If chatbot conversations repeatedly leave you more distressed, isolated, dependent, or drawn toward risky behavior, pause or cut back and talk with a licensed mental health professional or someone you trust. Those changes are reasons to check in—not proof that the chatbot caused a mental health condition. A chatbot can sound warm or sure of itself while still being inaccurate, missing important context, or reinforcing what you already believe.
Warning signs to notice
Pay attention to what happens to your feelings and daily life over time, not just whether one reply feels comforting. These are prompts to pause and check in, not a scored test or a diagnosis. A change can have more than one explanation, and a qualified professional can help assess the broader context.
Your mood or behavior shifts after chats
Notice whether conversations leave you agitated, distressed, or behaving differently—and whether that effect persists after you stop chatting. A single unpleasant exchange does not establish a pattern, but repeated changes are worth discussing with someone you trust or a clinician.

Chatting crowds out everyday life
Consider whether sessions are interfering with sleep, hobbies, school, work, or time with other people. The American Psychological Association (APA) advises watching for those effects rather than treating chatbot use as harmless simply because it feels supportive in the moment. Read the APA’s chatbot health advisory.

The chatbot becomes your main source of support
Be alert if you increasingly treat a general-purpose chatbot as your main emotional support, friend, romantic partner, or substitute for human relationships or professional care. The APA advises against relying on AI in those roles. Its advisory also describes social replacement as a pattern that warrants attention, particularly for people with relevant vulnerabilities.
You get stuck in reassurance or validation loops
If you keep asking the bot to reassure you or confirm a belief, and the cycle is becoming difficult to step away from, pause and consider discussing it with a professional. The APA advisory identifies reassurance- or validation-seeking as a pattern to monitor in relevant users; it is not, by itself, a diagnosis.
Rank #2
Replies reinforce risky behavior or beliefs
Take seriously any interaction that encourages or reinforces self-harm, substance misuse, disordered eating, aggression, or delusional beliefs. The APA advisory identifies these as examples of unsafe interactions. Do not treat a chatbot’s validation as evidence that a risky action or belief is safe or true.

Hearing or seeing things others do not, holding strong distressing beliefs others do not share, or having thoughts or urges about harming yourself or someone else calls for human help—not more chatbot advice. These concerns need assessment by a qualified person; a checklist cannot determine what is happening.

What warning signs can—and cannot—tell you
These patterns are worth acting on, but they do not prove that chatbot use caused a change or that you have a particular condition. The APA’s professional guidance describes risks to monitor; it does not provide a validated self-assessment that can determine whether a chatbot is making an individual’s mental health worse. A clinician can consider your wider circumstances and help distinguish among possible explanations.
The guidance cited here concerns general-purpose generative AI chatbots and wellness applications. It should not automatically be applied to regulated digital therapeutics, provider administrative tools, clinical decision-support systems, wearables, or traditional telehealth, which are different categories.

What to do if chatbot use seems harmful
- Pause or reduce use. If conversations repeatedly leave you more distressed, dependent, isolated, or pulled toward risky behavior, take a break or limit use. You do not need to settle the cause before taking a cautious step.
- Note what changed and when. Write down relevant changes in mood, sleep, daily functioning, relationships, and the kinds of chatbot responses that seemed to affect you. This can help you explain the pattern; it is not a diagnostic test.
- Tell a licensed mental health professional or health care team. Be open about using AI for support and what you have noticed. The APA recommends telling clinicians about AI use so they can relate it to your treatment goals.
- Do not use chatbot output to make clinical decisions. Do not rely on it to diagnose you, interpret psychological tests, choose treatment, or decide whether a crisis is serious. The APA says AI is not an accurate way to diagnose mental health conditions or a safe, effective replacement for licensed professional care. The American Psychiatric Association likewise cautions against treating generative AI as a substitute for care from a qualified professional. Read the APA’s statement on generative AI chatbots.
Get immediate human help for danger or crisis
If you may harm yourself or another person, have urges to self-harm, are experiencing hallucinations, or have strong distressing beliefs, connect with a person and seek immediate help. In the United States, the APA lists calling or texting 988, calling 911, or going to the nearest emergency department. If you are elsewhere, contact your local crisis line or emergency service; 988 is not a worldwide service.
In a 20 March 2026 update, WHO Director of the Department of Data, Digital Health, Analytics and AI Dr Alain Labrique said: “As AI increasingly interacts with people in moments of emotional vulnerability, we as WHO and its stakeholders must ensure these systems are designed and governed with safety, accountability and human well-being at their core.” Read the WHO update.
