To prevent focus loss and keyboard traps in a design system, define where focus goes at every component transition: when a widget opens, while users move through it, and when it closes or removes content. A modal dialog should contain Tab navigation while it is open, but users must have a way to close it; when it closes, focus should return to its opener or another logical element that still exists.
Why focus needs an explicit design-system rule
Keyboard focus is interface state: it tells users where keyboard input will go next. When a component opens, closes, or changes the page, focus needs to remain visible and attached to a useful element. If the focused node disappears and the application does not move focus elsewhere, the browser may move focus to the document body, leaving users unsure where they are.
ARIA attributes describe semantics; they do not implement a widget’s keyboard behavior. The W3C ARIA Authoring Practices Guide (APG) says authors must provide keyboard support for ARIA components. Specify behavior for each component using its established interaction pattern rather than assuming one keyboard rule fits all.

Document the component’s keyboard contract: which keys enter, move within, activate, and exit it; where focus starts; whether users navigate with Tab or arrow keys; and where focus goes when an item disappears or the interaction ends.

Composite widgets often have one Tab stop from the page into the component, with arrow keys moving among its internal items. That is different from placing every internal item in the page-wide Tab sequence. Follow the relevant APG pattern for the widget instead of inventing a universal rule.
Should Tab stay inside a modal dialog?
Yes—while a dialog is modal, its Tab sequence is intentionally contained. The APG modal dialog pattern specifies that focus moves inside when the dialog opens, and Tab and Shift+Tab cycle among its controls. Escape closes the dialog in that pattern. This containment is not a keyboard trap when users can leave through the dialog’s ordinary close or cancel controls, or through Escape as specified by the pattern.

- On activation: Move focus into the dialog. The first focusable control is common, but a static title or introductory element with
tabindex="-1"may be more suitable when the content is large or the task calls for reading from the beginning. - While open: Keep Tab and Shift+Tab within the dialog, and include a visible close or cancel control in the tab sequence.
- On dismissal: Support Escape as described by the pattern, then return focus to the invoking control unless it no longer exists or the next workflow step makes a different destination more useful.
How do you prevent keyboard traps in a design system?
Containment is appropriate only when the interaction is genuinely modal. A dialog marked with aria-modal="true" must actually prevent interaction with background content, and its visual presentation should make that content appear unavailable. Otherwise, assistive-technology users may be told that outside content is unavailable while other users can still interact with it. Do not use aria-modal as a cosmetic label for a panel that leaves the background active. See the APG’s modal dialog guidance.
For other components, users need an ordinary keyboard route out. Document the exit behavior alongside the keys that move focus within the component; do not let a custom interaction consume navigation keys without a predictable alternative.

How do I stop focus from getting lost when a dialog closes?
Make focus restoration part of the close action. Return focus to the opener if it remains available. If the opener has been removed, hidden, or is no longer the useful next step in the workflow, move focus to a suitable surviving destination.
Apply the same principle when removing focused content. For example, after deleting a focused list item, move focus to the next item, the previous item, or a containing control according to what remains and what the user is expected to do next. The APG explains that without focus recovery after a dialog or deletion, browsers can move focus to the body and cause a loss of position in the interface. Why does keyboard focus jump to the top of the page?
A common cause is that the focused element was removed or made unavailable without assigning focus to a replacement. The browser may then place focus on the document body; subsequent keyboard input can feel as if it has lost its place or returned to the start of the page. The APG describes this risk when an application does not move focus to the dialog’s trigger or to a list item following deletion.
When a component changes the DOM, identify the element that should receive focus after the change and move focus there as part of the same action. A surviving, task-relevant target is more useful than leaving focus on a node that no longer exists.

How should a team choose native or custom dialogs?
Neither native HTML <dialog> nor a custom ARIA dialog is universally superior on the evidence available here. MDN documents the HTML dialog element; the APG describes the expected interaction pattern. Choose and verify an implementation against the behavior your product needs:
- Does it provide the required modal behavior, including preventing background interaction and managing focus?
- Can it place initial focus appropriately and restore focus after closing?
- Does it provide a visible close or cancel control and keyboard interactions that fit the workflow?
- Has the interaction been verified in the browsers and assistive technologies the product supports?
The cited references do not establish a compatibility matrix or a universal ranking. Test the chosen implementation in the product’s actual supported environment.
How do you keep focus visible and predictable?
Retain a clear focus indicator and make it distinguishable from selection styling and other decoration. Consistent focus order and movement help users predict where navigation keys will take them. The APG advises relying on browser focus indicators unless there is a deliberate reason to override them; any replacement should remain easy to perceive.

Use keyboard-only interaction checks as part of component validation. This is a practical way to apply the W3C guidance, not a report of testing results:
- Open the component using its keyboard-accessible trigger and confirm focus moves to the intended starting point.
- Navigate forward and backward using the documented keys; verify that movement matches the component pattern.
- Activate available actions and confirm the focus indicator remains visible.
- Close or exit the component using its documented routes, then confirm focus lands on a logical surviving element.
- Remove focused content and verify that focus moves to the intended next item or control rather than being lost.
- Repeat in the browsers and assistive technologies included in the product’s support matrix.
