On-premises Exchange servers with missing security updates, incomplete required actions, or an unsupported lifecycle status need prompt attention. Inventory every Exchange server and management-tools-only computer, confirm each build and its update eligibility, then apply the current supported update path. If a server may have been compromised, investigate and remediate as well: patching closes a vulnerability but does not prove an attacker is gone.
Who is at risk?
The primary concern is Exchange Server installed on premises, including servers retained in Hybrid environments. Risk depends on the specific vulnerability, installed build, configuration, accessibility, and whether exploitation occurred; an old vulnerability report cannot establish whether a server is exposed to a newer issue.
- Servers missing applicable updates or required follow-up actions: treat them as exposed until you verify their build, configuration, and remediation status.
- Exchange Server 2016 or 2019 installations: these products reached end of support on October 14, 2025. As of October 9, 2026, Microsoft says organizations enrolled in the Extended Security Update (ESU) program are eligible for security updates released from December 2025 onward. Organizations not enrolled are directed to migrate to Exchange Server Subscription Edition to continue receiving the latest security updates.
- Hybrid servers kept only for managing Exchange-related objects: Microsoft says these servers still need to be kept current. A limited day-to-day role is not a reason to leave the server stale.
- Servers that were vulnerable before they were patched: they may already have an attacker foothold or persistence. Update status alone does not establish that the system is clean.
Microsoft’s 2021 Exchange resource center said Exchange Online was unaffected by the specific vulnerabilities addressed in that 2021 guidance. That historical statement does not establish the status of Exchange Online for later vulnerabilities; check the advisory for the issue you are assessing.
What is the current support status?
Microsoft’s end-of-support roadmap says support for Exchange Server 2016 and 2019 ended October 14, 2025. Its supportability matrix lists Exchange Server Subscription Edition RTM as a supported build. The build-history page says ESU-enrolled customers are eligible for December 2025 and later security updates for Exchange Server 2016 and 2019; those outside ESU are directed to migrate to Subscription Edition for continued access to the latest security updates.

Use Microsoft’s live supportability matrix, build history, and update guidance to confirm exact build numbers, supported configurations, and ESU eligibility before scheduling a change. The Exchange Server update FAQ was last updated May 9, 2025. Its general process advice remains useful, but its examples of particular cumulative updates predate the end of support for Exchange 2016 and 2019 and should not be treated as current version-specific recommendations.
Choosing a supported path

No path is universally preferable. The decision turns on whether your organization needs on-premises Exchange, the work and risk involved in migration, and the update eligibility available for the environment you operate.

What should administrators do?
- Inventory installations. List every Exchange server and every computer with Exchange Management Tools only. Record product version, build, role, configuration, and whether the system is supported or covered by ESU. Microsoft’s Exchange Server Health Checker can identify missing cumulative updates (CUs), security updates (SUs), and some required manual steps.
- Resolve lifecycle coverage. For Exchange 2016 or 2019, confirm ESU enrollment and applicable updates, or plan migration to a supported path. Microsoft’s end-of-support roadmap identifies Exchange Server Subscription Edition, Microsoft 365, and Office 365 as options; validate the fit for your architecture rather than assuming one is right for every organization.
- Install the applicable supported updates. Follow the current release guidance for the precise version and build, rather than relying on older FAQ examples. Microsoft’s general guidance is to install the latest applicable CU and SU, and to install SUs on Exchange servers and Exchange Management Tools-only computers. Keep Windows updated too: operating-system vulnerabilities can contribute to an attack chain.
- Complete and verify follow-up actions. Run Exchange Server Health Checker before and after security updates. Some vulnerabilities require additional administrator actions after an SU; installing the update alone may not complete remediation.
- Use mitigations only as a bridge. Exchange Emergency Mitigation (EM) service can apply temporary protections to Exchange Mailbox servers. Microsoft says it checks for mitigations hourly. A mitigation may disable vulnerable functionality, needs administrator visibility and follow-through, and is not a replacement for an SU. Microsoft also warns that mitigations can be bypassed; install the code fix when available.
- Investigate possible compromise in parallel. If there was exposure or there are indicators of compromise, investigate for exploitation and persistence while updating. Assess remediation needs, lateral movement, and further compromise. In its March 2021 guidance for the vulnerabilities exploited at that time, Microsoft’s Detection and Response Team, Microsoft 365 Defender Team, and CSS Security Incident Response said that applying those March 2021 updates was critical to prevent reinfection but would not evict an adversary already on a server. That is an incident-specific historical warning, not an announcement about current vulnerabilities.
- Protect incident communications. If compromise is suspected, follow your incident-response plan. Microsoft’s 2021 resource center advises using isolated identities and communications until the privacy of current infrastructure communications is assured.
Why update Exchange if it appears to work?
Security updates address vulnerabilities in code; an apparently functional server can still be exposed. Microsoft’s update FAQ states: “Your on-premises environments should always be ready to take an emergency security update.” Keeping systems current also reduces the chance that an attacker can exploit a known weakness as part of a broader attack chain. Update readiness includes required manual actions and a supported lifecycle path, not merely having installed an older CU.
Does a Hybrid server need updates if it is not actively used?
Yes. Microsoft explicitly says a Hybrid server that remains in place to manage Exchange-related objects still needs to be kept current. Include it in the inventory, confirm its support and update status, and apply the updates and required actions that match its version and build.

What else should fully up-to-date administrators do?
“Up to date” should mean more than installing the newest update you know about. Confirm against Microsoft’s current build and advisory information that each system is on a supported or eligible update path, then run Health Checker to find missed manual actions and verify the post-update state. Keep Windows updates in scope. If the server was exposed while vulnerable or shows signs of compromise, treat investigation and remediation as separate work from patching; a clean update check is not evidence that an attacker was never present.
For a live incident, consult the Microsoft advisory for the particular vulnerability and use the organization’s incident-response process. The 2021 guidance is useful for its distinction between patching and removing an existing attacker, but it does not identify current active vulnerabilities.
