Start with the rejection notice: identify the state, exact form, filing party, and specific error, then correct the form using that state board’s current instructions. There is no single nationwide amendment form, deadline, signature rule, or resubmission route for workers’ compensation paperwork. Keep the notice and a copy of everything you send.

First, find out what happened to the form

A form can fail at different points, and the right fix depends on which one occurred. A portal validation message may mean the submission was not completed. A board rejection means the agency identified a defect. An insurer or carrier may object to information on a form. A document may also have been received but not matched to the correct case.

Save the full rejection message, letter, portal alert, or reference number. Note who issued it and whether it says the form was rejected, not received, or received but incomplete. If the notice does not explain the problem, contact the agency or filing support unit responsible for that form and provide its name, the rejection wording, and any receipt or reference number. Do not assume that contacting the agency pauses or extends a filing deadline.

Correct the form without guessing

  1. Match the notice to the form. Confirm the state, form number and revision, who is supposed to file it, who should receive it, and the submission route the form allows.
  2. Fix each item named in the notice. Check the claim or case identifier, injury date, claimant and employer details, insurer or claims-office information when required, mandatory sections, signatures and dates, legibility, and required pages or attachments.
  3. Resolve unknown or conflicting information with the responsible party. Do not invent a case number, injury date, or other detail to make a form pass validation. Ask the issuing board or claim administrator how to handle information that is unavailable or inconsistent.
  4. Check the saved file. Open the final PDF and confirm that typed entries are visible, every required page and attachment is present, and the document is readable. Use the current version of the form and do not alter it in a way the board disallows.
  5. Resubmit using an accepted route. Follow the instructions for that exact form and filer. Save the completed form and proof of submission, delivery, or portal receipt, and send copies to other parties if the form instructions require them.

Common defects—and why jurisdiction matters

Examples from state agencies show why a generic checklist cannot replace the rejection notice or the current form instructions.

Jurisdiction and example Documented issue Practical response
Georgia The State Board lists missing Board Claim Numbers, incomplete sections on WC-1, missing or incorrect five-digit SBWC IDs, missing insurer or claims-office details, illegibility, altered or outdated forms, multiple injury dates on one form, and missing claimant or employer addresses among rejection reasons. It also says WC-1 and WC-14 must be typed or printed in black ink. New York The Board warns that missing case identifiers can leave a document unmatched to a case. Its March 30, 2026 bulletin says to put the WCB Case ID and accident date in the upper-right area of every page; if no WCB Case ID is available, use the claimant’s name and accident date. Follow the New York bulletin’s identifier placement for the documents it covers, so the identifiers remain visible in the resulting image. New York Board bulletin
New York PDF and web filing The Board warns that form data may not transmit if the completed form is not printed to PDF. For its web-submission process, a returned completed PDF is the success signal; the Board says that without a confirmation PDF it did not receive the form. Check the saved PDF for visible data. After web filing, save the returned confirmation PDF; if it does not appear, follow the Board’s direction to resubmit. Indiana Indiana says forms marked with an asterisk must be submitted electronically through an approved EDI 3.1 process; hard-copy forms sent before electronic submission will be rejected. Check whether the exact form is marked with an asterisk and follow the Board’s EDI instructions rather than mailing paper first. Indiana WCB forms

Verify the version and submission channel before resending

A form that was accepted previously may no longer be current, and an old filing method may no longer be permitted. New York’s forms pages identify cases where only the current version is accepted and warn that older versions may stop being accepted. Georgia likewise includes outdated forms among its rejection reasons. Check the exact form’s current page before sending a correction or replacement.

Submission rules can also change by form and filing unit. New York says that, effective April 1, 2026, faxed claim-related forms and documents are no longer accepted; its bulletin lists mail, email, electronic upload, web filing, and XML routes, with exceptions for certain non-claims units. Indiana’s asterisk-marked forms instead require an approved EDI 3.1 submission. These are state-specific instructions, not interchangeable options. Use only a route currently permitted for your form and filer.

New York’s claimant forms page also gives instructions about submitting two-sided or multi-page forms in the specified format and not sending instruction-only pages to the Board. Check the instructions attached to your own form for page, attachment, and copy requirements.

State examples: follow the process for your form

California injured worker filing a claim

California DWC says the injured worker completes only the employee section, signs and dates the claim form, and sends it promptly to the employer, keeping a copy. If mailing it, DWC recommends certified mail with return receipt requested to document mailing and receipt. Massachusetts Form 110 claim

Massachusetts instructs claimants to send the employee claim and supporting documents to the employer’s workers’ compensation carrier by certified mail. The state says a rejection letter is attached to the form and states what information is missing or incorrect. This is the Massachusetts Form 110 process, not a general mailing rule. Georgia correction to a previously filed WC-14

Georgia’s WC-14a is titled “Request to Change Information on a Previously Filed Form WC-14.” Its title does not establish that it amends other forms or is the right remedy for an initially rejected filing. Confirm the correct correction process with the Georgia board for the form and problem at issue. Georgia WC-14a

New York and Indiana filing routes

New York directs filers to the specific form page and its web-submission instructions for current forms, routes, and party-copy requirements. Indiana’s EDI rule applies to forms marked with an asterisk. Neither state’s process should be assumed to apply elsewhere. Keep a record of the corrected submission

  • Retain the rejection notice and the corrected form, including all pages and attachments.
  • Keep the confirmation, delivery record, tracking information, or other receipt the accepted filing route provides.
  • Record when and where you submitted it, and keep evidence that required copies were sent to other parties.
  • If the same defect recurs or no reason is given, contact the responsible board or filing support unit with the form name and rejection or receipt details.

For New York web submissions specifically, the Board says it returns a completed PDF when submission succeeds and that some forms require copies to the claimant, employer, carrier, or representative. Use the individual form’s instructions to determine whether additional copies are required.
