Measles often begins with a high fever, cough, runny nose and red, watery eyes; a few days later, a rash usually starts at the hairline or face and spreads downward. Small white Koplik spots may appear inside the mouth before the rash. Colds are more often milder and nose-centered, while flu often starts abruptly with fever or chills, aches and fatigue. These patterns cannot diagnose an illness: if measles is possible, call a healthcare provider promptly and call before visiting.
How the symptoms compare
The table describes common patterns, not a home diagnostic test. Symptoms overlap, and measles may not follow the typical pattern; some people, including some with weakened immune systems, may not develop a rash. A clinician may need testing to identify the cause.

How measles symptoms typically unfold
Early illness
CDC says measles symptoms usually begin 7–14 days after contact with the virus. The first symptoms commonly include high fever—which can exceed 104°F—cough, runny nose and red, watery eyes. These timing ranges describe what usually happens; they do not prove or rule out measles in an individual.

Koplik spots and rash
Koplik spots may appear inside the mouth about 2–3 days after symptoms begin. The rash typically starts 3–5 days after the initial symptoms, as flat red spots on the face or at the hairline, then spreads down the body. Spots can merge, and the fever may spike above 104°F as the rash appears. CDC’s clinical overview describes the rash as usually appearing about 14 days after exposure.
Measles can spread before the rash is visible. CDC considers patients contagious from four days before through four days after the rash appears. Its public guidance says the virus can remain in the air for up to two hours after an infected person leaves a space. These facts make it important to call ahead rather than arrive unannounced.

What makes a cold or flu more likely—and what does not
Common cold
A cold often develops gradually and is usually milder than flu. Symptoms can include a runny or stuffy nose, sneezing, cough, sore throat, headache and mild body aches. Older children and adults usually have low-grade or no fever. CDC says cold symptoms commonly peak within 2–3 days and usually last less than a week, though duration varies.
Flu
Flu often starts suddenly. Fever or chills, muscle or body aches, headache, fatigue and cough tend to be more common or pronounced than with a cold, but fever is not universal. A runny or stuffy nose and sore throat can occur with flu too.

Because colds, flu and other respiratory illnesses share symptoms, the difference may be impossible to determine from symptoms alone. A rash alongside measles-like symptoms, a known measles exposure or relevant travel is a reason to contact a healthcare provider promptly—not to wait for a checklist to provide certainty.
What to do if measles is possible
- Call a healthcare provider immediately. Explain the symptoms and any known exposure or relevant travel. CDC guidance is to call before going to a clinic or hospital so staff can arrange an assessment without exposing others.
- Stay home and limit contact while you get advice. CDC advises a person with measles to stay home until the fifth day after rash onset and avoid contact with people at home who are not sick. Follow the instructions of your healthcare provider and local public-health authority.
- Seek emergency care for warning signs. Trouble breathing or breathing faster than normal is among the emergency warning signs listed by CDC. If emergency care is needed, have someone call ahead and tell responders measles is possible.
Suspected measles requires clinical and public-health follow-up, not self-diagnosis. CDC says laboratory confirmation is essential for sporadic measles cases and outbreaks, and suspected cases should be reported to the local health department. The relevant contact and reporting pathways can differ outside the United States.

Why prompt attention matters
Measles is highly contagious: CDC says up to 90% of close contacts who are not immune may become infected. The agency also lists ear infections and diarrhea among common complications; serious complications include pneumonia and encephalitis, and measles can be fatal. People at higher risk of serious complications include children younger than five, adults older than 20, pregnant people and people with weakened immune systems.

CDC reports that ear infections occur in about 1 out of every 10 children with measles, and diarrhea is reported in fewer than 1 out of 10 people with measles. These are population-level figures, not predictions of an individual’s outcome.

Protection
MMR vaccination is the best protection against measles. CDC says two doses are about 97% effective at preventing measles and one dose about 93% effective. Check current vaccine or exposure guidance with a healthcare provider or public-health authority for your location and circumstances.
