A bill for care you never received, an unfamiliar diagnosis in your medical file, or a legitimate claim denied because your plan says you have used up your benefits can be a warning sign of medical identity theft. None proves by itself who caused the error. Check the records, dispute inaccurate information with the provider, report the issue to your insurer, and handle any fraudulent credit-report entry separately.
Warning signs worth checking
Look for care or coverage activity that does not match your own records. The Federal Trade Commission (FTC) lists these as possible warning signs:
- A provider bills you for services you did not receive.
- Your plan rejects a legitimate claim because its records say you have reached a benefits limit.
- Your plan will not cover care because your file lists a condition you do not have.
- Your medical record contains unfamiliar treatments, procedures, diagnoses, medications, charges, or claims.
These signs justify investigating and correcting records; they do not establish that identity theft occurred or identify who may be responsible. See the FTC’s warning signs of identity theft.

- Review relevant patient portals. Check the portals for hospitals, clinics, doctors, and laboratories where you have received care or where a related claim might appear. Download available records and claim details.
- Request records that are missing. If the portal does not show the information you need, follow the provider’s records-request process or contact the provider directly. The FTC notes that some requests may involve fees.
- Compare the entries with care you actually received. Check services, procedures, diagnoses, medications, charges, and claims. Note what is unfamiliar and the date or document where it appears.
- Check the coverage decision too. An exhausted-benefits denial or a refusal based on a condition you do not have may point to incorrect information even if you have not found an unfamiliar provider bill.
Federal law gives you the right to know what is in your medical files, according to the FTC’s medical identity theft recovery guidance.

Dispute inaccurate information with the provider
Contact the provider that holds the incorrect record. You can write or use its patient portal if available. Make the correction request specific: identify the entry, explain why it is wrong, and say what correction you are seeking.
- Include or point to the record containing the error and any documents that support your account.
- If available, include a copy of your FTC Identity Theft Report.
- Ask the provider to notify other providers that may have received or hold the same mistaken information.
- Keep a copy of your request and any attachments. If you mail it, the FTC recommends certified mail with a return receipt.
If a provider does not make records available after you submit a written request within 30 days, the FTC advises contacting the U.S. Department of Health and Human Services. For help obtaining records, you can also contact the provider’s privacy officer, patient representative, or ombudsman. The FTC lists these steps and related guidance on its recovery steps page.

Handle credit-report entries separately
If medical billing errors appear on your credit report, use the FTC’s identity-theft steps to notify all three credit bureaus and provide the requested identity-theft documentation. The FTC also explains that identity-theft victims may dispute or block fraudulent information and place a free credit freeze; follow the site’s current instructions on identity-theft rights.
A credit-report dispute addresses information on a credit report. It does not, by itself, correct a provider’s medical record or resolve an insurer’s claim decision. Pursue those issues with the provider and insurer as well.

Keep a dated log of calls and contacts, along with copies of medical records, claim documents, letters, emails, reports, and responses. This gives you a clear record of what you reported and what each organization said or did.

Follow each organization’s current process
The FTC provides a general recovery path, not a guarantee that a provider will make a particular correction or an insurer will change a claim decision. Deadlines and procedures can depend on your state and plan. Check the original claim notice and plan documents, and ask the provider or insurer for its current dispute process.
