled comparison across organs proving which combination improves outcomes in every setting.
