To set up health tracking on a Pixel Watch, pair it with a compatible Android phone, connect the Google Health app, and review the heart-health options available for your watch and region. Google documents all-day high- and low-heart-rate alerts, but its published setup guidance does not give a universal menu path or alert thresholds; check the heart-health or Health Guardian area in your own app rather than relying on assumed labels.
Before you start
- A Google Account, internet access, the Google Pixel Watch app, and an Android phone updated to a version compatible with your watch.
- Bluetooth enabled and location permission allowed on the phone.
- Both devices charged, with the watch and apps updated.
Google’s Pixel Watch setup instructions cover the current pairing requirements and process. Feature availability can vary by watch generation, software and app version, and country.
Pair the watch and connect Google Health
- Install or open the Google Pixel Watch app on your phone. Turn on Bluetooth and allow the requested location permission.
- Place the watch near the phone and follow the app’s pairing instructions. Confirm that the code shown on the watch matches the code on the phone before accepting.
- Sign in and complete the setup prompts. Select the wrist you wear the watch on and the correct crown orientation.
- During setup, choose Set up more and then Google Health, or connect the Google Health mobile app during setup or afterward. Follow the prompts in the app.
- Fit the band comfortably. For sleep tracking, Google says the watch needs at least 20% battery; charge it in the morning or before winding down so it has power for Bedtime mode.
Google’s Google Health setup guidance describes connecting the mobile app and preparing the watch for health tracking.
What health metrics and alerts are available?
Google says Pixel Watch can track steps, calories, and GPS data during activities, and can automatically detect and save activities. Activity features and available modes vary by watch and region. Heart-rate features include resting heart rate and Active Zone Minutes, as well as notifications for periods of abnormally high or low heart rate throughout the day.
#1 Best Overall
- The new Google Pixel Watch 5 offers a smarter way to a healthier you.[1,2]; Designed for Gemini Intelligence, get proactive help, and personalized insights from Google Health[3,4]; watch is compatible with Android devices only; not compatible with iOS devices
- Precision-crafted to do it all: The Google Pixel Watch 5 features the domed Actua 360 display with scratch-resistant Gorilla Glass and up to 40 hours of battery life[1,5]
- Metrics that matter: Advanced sensors help you stay ahead of your health and wellness; and Google Pixel’s most accurate heart rate tracking is fine-tuned for your activity[6,7]
- Understand your sleep like never before: Your watch captures insights like the time spent in sleep stages to breathing regularity, then summarizes it all in a personalized Sleep Score[8,9]
- Detects loss of pulse, then calls for help: Pixel Watch 5 is the only smartwatch to feature Loss of Pulse Detection, which can prompt a call to emergency services[10]
Sleep and overnight measurements
- Pixel Watch 2, 3, 4, and 5 have a sleeping skin-temperature sensor. Google says the watch establishes a baseline over three nights and then reports average deviation from that baseline.
- Pixel Watch 2 and 3 can report overnight SpO2 after a sleep period longer than three hours, subject to regional availability.
- Sleep tracking requires at least 20% battery.
Health Guardian, irregular-rhythm notifications, and ECG
Google lists Health Guardian for Pixel Watch 3, 4, and 5 with Google Health app version 5.09 or later. The listed tools include Irregular Rhythm Notifications, ECG, and Heart Rate Notifications, with some features limited by model or region. The Google Health Guardian feature information should be checked alongside what appears in your app, since availability is not universal.
High/low heart-rate alerts and irregular-rhythm notifications are different features. Google describes irregular-rhythm notifications as opportunistic: not receiving a notification does not establish that you do not have a heart condition. ECG analyzes heart-rhythm data for signs of atrial fibrillation (AFib), but it cannot diagnose AFib; only a doctor can. ECG is available only in select countries and is not intended for people under 22. See Google’s ECG guidance for eligibility and limitations.

Turn on or review heart alerts
- Open the Google Health app on your phone and confirm it is connected to your Pixel Watch.
- Inspect the app’s heart-health or Health Guardian area for heart-rate notifications and other features shown for your watch.
- Review the options actually presented in the app and follow its prompts. Google’s published guidance confirms high/low heart-rate alerts but does not establish one menu path or universal threshold for all users, so avoid assuming a setting name or value that is not shown on your device.
If an option is missing, first check the watch model, Google Health app version, software updates, and country availability. The feature list for Pixel Watch 3, 4, and 5 specifically requires Google Health app version 5.09 or higher for Health Guardian.

Google describes SpO2, skin temperature, body-response information, heart-rate notifications, and Health Guardian trends as wellness or informational features, not as diagnosis or treatment. An ECG result may help identify signs that merit discussion, but it is not a medical diagnosis. Contact a clinician about symptoms or health concerns, and ask one about possible interference if you use an implanted medical device.
