Prioritize vulnerabilities with evidence of active exploitation first, then raise urgency for systems that are exposed or support critical business and mission functions. Use CVSS to understand technical severity and EPSS to gauge estimated near-term exploitation likelihood; neither replaces checking whether the vulnerable asset exists in your environment, is reachable, or matters to your organization.
Should you patch the highest CVSS score first?
Not automatically. CVSS and EPSS answer different questions: CVSS provides a standardized way to describe vulnerability severity, while the Forum of Incident Response and Security Teams (FIRST) EPSS estimates the probability that a published CVE will be exploited in the wild in the next 30 days. FIRST publishes EPSS probabilities on a 0–1 scale and ranking percentiles daily; these are model outputs, not observed counts of attacks.
A high severity score matters, but a lower-scored vulnerability with confirmed exploitation on an internet-facing system that supports a critical service may deserve attention first. Conversely, a vulnerability that does not affect any asset in your environment is not a patching task for that environment. Treat the scores as inputs to a local decision, not as a universal queue.
How to decide what to patch first

Confirm the affected software and asset
Match the vulnerability record to installed software, version, and asset inventory. Check whether the scanner finding is validated and whether the affected component is actually present and in use. NIST SP 800-40 Rev. 4 describes enterprise patch management as identifying, prioritizing, acquiring, installing, and verifying patches, updates, and upgrades throughout an organization.

Check for evidence of active exploitation
Check the current CISA Known Exploited Vulnerabilities (KEV) Catalog and relevant vendor advisories. CISA describes KEV as a living catalog of vulnerabilities with evidence of active exploitation. Treat a confirmed exploitation signal as a strong reason to accelerate remediation, rather than waiting for a high CVSS score to make the decision.
CISA’s Binding Operational Directive 22-01 sets remediation due dates for Federal Civilian Executive Branch agencies. CISA also urges other organizations to prioritize KEV entries, but that recommendation does not make the directive’s deadlines binding on every organization.

Assess exposure and business importance
Determine whether the affected system is internet-facing, reachable through a high-risk path, or otherwise accessible to likely attackers. Then consider what depends on it: critical services, safety, sensitive data, or important business operations. CISA’s Cross-Sector Cybersecurity Performance Goals call for known exploited vulnerabilities on internet-facing systems to be patched or otherwise mitigated within a risk-informed span of time, with more critical assets prioritized first. That is not a single fixed deadline for every organization or vulnerability.

Use severity and likelihood as distinct signals
Record the CVSS assessment for technical severity and the current EPSS estimate and percentile as evidence about estimated near-term exploitation likelihood. Neither tells you by itself whether your specific asset is exposed, affected, or business-critical. EPSS is an estimate for a published CVE over the next 30 days, not a prediction that a particular system will be attacked.

Choose a patch or supported mitigation
When a suitable patch is available and can be deployed safely, acquire and install it using your normal change and maintenance controls. If immediate patching is not practical, use a supported mitigation where available, document the decision and accountable owner, and set a review point. Do not treat mitigation as proof that the vulnerability has been permanently removed.

Verify remediation and revisit the decision
Confirm that the patch or mitigation is actually in place and that the vulnerable condition is no longer present. A deployment ticket marked complete is not verification on its own. Recheck KEV status, vendor instructions, asset exposure, and EPSS as circumstances change; FIRST publishes EPSS daily.

Compare vulnerabilities using the same decision factors
Use a consistent record for each candidate so the team can explain why one item moves ahead of another. This comparison is a practical synthesis of CISA, NIST, and FIRST guidance, not a scoring formula published by any one of them.

Set remediation windows without inventing a universal clock
The sources establish a prioritization method, not a promise that every organization can patch in a fixed number of hours or days. Set remediation windows in light of applicable directives, vendor instructions, exposure, operational constraints, and your organization’s risk tolerance. Apply binding due dates where they actually govern your organization; otherwise, use documented risk-based targets and escalate when an exposed, critical asset or active exploitation warrants faster action.

NIST published SP 800-40 Rev. 4 on April 6, 2022. Its patch-management lifecycle is a useful operational check: identify, prioritize, acquire, install, and verify. A ranking that never leads to a completed and verified action does not reduce the underlying risk.
