Choose a calm, private moment; describe a specific change you have noticed; then ask how your family member is doing and listen without diagnosing or arguing. If they are open to help, offer one practical next step they can choose. If they are not ready, keep the conversation open rather than trying to force a decision.

How to start the conversation

  1. Choose a low-pressure moment. Talk somewhere private, with few distractions, when neither of you is rushed or already upset. Check your own feelings first. If your family member does not want to talk, do not force a long conversation.
  2. Describe what you have noticed. Stick to concrete changes rather than labels or accusations. For example: “I noticed you have been sleeping much less lately. How have you been doing?” This is an adapted example, not a quotation.
  3. Ask, then listen. Give them room to explain what is happening and what matters to them. Reflect what you hear and acknowledge their distress. You can validate how someone feels without agreeing with a belief you cannot verify; NAMI recommends focusing on emotional experience and the impact of symptoms rather than debating a particular belief. NAMI’s guidance on a caring conversation was last updated July 17, 2026.
  4. Ask what kind of support would help. You might ask, “Would you consider talking with a doctor or mental health professional?” Keep it an invitation, not an ultimatum.
  5. Offer one manageable next step. Ask whether they would like help finding options, calling their insurer, making an appointment, arranging transport, or having you accompany them. Let them choose what feels useful.

How to respond if they do not want help

You can express concern and offer support, but you cannot make another person accept care through one conversation. Avoid turning the discussion into an argument or repeating the same demand. You might say, “I care about you. I’m here if you want to talk or look at options together.” Give them space, keep the door open, and consider revisiting the subject later if circumstances allow.

NAMI describes LEAP—Listen, Empathize, Agree, and Partner—as an approach for communicating with someone experiencing mental health challenges. Its emphasis on listening and partnering around the person’s own motivations can support voluntary participation in care; it is not a guarantee that someone will agree to treatment. Choosing a useful next point of contact

The best starting point depends on what the person wants, how urgent the situation is, what is available and affordable, and what kind of support fits. Ask about their preference rather than ranking services for them.

  • PROMOTES EMOTIONAL GROWTH IN EVERY FAMILY, EVERY SEASON. Whether it is a new baby, a foster or blended family, a new marriage, moving away from home, empty nesters, or a loss, these cards help everyone explore their emotions, build emotional intelligence, and grow closer through the change. A thoughtful icebreaker game and meaningful gift for parents, teachers, and therapists, expertly crafted for sensitive or traumatic situations.
  • Primary care clinician: A familiar starting point for discussing changes in mood, sleep, or daily functioning and asking about possible next steps.
  • Therapist or other mental health professional: An option if the person wants to speak directly with someone who provides mental health care. Availability, cost, and insurance coverage can affect the choice.
  • School counselor or social worker: A possible starting point for a young person who needs help through school.
  • Crisis service: Use a crisis line or emergency service when the situation calls for urgent support, not as a substitute for routine appointment planning.

If you are a young person asking a parent or guardian

Plan a few specific examples of what has been happening and how it affects your day-to-day life. Choose a private time and make a clear request, such as asking to speak with a school counselor, social worker, pediatrician, or another health professional. NAMI’s guidance for talking with a parent or guardian suggests these kinds of starting points.

If a parent or guardian is not supportive, consider telling another trusted adult, such as a school staff member, coach, faith leader, or another safe person. Rules about minors’ independent access to mental health treatment vary by location, so seek local guidance rather than assuming one rule applies everywhere.

When the situation may be urgent

If someone may be at immediate risk of harming themselves or another person, treat it as urgent. In the United States and its territories, the 988 Suicide & Crisis Lifeline offers free, confidential support by call, text, and chat; a person does not have to be suicidal to contact it. Its guidance for helping someone else explains ways to support a person in crisis. Call 911 in the U.S. for immediate danger or urgent medical attention. If you are elsewhere, contact your local crisis line or emergency service. Do not promise absolute confidentiality when someone’s immediate safety is at stake.
