Use PHP only when the page language varies: select one supported language tag, then output it in the document’s single opening <html> element.
What the HTML lang attribute does
The lang global attribute declares the language of an element’s content. Its value is a single BCP 47 language tag, such as en, fr, or en-GB. It is language metadata; it does not translate text, choose translated content, or hide other language versions. See MDN’s documentation for the HTML lang attribute.
When an element has no lang or xml:lang attribute, its language is inherited from its parent. An empty value means the language is unknown, while a value that is not valid under BCP 47 is invalid.
Use a fixed value when the document language is fixed
If every version of the page is written in English, no PHP condition is needed:

<html lang="en">Choose the appropriate tag for the language the document uses. PHP is useful only if the application’s language selection changes the page language.
Set the attribute dynamically in PHP
When a route, locale setting, or translation system determines the page language, have the application select a supported tag and output it in the opening <html> tag. For example:

<html lang="<?= htmlspecialchars($language, ENT_QUOTES, 'UTF-8') ?>">The selection should come from the site’s configured language options, not an unchecked value copied directly into the document. Escaping the value for the HTML attribute context helps prevent unsafe output; the application must also ensure it selects an appropriate language tag.
Use PHP’s alternative if syntax in a template
If you want to write separate markup for each case, PHP’s alternative syntax lets HTML sit between the condition and its closing marker. Put a colon after if, use elseif and else as needed, and close the block with endif;:

<?php endif; ?>This illustrates the template syntax, but it emits a separate opening <html> tag for every branch. Usually, it is clearer to choose the language first and emit one document root with a dynamic lang value, as in the previous example. The PHP manual documents the alternative syntax and its elseif, else, and endif forms in Alternative syntax for control structures.
Do not mix the two PHP syntaxes in one control block
PHP supports both brace syntax and the template-friendly alternative syntax, but do not mix them at the same level of a control block. With alternative syntax, close the condition using endif;; with brace syntax, use braces throughout that block.

Keep language selection separate from translation
The lang attribute describes the language of the content PHP has rendered. Your application’s routing or localization logic decides which content to render; setting lang alone does not perform that work. Once the application has selected a language, output its corresponding tag on the document’s single opening <html> element.
