To make a scored quiz in Microsoft Forms, choose New Quiz—not New Form—then add questions, mark the correct answers, assign points, and preview the quiz before sharing it. You can control who responds and when, review submissions in Forms, give feedback, and open results in Excel.
Create a quiz and add questions
- Open Microsoft Forms and select New Quiz.
- Enter a quiz title and, if useful, a description that explains the instructions or expectations.
- Select Add new or a question type under Quick start with. Available types documented by Microsoft include Choice, Text, Rating, Date, Ranking, Likert, File upload, and Net Promoter Score. Pick a format that fits what you want respondents to do; not every type suits every assessment.
- Write the question and add its answer options or other required content. Set a question as required when respondents must answer it.
Forms saves as you work. You can adjust question settings, including branching, while authoring; it is usually easier to build the questions first and configure branching afterward.
Optional: draft with Copilot
Microsoft documents Copilot-assisted quiz drafting for users with a Microsoft 365 account and a Copilot license. Treat a generated quiz as a starting draft: check the title, wording, answer key, and points yourself before sharing. You can keep or regenerate the draft.

Optional: create math questions
Microsoft describes a math quiz feature for Microsoft 365 subscribers, including equation entry and suggested answers or questions. Availability depends on the account and current Forms experience, so check what appears in your account rather than assuming it is available to everyone.

Set correct answers, points, and feedback
- Open a question and select the correct answer or answers where the question type supports scoring.
- Enter the question’s point value in Points.
- Optionally add a message associated with an answer, such as a brief explanation respondents can see.
Review the key against the question before distributing the quiz, especially if you copied content or used a generated draft. Answer-specific messages are configured during authoring; you can also provide feedback when reviewing submitted work.
Preview the respondent experience
- Select Preview and inspect the computer layout.
- Switch to the mobile layout and check that questions and answer choices remain easy to read and use.
- Answer the preview quiz and submit it. Check whether the answer key, points, and feedback behave as intended, then return to editing if anything needs correction.
Use the test run to catch unclear wording, an incorrect key, or a layout problem before respondents receive the quiz.

Set response access, timing, and question order
Open Settings to configure when the quiz is available and how respondents encounter its questions. The exact access choices depend on the account and organization.
- Availability: Set a start or end date, or turn off Accept responses to close the quiz. You can enter a custom message for people who open it after responses have stopped.
- Respondent access: Decide who is allowed to respond before sharing. In Microsoft’s student-sharing guidance for education customers, organization-only responding is selected by default in that context; do not assume that default applies to personal or other organizational accounts.
- Question order: Turn on question shuffling if respondents should receive questions in a different sequence. You can lock selected questions in place. Shuffling is unavailable when the quiz has multiple sections or pages, so decide whether you need sections before relying on this setting.
When to use Take a Test
Take a Test is not a general-purpose setting for every respondent or device. Microsoft describes it as a secure assessment option for compatible Windows devices, with access restrictions. Students who cannot access the app cannot open a quiz configured this way. Check device compatibility and school configuration before choosing it.

Add branching without sending respondents backward
- Build the quiz questions first.
- Open a question’s options and choose its branching controls.
- Set a destination for each relevant response. A destination can be a later question or End of the form. Sections can also be used as branching destinations.
- Check the routes in Preview before sharing.
Branch only forward. Microsoft warns that routing to a preceding question can cause later questions to be skipped and take the respondent to submission. Map each route carefully so respondents reach the questions intended for them.
Share the quiz with respondents
- Select Collect responses.
- Confirm the audience or access restriction matches the people who should take the quiz.
- Choose a sharing method offered in your account, such as a link, email, or QR code, and distribute it to respondents.
Access options can vary by account type and organization policy. For an education account, check the organization’s settings rather than relying on a default from guidance intended for a particular EDU context.

Review submissions, give feedback, and export results
Open the quiz’s Responses view to inspect summary information, question-level results, or individual submissions. You can review and grade responses by person or question, add general or answer-specific feedback, and post scores to students.
To analyze results outside Forms, use Open in Excel. Changes made in Excel do not change the quiz itself. Deleting a response in Forms cannot be undone, so check the selected response before removing it.
