Get current written evidence of your homeowners insurance from your insurer or agent, then submit it using the insurance-document channel specified by your mortgage servicer. A declarations page is a common starting point, but your loan requirements and servicer determine what evidence is acceptable. Keep a copy and proof of submission, and follow up if the account still shows a coverage problem.
What counts as proof of homeowners insurance?
Ask your insurer or agent for current written evidence of coverage. Depending on the loan and servicer, that may be a declarations page, an insurance certificate, a copy of the policy, or similar written confirmation. The CFPB’s Regulation X recognizes these forms of evidence in connection with continuous hazard coverage, while Fannie Mae’s evidence requirements allow a signed certificate instead of the full policy when it contains the information needed to assess compliance.
Before submitting, check that the document is legible and current, and that it identifies the insured borrower and property, policy period, and relevant policy and coverage details. Coverage limits and deductibles may also matter. These are practical checks, not a universal lender form: the servicer evaluates the evidence against the requirements for your loan.
Check the mortgagee clause
The mortgagee clause identifies the mortgage lender or servicer’s interest in the insured property. If it is missing or incorrect, contact your insurer or agent and request a corrected policy record and updated proof before submitting. Ask your own servicer for its exact wording; do not copy a clause from another lender’s website.

Fannie Mae’s mortgagee-clause guidance generally calls for the lender or servicer and its address to be named as mortgagee, often with “its successors and/or assigns,” but it also addresses particular circumstances. Those requirements apply within Fannie Mae’s scope and should not be assumed to govern every mortgage. For example, Flagstar’s instructions tell its customers to include its mortgagee clause and provide their loan number and current contact details when reporting a policy change.
Submit the proof through your servicer
- Verify the route. Sign in to your servicer’s website or app using a link from a trusted mortgage statement, or verify the instructions in a current notice by contacting the servicer through a known, official channel. Your mortgage servicer may handle insurance documents even if a different company owns the loan.
- Find the insurance-document instructions. Check whether your servicer accepts an account upload, email, fax, or mail for insurance evidence. Follow its file-format and identification requirements; do not use contact details published for another servicer.
- Send the requested document. Submit the declarations page, certificate, policy, or other written evidence requested. Include your loan number if instructed. Save the exact file you sent and any receipt, confirmation number, sent email, fax confirmation, or mailing record.
- Check the account and follow up. If the insurance record does not update or a notice continues, contact the servicer using a verified channel. Ask what specific item remains unresolved and how to provide it.
Servicers’ procedures differ. STCU’s published instructions list email attachments in PDF, PNG, TIF, or JPG, as well as fax, mail, and an online option tied to certain notices. HomeLoanServ’s help center gives separate fax and email instructions and specifies PDF or TIF. Flagstar lists an upload route and email or mail alternatives. These are examples of servicer-specific methods, not universal contacts or accepted formats.

If your servicer says it has no proof
Respond promptly to a missing-coverage notice through a verified channel. Ask whether the problem is that evidence is missing, the mortgagee clause is wrong, the policy dates leave a gap, the coverage does not meet the loan terms, or another detail needs correction. If something is inaccurate, ask your insurer or agent to issue corrected evidence and send that updated document.
Sending a document does not itself establish that the servicer has accepted it. Under Regulation X, a servicer may seek written evidence of continuous hazard coverage and may reject borrower-submitted evidence if the insurer or agent does not confirm it or if the policy does not meet the mortgage contract. The rule is also relevant to notices about force-placed insurance. Do not assume an upload immediately stops charges or that every declarations page must be accepted regardless of the policy terms.
