Neither indoor nor outdoor walking is universally better for fitness. The better choice is the one you can do safely and regularly at an effort that contributes to your activity goals. Treadmill and overground walking differ in some measured responses, and outdoor exercise may offer extra benefits for some outcomes, but current evidence does not establish that either setting produces better long-term fitness for everyone.
What counts most for fitness
The Centers for Disease Control and Prevention recommends that adults get at least 150 minutes per week of moderate-intensity aerobic activity, or 75 minutes of vigorous activity or an equivalent combination, along with muscle-strengthening activity on two or more days each week. The target is based on how much activity you do and its intensity—not whether you do it indoors or outdoors. Brisk walking is one example of moderate-intensity activity. CDC adult activity guidelines describe the recommendation; NIDDK activity guidance describes brisk walking as 3 miles per hour or faster and notes that activity can be built up gradually.
There is no requirement to reach 10,000 steps a day. An NIH summary of step-count research says health benefits accrue below that commonly cited goal. Step totals do not show that one walking setting is superior to the other.

What studies show about indoor and outdoor walking
Treadmill and overground walking are not identical
A 2024 systematic review and meta-analysis by Vickery-Howe and colleagues compared motorized treadmill walking with overground walking at similar speeds in healthy adults ages 18–65. It included 55 studies and 1,005 participants; its literature search ran through September 2022. The review found greater relative oxygen consumption and cadence on treadmills, along with shorter stride and step lengths. Treadmill construction, stability, and familiarity may affect these differences. These are measured responses under study conditions, not evidence that treadmill walking produces better real-world fitness over months or years. Outdoor exercise may have added benefits, but the evidence is limited
A 2023 systematic review of longitudinal trials comparing outdoor and indoor exercise included 10 trials and 343 participants. Of 99 comparisons, 25 were statistically significant, and all 25 favored outdoor exercise. The review also emphasized that the evidence was limited: there were few studies, outcomes and protocols varied, and risk of bias was generally high. Larger, more rigorous trials are needed. The results suggest outdoor exercise may benefit some outcomes; they do not establish that outdoor walking is a universal fitness winner. Read the outdoor-versus-indoor exercise review.
These findings answer different questions: one review measured immediate physiological and movement differences, while the other considered outcomes in a small set of longer-term trials. Neither provides a reliable answer to whether the same person will improve fitness more by walking indoors or outdoors over time.

The CDC’s review of mall walking identified lighting, restrooms, and places to pause as potential facilitators. Its scoping review found mall walking could offer a safe, accessible, and affordable environment for middle-aged and older adults, with possible physical, social, and emotional benefits. The underlying evidence was limited and weakly designed, so an indoor venue is best understood as a practical way to address barriers—not a proven superior intervention. Choose a setting you can use safely and consistently
Compare the options on four practical measures: how much weekly time and moderate effort you can achieve, safety and route accessibility, reliability in your schedule and weather, and how likely you are to enjoy and repeat the walk. Weekly time and intensity align with public activity guidance; the other measures help determine what is workable for you, rather than ranking proven health effects.

- Choose outdoors when you have a pleasant, accessible route and conditions you can manage.
- Choose a treadmill or indoor venue when weather, mobility, security, lighting, or convenience makes an outdoor route difficult.
- Alternate settings if that helps you stay active. You do not need to buy a treadmill, and neither setting guarantees greater weight loss or fitness gains.
Build up gradually and watch for warning signs
If you have been inactive, NIDDK advises starting slowly and increasing duration and frequency gradually. For outdoor walks, dress for the weather, use sun protection, stay hydrated, and look for safe, well-lit places where other people are around. Comfortable, well-fitting walking shoes with support are recommended, but buying new shoes is not a condition for choosing a setting.
Stop and seek help if you experience chest or neck, shoulder, or arm pressure; extreme shortness of breath; dizziness; or sickness during activity. If activity causes persistent pain in your joints, feet, ankles, or legs, discuss it with a health professional.
