Para entendê-lo, comece pela pilha de chamadas e pelo princípio de que cada trecho de trabalho termina antes de outro começar; depois estude as regras específicas do navegador ou do Node.js, que não usam exatamente o mesmo modelo.
O que o event loop faz
JavaScript executa código em uma pilha de chamadas. Um job começa, roda até concluir e só então outro job pode ser processado. Esse princípio, chamado run-to-completion, ajuda a prever alterações de estado: outro job não interrompe o trecho atual no meio.
Isso não significa que uma operação assíncrona terminou quando foi iniciada. O ambiente hospedeiro — como o navegador ou o Node.js — pode cuidar de eventos, timers ou I/O e, quando houver trabalho pronto, disponibilizar um callback para execução posterior. O event loop coordena essa execução com o trabalho JavaScript, segundo as regras do ambiente. A MDN explica o modelo de execução do JavaScript, incluindo agentes, pilha e jobs.
O que acontece quando um timer é agendado
Um timer não interrompe o código síncrono que está rodando. Ao chamar setTimeout, o ambiente agenda o callback; o trecho atual continua até terminar. Depois, o callback só pode executar quando as condições do loop permitirem. Portanto, setTimeout(fn, 0) não significa “executar agora”: o valor não transforma o callback em uma interrupção nem garante sua execução imediata.

Eventos e timers podem gerar tarefas. Em explicações informais, às vezes se usa “macrotarefa”, mas o modelo HTML documenta tasks (tarefas) e microtasks (microtarefas). Esses termos e a ordem precisa de execução dependem do host; não existe uma única fila universal que descreva igualmente todos os ambientes.
Promises e microtarefas no navegador
No modelo de navegador documentado pela MDN, callbacks de Promise são executados como microtarefas. Quando a pilha fica vazia, o navegador drena a fila de microtarefas antes de passar à próxima tarefa. Assim, uma microtarefa pode executar antes de um callback de timer ou de evento que esteja aguardando como tarefa.

No navegador, o modelo HTML organiza tarefas e microtarefas e convive com APIs da plataforma web, eventos e renderização. No Node.js, o event loop tem fases próprias associadas a tipos de trabalho, incluindo poll e check.

Segundo a documentação oficial do Node.js sobre o event loop, a partir do Node.js 20, com libuv 1.45.0, os timers passaram a ser executados depois da fase poll em cada iteração. Por isso, ao estudar ou comparar a ordem de callbacks no Node.js, identifique a versão usada; um resultado observado em determinada versão não é uma garantia universal da linguagem JavaScript.
Por que trabalho síncrono longo atrasa callbacks
O event loop não torna código limitado por CPU automaticamente paralelo. Se uma função síncrona demora muito, a thread JavaScript continua ocupada com ela, e outros callbacks precisam esperar até que o trabalho atual termine. No Node.js, esse atraso pode fazer clientes e callbacks de I/O aguardarem sua vez, afetando a latência. A documentação do projeto resume o papel do mecanismo desta forma: “The event loop is what allows Node.js to perform non-blocking I/O operations — despite the fact that a single JavaScript thread is used by default — by offloading operations to the system kernel whenever possible.” Isso descreve I/O não bloqueante delegado ao kernel quando possível; não quer dizer que uma tarefa JavaScript síncrona pesada deixe de bloquear a execução.

Para analisar um atraso, procure primeiro por trabalho síncrono que ocupa a thread por tempo excessivo. Separar ou redesenhar esse trabalho exige considerar o ambiente e a tarefa concreta; o event loop, por si só, não executa código CPU-bound em paralelo.
