Before sending a workers’ compensation claim form, check that you have the current form for the right jurisdiction and claim type, that each answer matches a reliable record, and that you have completed only the section assigned to you. The form’s instructions control what to enter, what may be left blank, who signs it, and where it goes; a careful review can catch mistakes, but it cannot guarantee that a claim will be accepted.

Start with the correct form and instructions

Workers’ compensation forms and filing routes vary by state, federal program, and claim type. First identify the system handling the claim, then use the responsible agency’s current form and instructions—not a generic template or an old saved copy. Check whether the form is for the worker, employer, medical provider, or more than one party. A worker should not fill out another party’s section unless the official instructions direct them to.

The examples below illustrate why that distinction matters; they are not a national form or a complete survey of every jurisdiction:

  • California: The California Division of Workers’ Compensation says the worker completes the “employee” section, signs and dates it, and sends it to the employer. The employer completes its section and forwards the form to the insurer. See the agency’s How to file a claim page, marked June 2025.
  • Arizona: The Industrial Commission of Arizona’s Worker’s and Physician’s Report assigns the Worker’s Report to the worker and the Physician’s Initial Report to the treating physician or medical provider. Each completes and signs and dates their assigned section. See the Commission’s Worker’s and Physician’s Report of Injury Form guidance.
  • New York: The employee’s C-3 claim and the employer’s C-2F report are different forms with different responsibilities. The state’s C-3 filing page describes information for the worker, while the Board’s C-2F instructions address the employer’s report.
  • Federal Longshore claims: Department of Labor form LS-203 applies to claims under the Longshore and Harbor Workers’ Compensation Act and specified extensions. It is not a general federal or state workers’ compensation form. Consult the current LS-203 form and instructions.

Check each field against a reliable source

Use the form’s instructions to decide which fields apply and what level of detail is requested. Compare entries with documents or records you trust, rather than relying on memory where a record is available. Depending on the form, review these items:

  • Your identity and contact details: Confirm your legal name, mailing address, and other requested contact information are complete and spelled correctly.
  • Employer and job details: Check the employer’s name and address, your job title and duties, employment status, and wage information if the form requests them. Distinguish gross pay from take-home pay when the form specifies gross pay.
  • Dates and times: Compare the injury or illness date, time of injury, date it was reported, and date the employer learned about it, where those fields appear. Use the form’s instructions if a date is uncertain; do not silently substitute a guess.
  • Incident facts: Check the work location or accident address, what you were doing, how the event happened, and any requested witness information. Keep the account factual and consistent with the other answers.
  • Injury or illness: Name the condition and affected body part or parts as the form requests. If it asks for side or location—such as left, right, or bilateral—make sure that answer matches the description.
  • Treatment and work impact: Include requested treatment, lost time, or work-status details. Check dates and descriptions against available records and the form’s scope.

New York’s C-3 service specifically asks for information such as employer details, job title and duties, gross pay rate, how and where the injury happened, the injury or illness and affected body parts, treatment, and lost time. That is a useful example of a form-specific checklist, not a universal list of fields.

Make the answers consistent without guessing

Review related answers together. For example, the incident date in one field should not conflict with the date in the narrative; the body part selected should agree with the written description; and the stated work absence should make sense alongside the dates provided. If the form requests a narrative, explain what happened, where and when it happened, what was affected, and what followed, using the detail requested. Avoid speculation or adding conclusions you cannot substantiate.

If information is unknown, consult that form’s instructions or the claim administrator rather than inventing a value. The New York C-3 guidance says to complete as many fields as possible with as much detail as possible. Separately, New York’s employer C-2F instructions explicitly allow the Board case-number and claim-administrator claim-number fields to be blank when those numbers are unknown; that permission applies to those identifier fields, not automatically to other fields or forms.

Confirm signatures, submission route, and proof

  1. Review the signature and date requirements. Sign and date the section assigned to you, and check whether another party must complete or sign a separate section. Do not sign for an employer or provider.
  2. Follow the official filing route. Check the agency’s current instructions for where and how the form must be submitted. Do not assume that mailing it to an employer, insurer, or agency is interchangeable.
  3. Keep a complete copy. Save or photocopy the completed form and any supporting material you submit. Keep evidence showing when and how it was sent or filed.
  4. Verify receipt or follow-up. California advises workers mailing the claim form to the employer to use certified mail with return receipt requested, and says to request a copy of the employer-completed form if one is not received. This is California guidance, not a universal mailing rule.

Check local deadlines separately

A complete form does not replace any required notice, filing deadline, or follow-up report. Deadlines depend on jurisdiction and circumstances, so verify them with the responsible agency rather than borrowing a rule from another state or program.

  • California: The DWC says the employer must provide or mail a claim form within one working day after learning of an injury or illness. This is California-specific information from the agency page marked June 2025.
  • Arizona: The Commission says the medical provider must file its report within eight days after first rendering treatment. Its guidance describes a one-year written claim filing period, with accrual tied to when the injury becomes manifest or the worker knows, or reasonably should know, of a compensable work-related injury. These are Arizona rules, not general deadlines.
  • New York: The state says workers must notify the employer within 30 days and file a C-3 with the Board. See the state’s current filing guidance.
  • Longshore claims: The LS-203 instructions describe different time rules for covered circumstances, including injury, hearing loss, and occupational disease; they include a one-year period in several situations and a two-year period for certain occupational-disease claims. Confirm which rule applies in the current OWCP instructions.

These examples do not establish a single nationwide deadline or filing process. Check the responsible authority’s current instructions for your location and claim type.
